COWBRIDGE HEALTH CENTRE is purpose built with access for the disabled in the form of ramps to the Health Centre grounds and the Health Centre building, which has adequate doorways and toilet facilities.

THE PARTNERS This is a small group practice with 5 partners as described below. The practice was initially founded in 1881 by a single handed GP, Dr. Charles Booth Mellor, who visited in a horse and trap and performed minor operations in his house.

Community Psychiatric Nurses Make home visits on psychiatric patients and the elderly confused under psychiatric care.

District Midwives See patients at ante-natal clinic on Tuesday afternoons and attend patients at home after delivery.

Health Visitors – Tel: 01446 776033 Mrs. Tracey Davies Mrs. Sian Williams The Health Visitors are primarily responsible for the care of young children. They visit mothers and babies at home and are available for advice to mothers before confinement and for children up to school entry. With the help of a community paediatrician they run a Well-Baby Clinic. Thursday mornings: 9.30 am – 12.00 midday

Physiotherapist Mrs. Yasmin Ford Available for referrals from a GP Monday and Friday: 8.00 am – 4.30 pm

Chiropodist/Podiatrist Available for referrals from a GP and self-referrals by pensioners and certain at-risk groups.

HOW THE PRACTICE WORKS

Consultation by Appointment Monday – Friday: 8.30 am – 11.30 am and 5.00 pm – 6.30 pm (on certain days appointments start at 8.00 am) Tuesday: 1.00 pm – 3.00 pm Any patients requesting an urgent consultation will be seen that day after booked appointments or possibly earlier by the doctor on call for home visits.

To Make an Appointment Please call at the surgery or telephone Cowbridge 772237 between 8.00 am – 6.30 pm The telephone is less busy in the afternoon.

To arrange a Home Visit Please telephone Cowbridge 772237 if possible between 8.00 am – 10.30 am. Telephoning before 10.30 am will help the Doctor plan visits. You may be asked for information about the illness as this will assist the Doctor to assess the urgency of the call. To request an urgent visit or advice outside normal working hours: If you require a doctor outside of our opening hours, please ring the surgery on the normal number (01446 772237) and you will automatically be put through to the out of hours service provider "Primecare". NHS Direct also deals with enquiries on 0845 4647

Closure for training Every two months the surgery is closed on one afternoon for training. We put notices around the surgery to advise patients of the date for approximately two weeks prior to the closure. We hope that this does not inconvenience patients as this will assist in your continued care. During these closures, Primecare will deal with calls. 34501 CHC Info Booklet 25/4/06 12:13 pm Page 7

To request a repeat prescription Please request prescriptions 48 hours in advance, in person, by post or by telephone. Telephone requests are dealt with after 10.30 am. The telephones are less busy in the afternoons. A post box is available at the Health Centre, at the reception desk, for repeat prescription requests. Repeat prescription forms are issued by the computer and should be submitted with the request if possible. These forms can then be reviewed periodically by the Doctor.

Other Clinics and Services All partners are available during normal surgery hours for cervical smear tests, ante-natal care and contraceptive advice. The Practice Nurses are also available for cervical smear tests.

Childhood Immunisations Thursday 9.00 am – 10.30 am by appointment only.

Child Health Surveillance Services These are undertaken on alternate Thursday mornings with a Paediatrician and our Health Visitor and are by appointment only. 34501 CHC Info Booklet 25/4/06 12:13 pm Page 8

HEALTH PROMOTION

The Practice carries out Health Promotion programmes to reduce the risks of coronary artery disease and to promote teenage health. We also undertake chronic disease management of asthma and diabetes. Health promotion is undertaken during routine surgery visits and also during Practice Nurse run clinics. Chronic Disease Management takes place at the Diabetic and Asthma clinics, run with the G.P.’s. Additional asthma monitoring appointmnts with Sister Boult are available on Mondays, Thursdays and Fridays.

Asthma Clinic Alternate Mondays 5.00 pm – 6.30 pm

Well Woman Clinic Alternate Mondays 2.00 pm – 4.00 pm For cervical smears, contraception, hormone replacement therapy, antenatal and postnatal checks with a female doctor (Dr. O’Hanlon).

Minor Surgery This is undertaken at the practice by Dr. Jemmett, Dr. Stuart and Dr. Arnold during normal surgery hours with an additional dedicated clinic run on some Friday afternoons by appointment.

Confidentiality and Test Results All medical results, be they paper or computer, are dealt with in the strictest confidence. To maintain this confidentiality, results of investigations will only be given to the patients themselves, or the parents of a child. Appropriate identification may be requested. If possible please contact the Surgery for results in the afternoons after 2.30 pm Any person given access to your record will be subject to a duty of confidentiality, you have a right to object to disclosure. 34501 CHC Info Booklet 25/4/06 12:13 pm Page 9

Referrals When referring you to other health care workers, the doctors will make available to them details of your medical history. The doctor will respect the wishes of any patients who object to particular information being shared with others providing care, except where this would put others at risk of serious harm.

RMC Information In the majority of cases we are required to route our clinical referrals through the Referral Management Centre which is run from the Local Health Board. Should you have any objections to this it is important you inform us of this immediately. Once your referral has been processed you can now check the estimated waiting time in several ways. Either telephone NHS Direct on 0845 46 47 and they will be able to give you details (you will need to know the name of the hospital, the Consultant and the type of surgery you will be receiving before telephoning), you can check waiting times by visiting www.howis..nhs.uk., or by phoning the RMC on 029 2035 0630.

Research and Planning Information about patients is requested for a wide variety of purposes including education, research, monitoring, epidemiology, public health surveillance, clinical audit and planning. The practice will render data anonymous where possible. Only where it is essential for the purpose will identifiable records be disclosed. Such disclosures will be kept to the minimum necessary for the purpose. You have the right to object to any such disclosures and your objection will be respected.

Financial Audit The Local Health Board (LHB) has a duty to occasionally check on claims made by practices for payments. The person carrying out the check will have been properly trained and authorised by the LHB and will be subject to a duty of confidentiality in their employment contract or because of their registration with a statutory regulatory body. Objections If you have any objections to the disclosure of information for any of these purposes please tell your doctor.

Private Medical Examinations and Reports We are happy to undertake Private Medical Examinations, by appointment, and also complete other Private Medical Forms, eg from BUPA, Solicitors or Insurance Companies, etc. This work is, however, outside the NHS system and will attract a fee in accordance with BMA rates. Please check the charge when making an appointment for an examination or handing in a form for completion.

Registering with the Practice Patients wishing to register should bring their NHS card or complete the appropriate registration form available from reception. All new patients will be required to complete a questionnaire which gives us details of your past medical history and to provide appropriate care in the interim between requesting and receiving your notes from your previous GP.

Temporary Registrations You are able to see a doctor anywhere in the UK if you are away from home and in need of medical help. You can do this simply by asking to see a doctor as a temporary resident. We will be happy to see any relative or friend staying with you if they need a doctor.

Change of Address or Telephone Number If you change your address or telephone number, please inform a Receptionist as soon as possible. This will ensure that your medical record is updated should we need to contact you for any reason.

Finally May we point out that due to the nature of general practice, delays are sometimes unavoidable. Furthermore, there may be occasions when you are upset by ourselves, our staff or even the "System". If so please talk to us about it. Often the problem can be resolved and, where appropriate, the system improved. There is, however, an official complaints procedure which you may use if you wish. Please contact the Business Manager, Lindsey Richards, should you wish to do so.
