Lost Your Card? You should get a card when you get your COVID-19 that includes the name of the vaccine product and the date you got it. It is important that you get the same vaccine product for both doses.

If you lost your card BEFORE getting both doses, you should know: . It is OK if you lose your card. If you lose it between shots, ask for a new one when you get your second and ask them to record the first dose on it. . If you have not made an appointment for your second dose already and need the information about your first dose, you can ask for your vaccination record from the Minnesota Department of Health. To request your record, go to Find My Record (www.health.state.mn.us/people/immunize/miic/records.html). . Your provider can also look up your vaccination record to help determine when you should get your second dose. If you lose your vaccine card AFTER you are fully vaccinated, or end up with two vaccine cards, please note: . MDH does not have replacement cards and cannot combine two cards. . You can ask for a copy of your vaccination record at Find My Immunization Record (www.health.state.mn.us/people/immunize/miic/records.html). It has the same information as the vaccine card.

The State of Minnesota will not require proof of vaccination for events, such as traveling, concerts, or other activities.

Wear a mask. Wash your hands. Stay 6 feet from others. Stay home if you feel sick.
