FINAL REPORT 47 PART III THE INQUIRIES CONDUCTED AFTER THE TRAGEDY 1. THE ADMINISTRATIVE INQUIRY BY THE ITALIAN AIR FORCE 1.1 Introduction The Committee of Italian Air Force officers convened by the Command of the 1st Air Region on 4 February 1998 (known as a technical inquiry) drew up its report in accordance with STANAG 3531 on investigations of accidents/incidents involving military aircraft from two or more NATO nations. This Committee worked in conjunction with its US counterpart, from which it obtained information as required. It did not, however, obtain copies of the documentation consigned to the official records, although it had free access for consultation purposes. The Committee’s investigations covered its fields of interest, where compatible with the procedures of the Italian judicial investigation, taking due account of the fact that many of the objects, materials, and documents had been seized. The investigation was affected by the fact that the crew members had opted to exercise their right to remain silent under questioning about the event. The Committee therefore recognized that it could not obtain certain fundamental information from the parties directly involved, such as data on the planning and execution of the mission, and that it could not ascertain the exact dynamic of events or the conduct of the crew members during the mission. 1.2 Summary of the Air Force investigation report The report contains the essential data on the most significant parts of the mission denominated « EASY 01 »: from pre-flight briefing to take-off from Aviano, the impact with the cableway and the emergency landing.

The Association for Diplomatic Studies and Training Foreign Affairs Oral History Project ERIC TERZUOLO Interviewed by: Mark Tauber Initial interview date: December 14, 2017 Copyright 2019 ADST INTERVIEW Q: Today is December 14, 2017. Eric, where and when were you born? TERZUOLO: I was born in Los Angeles, California, 60-some years ago, and resided primarily in Los Angeles until I was about three years old, when my parents and I moved to Minneapolis, Minnesota. Q: What brought them to Minneapolis? TERZUOLO: I should note I am the child of immigrants to the United States. I actually remember vaguely when my parents were naturalized. My father was part of the continuing Italian brain drain. My parents emigrated as adults. They were accomplished, educated people. Let’s say I was part of a very privileged immigration, but nonetheless experienced a lot of the same sorts of issues that less fortunate immigrant children do in terms of acculturation and so forth. Q: Well then, before we go further, from where did your parents emigrate? TERZUOLO: From Italy. Q: Do you know anything about the home area they came from? TERZUOLO: Oh, yes. Actually, I know it well, because my parents were the only people from their respective families who left and came to the United States and, unlike a lot of immigrants, retained strong ties to home. I think that, for the first few years, they didn’t get back to Italy at all, but otherwise we would tend to go pretty much every year. I knew the rest of the rather small family on both sides.

AIM SG/3-IP/3 07/05/2017 International Civil Aviation Organization MIDANPIRG AIM Sub-Group Third Meeting (AIM SG/3) (Cairo, Egypt, 15 - 18 May 2017) Agenda Item 4: AIM Planning and Implementation in the MID Region AIM-RELATED ACCIDENTS AND INCIDENTS (Presented by the Secretariat) SUMMARY This information paper presents AIM-related contributory factors and recommendations related to some of accidents and incidents. Action by the meeting is at paragraph 3. REFERENCES - EUROCONTROL 1. INTRODUCTION 1.1 For the sake of safety promotion and continuing on the experience, this thread is created by the EUROCONTROL to share the incident/accident reports where the aeronautical data and/or aeronautical information products (e.g. AIP, charts, NOTAM, etc.) were considered among the findings, contributory factors and/or safety recommendations. 2. DISCUSSION 2.1 Some of AIM-related contributing factors and recommendations of accidents/incidents are as follows: A333, Kathmandu Nepal, 2015 Use of resolution and rounding in publication and calculation of runway threshold coordinates Use of AIRAC for publication/cancellation of AIP SUP Coordination between aeronautical information services and aerodrome authorities Provision of raw aeronautical information/data by the aerodrome authorities in accordance with the accuracy and integrity requirements SW4, Sanikiluaq Nunavut Canada, 2012 The lack of required flight documents, such as instrument approach charts, compromised thoroughness and placed pressure on the Captain to find a work- around solution during flight planning. It also negatively affected the crew’s situational awareness during the approaches at Sanikiluaq. AIM SG/3-IP/3 - 2 - CL60, Lexington KY USA, 2006 Because of an ongoing construction project, the taxiway identifiers represented in the airport chart available to the flight crew were inaccurate, and the information contained in a local NOTAM about the closure of taxiway was not made available to the crew via ATIS broadcast or the flight release paperwork.
