Primary Care Licensed Practical Nurse Full-Time, Variable Day Shifts

Clifton-Fine Hospital has an immediate opening for a Licensed Professional Nurse in our Primary Care Clinic. Clifton Fine’s Primary Care Clinic is a nationally recognized PCMH (Patient Centered Medical Home). The LPN is responsible for care under the supervision of and/or (RN) and within the appropriate scope of practice. The LPN performs the functions associated within the clinical aspect of a patient visit. Maintains responsibility for direct patient care, documentation, administration of medications and injections, minor laboratory testing, and communication with patients, providers, insurers, pharmacies, and fellow staff. Occasional Saturday weekend coverage is possible.

Job duties: Patient Care and Services: Promote and restore patient’s health using the , collaborate with physicians and multidisciplinary team members, perform various treatment procedures, clinic nurse visits, and provide physical, emotional, and mental support to patient’s and family members.

Medication Administration and Injections: Administers medications and treatments as directed and per scope of practice guidelines including injections (allergy, immunizations, and therapeutic). Reviews medications at time of visit as directed by provider. Maintains inventory of and order medications and medical supplies with attention to expiration dates and prevention guidelines.

Electronic Recordkeeping: Documents interventions, patient/family responses, medications dispensed/prescribed, and testing results in electronic medical records (EMR). Monitors and follows up on electronic messaging promptly and accurately with appropriate documentation.

Laboratory Testing: Collects specimens for laboratory analysis and accurate recording of lab requisitions. Performs Point of Care Testing; HCG’s, Rapid Strep, Urinalysis, Fecal Occult Stools. Records and maintains appropriate laboratory logs.

Other duties as assigned

Requirements: New York State Licensed Practice Nurse License, Current Basic Life Support Certification (American Heart Association) or successful completion upon hire. The ideal candidate would have a minimum of one year experience as a Licensed Practical Nurse. New graduates are encouraged to apply. One year of experience in a medical office practice or hospital preferred, demonstrated experience providing excellent customer service is required, demonstrated experience working as part of a dynamic team in a fast-past environment is required, knowledge of requirements regarding and experience working with confidential material (including medical records) is preferred.
