The Licensed Practical Nurse, LPN, provides continuity of care for the residents of the center, working with the staff and resources available during shift.

The LPN must have the ability to pass medications, do ordered treatments, document in computerized (will train in Point Click Care if needed), supervise CNAs, and converse with medical providers and families in a professional manner. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES AND STANDARDS  Demonstrates the ability to implement systems that address each residents needs. Having a broad scope of knowledge and experience to be able to address the needs of the medically complex as well as the mentally compromised residents.  Demonstrates the ability to assess the needs of the resident, make a diagnosis, transition this into an interdisciplinary plan of care utilizing all resources available. Then is able to communicate this plan to all care providers in a way that ensures continuity of care for resident.  Demonstrates the ability to establish and maintain functional communications with staff, other disciplines, residents, families, medical staff and community.  Demonstrates the knowledge to assess the effect of medications and treatments and monitor these effects to provide appropriate feed back to interdisciplinary team and medical managers to continuously provide the resident with the treatment appropriate to their individual needs.  Demonstrates the ability to provide education to residents regarding their healthcare issues. Provides ongoing education regarding basic nursing practices to nursing assistants under his/her supervision.  Demonstrates knowledge and ability to apply knowledge regarding control, fire and disaster policies as well as other facility and/or state and federal policies and practices that provides a safe environment for residents, community and staff. ..  Demonstrates knowledge of and is able to apply knowledge regarding the protection of resident rights, and the treatment of residents, staff and community with respect and dignity. Treating resident and facility property and information with honesty, confidentiality and respect.  Demonstrates the ability to document data in an accurate and timely way that maintains the facility records to meet facility, state and federal requirements.  Remains up to date on all required educational components and seeks skill growth independently.

EDUCATION Graduate from an accredited school of nursing.

EXPERIENCE At least 1 year working as a licensed practical nurse.

QUALIFICATIONS  A current unencumbered license to practice nursing in the state in which the center is located.  Demonstrates the ability to read, write, and speak English, as well as add, subtract, multiply and divide.  Proficiency in basic computer skills
