‘Palliative medicine’ is as old as medicine itself, though not by that name. The nurturing and comforting we associate with caring for a sick child or spouse; the anticipating and plan- ning parents and doctors invest in preparing for a birth; and the tenderness and honesty with which a hospice team tends to the dying – these familiar and timeless values embody ‘palliative care.’

The vast majority of modern medicine now seems devoid of comforting, honesty, anticipating, and planning. Crisis management and prevention of ‘organ failure’ have superseded caring for a whole, unique person.

The specialty of palliative medicine allows me to re-claim and practice those values that drew me to medical training. Palliative care attends to social, emotional, spiritual, as well as physical needs, in particular for those with serious or life-limiting illness.

I feel this movement has ‘saved’ us from the daily grind of modern medical practice. Palliative care delivers ‘personalized’ or ‘whole person’ care to people with the most serious and complex medical needs, including end of life. Support of this movement continues to grow through the generosity of philanthropy, as well as the foresight of the non-profit and scientific communities.

We owe the exponential growth and quality of palliative care services in Monterey County largely to Hospice Giving Foundation. From staffing to training to program development, the robust palliative care movement in our county would not exist as we know it without HG Foundation.

We have so much to learn. If we don’t get our brains around this, we are missing an incredible opportunity. Hospice care is the answer; it’s as close as we are going to get to the guidebook. We all need guidance. It’s a community way of thinking. These days we may not learn it from the elders in our family, but we can learn it. The sooner we wrap our heads around this conversation, the better. I often think about my Dad, he saw his grandfather die at home. I think people in older generations had the benefit of witnessing the end of life. We’ve lost that. We move away, we don’t have the conversations, we send our elderly away to nursing homes. What I’ve learned in this process has changed my life. It has been such a privilege. Both of my parents died when I was working on this project and that was profound. But what was immeasurably helpful was that they planned everything. I knew so clearly what they wanted and that I could give them that. It doesn’t take away the sadness or make it less hard, yet it brings enormous comfort. HGF: And that your last loving gift to them was that you could honor their wishes. CJ: If we could recognize what a gift it is to honor other people’s wishes; being willing to listen; not turning away when people want to talk about death. The gift is leaning into that conversation, absorbing it, and knowing and not being afraid of it. I wish we could help people develop a real understanding of what palliative care is, what hospice is, and how to gently move people into hospice care. I can’t help but think about the home hospice care that Berthold Cohen had in Defining Hope. Over the course of 3 months, the hospice nurse helped bring Bert’s wife into the realm of understanding what his choices were and why it was so important to respect his choices. That was a beautiful dance to watch. When I met her, she didn’t think she could ever live a day without her husband. And now, HG Foundation hosts screenings of Defining Hope. Coping with Grief During the Holidays On Saturday, December 1st. we held our annual ‘Coping with Grief During the Holidays’ workshop at the Carmel Presbyterian Church. Over 30 people attended and received encouragement, support, and coping tips for handling grief during the holiday season and beyond. GIVING Fundraising Events The Party – The Scramble | September 2018 Signature events have always been a part of Hospice Giving Foundation. Friends and supporters joined us again for our two-day Party-Golf festivities held at Monterey Peninsula Country Club on September 9th & 10th. Thanks to the generosity of our guests, the event raised over $200,000 to support an array of end-of-life care and services. Our 2019-20 events will be on two distinct dates to allow ample opportunities for the community to gather, support, and celebrate hope, care, and compassion through end of life. Joann Dost Photography Auction | May 2018 HG Foundation board member, Joe Martello, an avid golfer and collector of golf art, hosted a private reception and photography auction May 25, 2018 at Monterey Peninsula Country Club. The exquisite collection of photographs was generously donated to the Foundation by world renowned, local photographer Joann Dost. The photographs, some beautifully framed, captured the splendor of some of the world's most exclusive golf courses. Hospice Giving Foundation / Annual Report 2018 / Page 11 Latino Outreach HISTORY OF GIVING HG Foundation worked with the Stanford Alumni Consulting Team (SACT) this year to explore the reported underutilization of hospice Total Grants Disbursed Through 2018 $25,640,599 care among Latinos in Monterey County. COMPASSION Impact: Grants Through the generosity of our donors, Hospice Giving Foundation is able to award grants to non-profit agencies in Monterey and San Benito Counties providing dignified and compassionate care at the end-of-life. In 2018 Hospice Giving Foundation awarded $761,000 to twelve non-profit organizations in our community. Our Board of Directors takes great care in determining the most impactful ways to support a continuum of care through end of life. The cumulative giving from HG Foundation in its 21-year history proudly exceeds $25,000,000. The generosity of our donors enables HG Foundation to enrich services and programs throughout our funding region. This investment has allowed our community to be at the fore front of excellent end-of-life care.

Here are some highlights Grants Disbursed in 2018 (based on 2017-18 FY) $761,000 from our 2018 funding cycle:

• Funding for hospice care included inspiring and Hospice and Palliative Services $471,000 innovative programming such as Hospice of Pediatric Palliative Care $230,000 Santa Cruz ‘Camp Erin’ for grieving children and End-of-Life Services $45,000 music therapy. Education/Outreach/Advocacy $15,000 • Specifically focused grants supported outreach to address under-utilization of end-of-life care in select Monterey County communities.

End-of-Life Services • Grants for palliative care providers helped fund clinical staff, membership in professional education Education/Outreach/Advocacy organizations, and quality assurance programs. • Grants to local pediatric providers supported families and their seriously ill children with in-home medical care, medical transportation, and a multitude of psycho-social support services. • Programs serving families and communities included grief and bereavement support, care for Pediatric Palliative families of Alzheimer’s patients, and expanded Care Hospice and end-of-life preparedness services in South County.
