What is important to me

 Spending every Sunday with my Mum, we go to church and for lunch  To be as independent as possible  I have opportunities to keep in touch with and friends and go out for food, meet up for activities and to be busy.  I have opportunities to sleep in. I love my sleep I will sleep until 11am easily. If I need to get up early for some reason you may have to coax me. I like playing games on my phone until late at night.  I have enough money to do what I like and it’s important that I don’t l leave home without some money in my purse and my mobile phone.  If I’m cooking at home I need to have 1‐1 supervision. I can do most things myself I just need reassurance and guidance – remind me to ask if I need any help.  I can be quite messy on times and I need lot of prompting to keep my flat tidy or I can have days when I will clean my flat and tidy everything up. If I don’t clean my flat then my support must clean my flat

 My family  To be included in everything, I can hear and understand what you are saying so please include me in all conversations.

there’s not many foods that I don’t like but  Smiling and laughing please offer me food and drink throughout  Communicating my the day. needs  I love my music and have quite a varied taste  Telling jokes of music, but I don’t like sad slow songs or  Cleaning Christmas songs, I tend to get upset and  Picking out clothes for sometimes cry so if you can please change myself the song or radio station.  Socialising  Maintaining contact with my relatives which I can do on my Hudl on Facebook.  One of my favourite things to do is go shopping and being shown different clothes. If you put them in my hands I can touch the different materials.  I have loads of books in my room as well and I love being read to, some of my favourite books are the Roald Dahl stories.

What you need to know to support me

 Having fun Go to my activities planned for the week  Keep in contact with my relatives and friends  Try out different activities  Look my best as I take pride in my appearance  Visit my friends and my Mum  Send cards and presents to my relatives who live further away  My staff are aware and trained to manage my epilepsy  . I really love Jeremy Kyle and tend to watch it most days when I am in the house. I also love to go to the cinema and watch happy/comedy films.  I do like to have a nap each day.  I like to have my nails painted every week and have different colours and I like to have the option of a few so I can decide which colour I would like.  I attend Bikeability and I really enjoy going as it’s a sensory activity for me and it is loads of fun!  I can get distressed at times so please talk to me as I may not know what’s going on. I can be quite impatient but if you explain to me what’s happening and ask me to not worry so much I normally will

What is important to me I visit my family as they are very important to me. My staff need to help me arrange a day and time to visit my parents at their house.  I am included in everything, I never like to miss out on anything so please include me in all activities and conversations.  My staff need to support me when I have a seizure in the safest way possible and would have undergone specific training.  I go out and do various activities, I really enjoy going to the cinema and going out for food and trying new things, my staff need to bring my car mobile phone with them should a situation arise.  It is important that I am kept busy as I do get bored sometimes, my staff need to give me options of what to do when in the house so I can pick an activity. I like to look at holidays on my iPad for the future and like to bake cakes!  I take great pride in my appearance

What you need to know to support me  I have limited vocabulary so my staff need to understand what it is I am saying as I can get irritated by this. If I have difficulty in getting my point across my staff need to support me to use my iPad as I have a communication aid app called ‘Prologue2Go’  When I go out in the community, I always need to carry my bag as all my medication is placed in there  I have epilepsy  I love taking photos on my iPad

 I like to be included in any decisions made about me from the little things to the really big things, including being included in all conversations and I will always remember what has been said.

 I like to be independent.

 I like to book things such as concerts as music is very important to me and I have a large collection of all kinds of music.

 I always like to look my best when I go out with my family and like to buy a new outfit and get my hair and make-up done.

 Keeping in touch with family and friends is important to me.

 Attending my Computer and Art courses is important I enjoy making Cards for upcoming Birthdays etc. in Cards and practicing my typing. I enjoy going to Art and doing things such as felting.

 I also like going to my family caravan on holidays and need support to arrange

 Being independent in my motorized wheelchair I like to steer myself around the house, but I understand I need support when out the back or when I am in the community.
