ROYAL ARCH MASONS FEE: $50.00 to accompany this Petition ( ) I hereby petition for the Capitular Degrees of Freemasonry given in Covington Chapter No. 71 and represent that I have not previously been rejected by any RAM. and that if this petition is granted, I promise to comply with all the forms, usages, laws, rules, orders and regulations of your Chapter, the Grand Chapter of Georgia and the General Grand Chapter International.

ROYAL and SELECT MASTER FEE: $50.00 to accompany this Petition ( ) I hereby petition for the Cryptic Degrees of Freemasonry given in Covington Council No. 41 and represent that I have not previously been rejected by any R.&S.M. Council. I promise to comply with all the forms, usages, laws, rules, orders and regulations of your Council, the Grand Council of Georgia and the General Grand Council international.

KNIGHTS TEMPLAR FEE: $100.00 to accompany this Petition ( ) I hereby petition for the Masonic Orders of Knighthood given in St. Bernard Commandery No. 25. ( ) Must ! I also confirm that I am a firm believer in the Christian faith. I also represent that I have not previously been rejected by any Commandery and that if this petition is granted. I promise to comply with all the forms, usages, laws, rules, orders and regulations of your Commandery: the Grand Commandery of Georgia and the Grand Encampment of the United States of America.
