Weekend Services Available for our , and Patients

Rapid Response - Nursing • Input from community nursing service for up to 72 hours. • Will accept referrals from front of house and the discharge facilitators • All patients must be over the age of 18 years • All patients must have been seen be a Dr and have a medical management plan before transfer of care. • Will respond within two hours. • Is aligned to Therapy rapid response services. • Contact: Middlesbrough – 07810056317 Redcar – 07826875656 • Last time for referral is 9pm as the service finishes at 11pm • Referral forms available from the FoH Therapy Team

Rapid Response - Therapies • Input from community Physiotherapy and Occupational Therapy for up to 72 hours. • It prevents unnecessary re-admission to hospital. • Facilitates hospital discharge from FoH and A&E teams. • Patients must have been seen by GP or community matron prior to community therapy if referral is from the community. • Can refer patients onto the core community therapy teams as appropriate. • Referrals received from FoH Therapy, A&E Teams, community matrons and GP’s • 7 days/week, • Last time for referral is 6pm as services finishes at 8pm Contact: Middlesbrough 07810056317 07826875656

Social Services Middlesbrough: • emergency out of hours social services duty team - provide advice in emergency situations but will not be able to support planned hospital discharges - contact: 08702 402994 • Home Care Rapid Response Service - available for all wards - complete a referral form (available from FoH team), fax to 01642 726983 and call 01642 726009 to confirm - Available to all wards and is a free service to patients for 10 days to facilitate early discharge

Redcar & Cleveland: • emergency out of hours social services duty team - provide advice in emergency situations but will not be able to support planned hospital discharges - contact: Emergency Duty Team on 08702 402994 • Home care Rapid Response Service • - available for FoH wards to prevent admission for up to 7 days, working 7am – 10pm - complete referral form and contact: Tel: 01642 837270 or Fax: 01642 771392 - The call will be answered by Coast & Country Housing Contact Centre , last call taken at 8pm • Recovery & Independence Team - will respond and assess patients in hospital – this is only applicable to patients who have rehabilitation potential and who live in R&C - Contact: 01642 303650

Community Hospitals • , Redcar and East Cleveland: patients should be accepted by the CoE team prior to transfer into a consultant bed • Rutson Unit: Patients should be accepted by Dr Jay prior to transfer • Lambert beds: patients should be accepted by Dr Spence prior to transfer

NB: patient transfers into GP beds cannot take place during weekends and / or bank holiday periods
