ITEMIZED SELECTIONS The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF ...... $ 2,160.00 Our fee for the basic services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family and responsible parties; arrangement of funeral; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notice; and coordination with those providing other portions of the funeral, e.g., cemetery, crematory, vault companies and others as required. Also included in this charge are the overhead expenses relative to our facilities such as sheltering of remains, professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, recordkeeping, and secretarial and administrative expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. Outer Burial Container Price List These prices are effective -October 21, 2019-

State or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements.

Things you will need to bring to the Arrangements Conference: 1. Social Security Number 2. Their final DD 214 if the deceased was a veteran. 3. A photo of the deceased. It does not matter if other people are in the photo, as we can crop them out. Please make sure that the image is as clear as possible. Please do not scan and print the picture or save to a disk because we need the photo scanned at a minimum of 300 DPI. 4. Clothes for burial. Such as, under garments, socks, stockings, shoes and glasses if you want, etc. Terms of Payment Policy: Payment for our goods, professional services and cash advancements is expected in full at the time of the funeral arrangements. For your convenience, we accept cash, personal checks, credit cards (VISA, American Express, Master- Card and Discover), and verified life insurance policy assignments. If you plan to use a Life Insurance policy to pay for the services, please bring the policy with you.

We also accept Verifiable Insurance policies: 1. We can only take Life Insurance policies which are cleared and accepted by a Capital Funding Company which we file claims through. If these companies can not Verify, the policies we will not be able to accept them for payment of services. The funding companies charge a fee for filing the claims on the policy or policies. We use these companies for filing Life Insurance policies because of the amount of paper work, and the amount of time we must wait to receive the funds. If you are paying by Life Insurance Policy, All Cash Advance Items Must Be Paid At Time Of Arrangements. 2. We are unable to accept Government (FEGLI) VA or Public School Employees Life Insurance Pol- icies because they are Non Assignable. 3. We are unable to file against estates or wait for funds from legal actions.

Radio Broadcast: WRJW-AM 1320 broadcasts obituaries during their news programs at 7:30 AM, 12:15 PM, 4:30 PM Monday - Friday. There is no charge for this service by WRJW or the McDonald Funeral Home. Internet Listing: The McDonald Funeral Home also places your loved ones obituaries on our web site, which can be seen anywhere there is Internet connectivity. We never take the obituaries off our web site. You can go to www.mcdonaldfh.com to see obituaries on our web site. Visitors to the obituaries page of our web site may also sign the register book and send condolences to the family of the deceased. In an effort to publish your loved ones obituary more effectively we also utilize Facebook and Twitter. Both of these internet opportunities give their users a link to our main website where they can read your loved ones obituary. 24 Hour Obituary Telephone Line: For those who would rather use their phone to hear obituaries they may call 601-798-5588 24 hours a day. Our obituary line is updated each day in the late afternoon. Local Florist: We fax the local florist the obituaries to help them keep informed of our current services. On our website, www.mcdonaldfh.com, we have a listing of local florists with their phone numbers for local or out of town friends and relatives who want to order flowers.

MEMORIAL ITEMS: A. Memorial Slide Show DVD: We provide 2 copies with the purchase of a full service funeral or cremation at no charge. These DVDs are made by our staff from photos that the family brings in to use. These programs are made up of up to 40 photos. Additional Copies: You may obtain copies of the Memorial Slide Show for a charge of $ 15.00 each. B. Laminated Obituaries: We provide each family with one laminated obituaries after each service. Additional Copies: If you wish you may purchase extra copies at a charge of $ 6.00 each. These obituaries contain a picture of the deceased if we are provided with one to scan, and the obit. C. Memorial Folders: We provide memorial folders to each family that purchases a register book. However, memorial folders may be purchased in increments of 50 for $25.00. D. Angels: We present each family with a Memorial Angel which bears their loved ones name and dates of birth and death. These Memorial angels may be used as ornaments on Christmas trees. If you are interested in purchasing additional Angels, they are priced at $ 25.00 each. If you would like a ornament stand they are $ 10.00.
