MERIT BADGE PREREQUISITE FORM SAW 2018 -

SCOUT FULL NAME: ______TROOP: ______COUNCIL: ______MERIT BADGE: MEDICINE – Read the MB book; bring a notebook /pen or pencil 1A (may use a merit badge workbook to record notes for discussion) 7A (bring date of last visit to physician & current BSA health form) 10 (bring documentation of volunteer service to complete MB at SAW)

The Scout named above realizes that this Merit Badges cannot be completed at Scouting Advancement Weekend (SAW) unless prerequisite requirements are completed before arriving at SAW. He also realizes that this form must be completed correctly, signed, and given to the SAW Merit Badge Counselor as verification that the prerequisite requirement(s) have been met. The SAW MBC can then grant the Merit Badge upon successful completion of the course in the above mentioned Merit Badge.

10. Volunteer at a health-related event or facility in your community (e.g. blood drive, "health fair", blood pressure screening, etc.) and bring documentation from your supervisor.
