We reviewed the first- and second-line therapies listed in The Medical Letter5 and UpToDate®. In general, if either source listed a regimen as a first-line therapy we included it as a first line therapy. Where the two sources differed we used a review article, recent RCTs and own judgment to designate first- and second-line therapies.
