We are a class II landfill located ~40km east of Rainbow lake and ~100km west of High Level on hwy 58 on the North side of the road. We are licenced to take non Hazardous material.

Prior to acceptance all material need to have a Class II landfill analytical completed on it to prove that the product is not hazardous. This along with a WAA (Waste Acceptance Approval) form needs to be submitted to the facility to be reviewed (form attached). Once both are received the facility will review and send out a waste confirmation form stating that the product is good for acceptance. This form will have an approval number specific to the substance and generating site for tracking purposes. All products arriving at the facility are weighed in and visually inspected to ensure that there are no free liquids and that the product is acceptable for disposal within our class II landfill.

Our hours of operations are 7am to 7pm 7 days a week.

With the lab and WAA completed I do not see any problems with the acceptance of the product you described. Please let me know if you have any questions or concerns

Hello Tyler,

I was cc’d your email on a correspondence I had with Rob Menzies concerning some hydrovac waste sludge that was going to brought to the Tervita facility in Rainbow Lake. I have attached a copy of the email for your reference.

A couple of things has happened since I last spoke with Rob and yourself. 1. The project has since been delayed until September. 2. The sludge is going to be treated before coming to your facility.

The sludge is going to be treated with a polymer agent, solidifying it in the shale bin before it is brought to the rainbow lake facility. Are you able to send an email back stating that this will be acceptable? I have attached a brief outline of the treatment procedure and msds of the polymer being used.

Additionally, I hereby indemnify Tervita and save it harmless from and against any claims, actions, damages, liabilities and expenses including lawyers and other professional fees, in connection with the loss or injury whatsoever arising from or out of any inaccuracy or untruthfulness in the information provided herein. EARTH MATTERS This form is to be utilized to describe “Solid Waste” offered to Tervita for disposal and/or management. These materials include; but are not limited to: • Industrial Waste • Oilfield Waste Waste not included in this definition is: • Municipal Solid Waste (MSW) In accordance with Provincial Regulations, Generators must determine whether their Waste is a dangerous/hazardous or non-dangerous/non-hazardous waste. In order to manage your Waste, Tervita requires certain information about your waste to confirm that your determination of it meets landfill disposal acceptance criteria and that it can be managed in a safe, environmentally sound, and lawful manner. This information will serve to protect you, the Waste Generator, as well as Tervita Corporation.

GENERAL INSTRUCTIONS 1. The Generator or Generator’s Authorized Representative must complete the Waste Approval Application (WAA) form. Answers should be as thorough and accurate as possible. The entire WAA form must be completed, answers must be legibly printed in ink or typewritten, and the completed WAA form must be signed and dated. Write “N/A” where the data requested is not applicable, and “N/D” if the information has not been determined. Please attach any additional relevant information such as SDS, Analytical Data, or explanations that will help to describe the Waste and expedite its review. 2. Send the complete and signed form, along with supporting documentation, to your Tervita Representative. If you have any questions concerning the completion of this WAA form, please contact your local Tervita Representative or call the main office at (403) 233-7565.

SECTION 1 GENERATOR INFORMATION a) Generator’s name – name of the company generating the Waste. b) Generator’s address – mailing address including the street, city and province of the generating facility. c) Generating location – physical location (i.e. LSD location) of the facility, well, pipeline, etc. generating the Waste. d) Generator’s Authorized Representative contact – The name of the Generator’s employee or authorized representative completing the WAA form and their telephone and/or fax numbers. This person must also sign the form in section 7 or by the Generator’s Authorized Representative identified in Section 3(c).

SECTION 2 INVOICING INFORMATION This section is intended to provide Tervita with accurate information related to invoicing for the receipt of Waste. Waste brokers, if being invoiced for the Waste, should fill in this section with their information. a) Check the box if the invoicing information is identical to the information in section 1. b) Company name – name of the company that shall be invoiced for the Waste. c) Company address – mailing address including the street, city and province of the company to be invoiced for the Waste. d) Contact – name of the person to be contacted regarding invoicing of the Waste, including phone and/ or fax numbers. e) Job# / EDI# - The appropriate Generator/Consultant number specific to the waste stream being offered for disposal is required when applicable. It is an EDI, AFE or PO number.

This person if applicable must sign the form in section 7 as the Generator’s Authorized Representative.

SECTION 4 LANDFILL DESTINATION Select the Tervita landfill where the Waste is to be disposed. Tervita is pleased to offer disposal options at our partnered facilities; your Tervita Representative can provide information regarding these facilities.

SECTION 5 WASTE STREAM INFORMATION a) Waste description – check the Waste type that most accurately describes the Waste to be disposed. b) Process generating Waste – accurately describe the process that generated the Waste (i.e. crude oil pipeline spill cleanup). c) Anticipated volume – indicate the tonnage (tonnes) or volume (cubic meters) of Waste to be disposed. d) The shipping method for bringing Waste to the facility. e) The frequency of generation of the Waste described is to be noted. f) In accordance with OH&S Hazard Communications requirements and/or GHS requirements, provide any special handling information, personal protective equipment recommendations, and other relevant information that will prevent injury or illness resulting from the safety hazard or if it can be handled safely without the use of personal protective equipment or special handling procedures.

SECTION 6 WASTE DESCRIPTION The following questions help determine if the Waste is acceptable at the designated receiving facility, they do not include all classification criteria. All Waste must be accurately characterized and classified as dangerous/hazardous or non-dangerous/non-hazardous as per Provincial Regulations. a) Flash point – check applicable box. b) pH – check applicable box. c) BTEX – check applicable box. d) NORM – Check applicable box. e) This is confirmation by the Generator or Generator’s Authorized Representative that the Waste is classified as NON-DANGEROUS/NON-HAZARDOUS. Disposal of dangerous/hazardous waste and misrepresentation of dangerous/hazardous waste as non-dangerous/non-hazardous is strictly prohibited. Liability for improperly classified Waste remains solely with the Generator.

In the case of “Supporting Lab Analytical I.D. no.” write down the I.D. no. from the supporting lab analytical. This should be the I.D. no. given by the lab to the analytical data. Tervita can then verify the WAA data with the lab if required. (Note: Tervita requires that supporting signed lab analytical be from accredited labs).

SECTION 9 GENERATOR’S CERTIFICATION The Generator identified in Section 1 (d) or Generator’s Authorized Representative identified in Section 3 must sign and date the certification. The WAA will not be processed and the Waste will not be approved by Tervita management without completion of this and all other sections.
