Basic Testing Information: Each model contains 8 test slides. Tests are administered at Far, Near, and Intermediate distances. For ease in administration, follow test sequence under each category. If you wish to skip a test, continue with the next test in the sequence.

Testing Function Tests vision with Both working together.

Consists of 2 cubes for each . Description Left Eye - White & Blue Right Eye - Red & White

Age Tested All ages.

The 2 white cubes should fuse together for a subject with good tips binocular vision. The functioning eye can be determined by asking the subject to identify the colors.

2 Slide 2 - SAB-2 Acuity, Both Eyes

Tests acuity for Both Eyes. This test checks for sharpness of the eye. Right and Left Eye images are the same, normally they fuse into a single image. Testing Function ADMINISTER TEST AT FAR, NEAR AND INTERMEDIATE: The intermediate lens must be inserted with the label facing the front of the instrument.

Description Test uses Sloan Letters to test .

Age Tested All ages.

3 Slide 3 - SAR-2 Acuity, Right Eye

Tests acuity. This test checks for sharpness of the eye. Image shown for Right Eye only, no image on Left Eye. Testing Function ADMINISTER TEST AT FAR, NEAR AND INTERMEDIATE: The intermediate lens must be inserted with the label facing the front of the instrument.

Description Test uses Sloan Letters to test Visual Acuity.

Age Tested All ages.

Start with the line the subject can see most clearly. This will save time. For conventional testing, DO NOT occlude any eye. For specific purposes, occlude the Left Eye, for example: tips 1. When a subject with a good test score for Both Eyes hesitates unduly or experiences difficulty at a low score level, record both occluded and un-occluded scores. A subject with no vision in the Right Eye will, of course, see no letters with the Right Eye occluded. In cases of Right-Eye suppression, the score can improve markedly when Left Eye is occluded. 2. To obtain medico-legal vision records. Specify binocular and monocular when this procedure is used.

5 Slide 5 - SSD-1 depth

Testing Function Tests or ability to judge relative distances.

Contains 9 targets with rings. Each target contains one ring Description that appears to float out.

Age Tested All ages.

Use target No. 1 as a demonstrator. This test requires tips simultaneous use of Both Eyes. It should be omitted in cases of little or no vision in one eye.

Look at target No. 1. You will see 4 rings. Does the bottom Patient Question ring seem to float out towards you, compared to the background? In target No. 2, which ring floats out? No. 3…?

Subject identifies Top, Bottom, Left or Right ring that floats.

6 Slide 6 - SCD-1 color perception

Tests red /green color perception. Test uses accurately Testing Function reproduced Ishihara Pseudo-Isochromatic plates.

Contains 6 circles, 5 numbers, one with no numbers. There are Description a total of 8 numerals in the 5 numbers.

Age Tested All ages.

This screening test checks for a problem in recognizing colors but does not classify type or degree of severity. For further testing, a more sophisticated color test (example, Ishihara color book) is recommended. Individuals with low visual acuity (20/50 or less) in Both Eyes may fail this test due to low acuity rather tips than faulty color perception.

NOTE: To avoid memorization of numbers in circles, ask subject to respond randomly. (example, what number do you see in circle E, circle B, circle D,...?

7 Slide 7 - SVP-1 vertical phoria

This is a Muscle Balance Test to define a value for the balance of the eyes in the vertical direction. This identifies if there is Testing Function a tendency of Left Hyperphoria (one eye in the line-of-sight elevated relative to the other eye) or Right Hyperphoria (one eye in the line-of-sight depressed relative to the other eye).

Description Consists of a red line and a of musical notes which overlap, when seen with Both Eyes.

Age Tested All ages.

Occasionally, the subject may be confused. When this happens, momentarily occlude the Left Eye, which sees the red line. Ask the subject to respond immediately when the red line reappears tips (Where do you first see the red line?). Remove occlusion. This is a binocular test. It should not be given to an individual with vision in only one eye.

Do you see a red broken line? Through the round part of Patient Question which musical note does the line pass?

This is a Muscle Balance Test to define a value for the balance of the eyes in the horizontal direction. This identifies if there Testing Function is Esophoria (one eye turns inward compared to the other), (one eye turns outward compared to the other), or Orthophoria (balanced eyes).

Consists of an arrow and a set of musical notes which overlap, Description when seen with Both Eyes.

Age Tested All ages.

Occasionally, the patient may be confused. When this happens, momentarily occlude the Left Eye, which sees the arrow and horizontal lines. Ask the patient to respond immediately when tips the arrow reappears (Where do you first see the arrow pointing?). Remove occlusion. This is a binocular test. It should not be given to an individual with vision in only one eye.

9 Peripheral Vision Test USE test slide 1 for

Testing Function This test checks the ability to see from the corner of the eye, in the nasal and temporal horizontal visual fields.

Charts the eye’s field of vision in the horizontal plane, using Description light flashed at various locations in the periphery of the eye, from the temporal to the nasal area.

Age Tested All ages. Mainly adults are tested.

It is recommended that eyeglasses are removed prior to testing. In some cases, the individual may fail due to eyeglass frames blocking the flashing lights. tips Test Left Eye first, pressing pad for each degree. Light will flash. 85, 70 and 55 degrees are for temporal fields and N (45 degrees) is for nasal fields.

Instruct subject to look ahead at the test slide in view. Patient Question (Test No. 1 at FAR is recommended). Do you see a light flashing? Ask subject to raise hand each time light is seen.

Normal Peripheral Vision: Pass at 85°, 70°, 55°, N (45°) Pass/Fail Criteria Some Restriction: Fail at 85° Severe Restriction: Fail at 70° or 55° Nasal Restriction: Fail at N (45°)
