Clinical Research Network Greater Manchester What are the challenges?

TIME COMMITMENT allenges?

SPACE FEASIBILITY What are the benefits?

9 Vast majority of care delivered entirely in the community

Majority of people with long term conditions now managed completely within primary care Access to ‘ healthy’ people

Established GP/patient relationships. Some SLS practices recruited around 70% of eligible patients

Increasing research expertise in primary care in GM with good track record of delivery Access to larger patient populations

Extended hours PC facilities- better access for researchers and patients

Collaborations of practices (led by GP federations, PCN )

Closer PC / SC working – hybrid models for research delivery

Expanding workforce – e.g.‘ in practice pharmacists’ nurse practitioners

Linked IT systems (Farsite, CPRD , EMIS)

Dedicated time/staff for research – more part time ‘ portfolio ‘ GPs and other PC professionals e.g. pharmacists, physician associates with a research interest Examples Primary Care Models in GM

• Single sites of varying sizes e.g. Swan Lane ( 3 GPs) , Tower Family Healthcare Bury (30 + GPs)

• Neighbourhood hub and spoke e.g. Chadderton cluster ( 5 practices)

• GP Federation led approach – multiple sites/ hub and spoke models encompassing all practices in CCG area e.g. South Manchester

• SILK Real World Research, company led by experienced researchers in East Cheshire providing ‘ gateway to’ and wrap around services for all practices in East Cheshire. The new GP Contract January 2019

8. Supporting research and testing future contract changes •Research 8.2 Research participation within general practice has many additional benefits: quality improvement, professional development, and generating income, amongst others. •Working with the Royal College of General Practitioners, the National Institute of Health Research and the Clinical Practice Research Datalink, we will use the opportunity created by Primary Care Networks to increase general practice research participation levels. •During 2019, we will develop a way of helping networks to do this, embedding approaches such as the RCGP Research Ready quality assurance model 59. •We welcome also the National Institute of Health Research’s intention to prioritise an expansion in academic research capacity into primary care.

AIM – to provide the best possible model for research delivery for everyone involved in order to ensure delivery to time and target How Can Practices get Involved ?

Large and diverse study portfolio including most specialties/methodologies Academic and commercially sponsored trials

1. Promote studies and NIHR campaigns • Research For The Future / Join Dementia Research/ Be Part of Research • Study specific posters to aid recruitment 2. Patient Identification Centres • Identify patients (FARSITE) screen eligibility • send out invitation letters (DOCMAIL) 3. Delivery Sites Recruitment Centres • Recruit patients and deliver at practice Real World Research

The Lung Study Scale of the project A first in the world achievement for 80 GP (family doctor) sites the UK 130 ‘high street’ • 2 late phase pRCTs sponsored by GSK pharmacies >300 study staff • 2802 COPD patients and 4233 asthma >3000 staff GCP trained and research ready patients were recruited in primary care and monitored in near real-time for safety and Specialist safety team outcomes using city-wide linked electronic covering 2 hospitals health records Bespoke e CRF and data monitoring system - • Study is as near to “everyday care” as designed built and possible working

• Scientific rigour is maintained NIHR Research Targeting Tool
