We are very pleased to present the Jefferson Community Directory as a resource guide for the South Hills and lower Mon Valley community. Our intention is to provide a tool which will increase the quality of life for patients, residents and families through effective local connections and referrals. By strengthening the connections between community organizations and health partners, we can ensure that our residents, patients and family members receive the assistance they need.

We all need the support of our community at various times and this Directory is being distributed to the key connectors in the area served by Jefferson Hospital – community- based organizations, churches, libraries, physician practices, hospital case managers, food pantries, the local offices of elected officials and more. We recognize that health and quality of life are intertwined, and so the listings cover a wide variety of human needs, including child/youth programs, behavioral health, civic-neighborhood resources, healthcare, human services, legal resources, senior services and transportation.

This Directory was created through an important partnership between the Jefferson Community Collaborative, Jefferson Regional Foundation and Jefferson Hospital. The intention was to list contacts at services and programs which are located in the Jefferson area to facilitate direct connections—not to replicate other regional guides. The Directory includes a listing of all the local organizations, an area map, detailed program information by category, a notes section on each page for users, and additional resource lists of local school districts and local legislative offices.

Special thanks to the 55 member organizations of the Jefferson Community Collaborative (see next page) and its Directory Advisory Committee, Jefferson Hospital President and CEO Louise Urban, and the Board of Directors of the Jefferson Regional Foundation. The Collaborative members offered significant and essential ideas about this guide and are committed to its use. Jefferson Regional Foundation is dedicated to an exciting mission—to improve the health and well-being of the community served by Jefferson Hospital through grantmaking, education and outreach. The Foundation will serve the community with integrity and transparency. In March 2013, the agreement which guided the affiliation of Jefferson Hospital with the Allegheny Health Network provided $75 million for this new mission. Our grantmaking area is the South Hills and lower Mon Valley area and our priorities include: increasing health access and prevention, improving child and family outcomes and strengthening vulnerable populations and communities.

The Foundation’s focus on improving health for all means a commitment to addressing the “social determinants” of health—and an understanding that “place matters” and community resources are essential to health and well-being. The Foundation recognizes its role in covening and sharing information through its annual community learning event called the Jefferson Forum, through the Jefferson Community Collaborative, and in its support of this Jefferson Community Directory. Jefferson Hospital, part of the Allegheny Health Network, an integrated healthcare delivery system serving the western region, is a community-based healthcare facility in Jefferson Hills serving the 500,000 residents of the South Hills of , PA. Nearly 400 physicians provide comprehensive inpatient and outpatient medical and surgical services in more than 40 specialties, from primary care to a new Women and Infants Center and Cancer Institute. Services include leading cardiovascular, orthopedic, bariatric and gynecologic surgery programs and advanced capabilities in neurosurgery, minimally invasive robotic surgery, rehabilitation services, behavioral health and more. The Jefferson Community Collaborative was convened in the fall of 2015, building on the energy and enthusiasm generated in June of that year by the first Jefferson Forum: Collaborating for a Changing Community. The Collaborative is built on the collective strengths of 70+representatives of 55 partner organizations (listed on the next page) who are committed to the well-being of residents in the Jefferson area. The Collaborative is built upon the shared values of diversity & inclusion, collaboration and consensus building, commitment to sustainability, transparency, leadership and respect. Work on short-term and longer-term goals is achieved through three teams; Healthy Residents, Strong Families and Thriving Communities, which meet monthly at one of two generous host sites: CCAC South and the Steel Center for Career and Technical Education. Jefferson Community Directory 7 How to Use The Jefferson Community Directory

The Jefferson Community Directory is designed as a tool to facilitate direct connections between community residents and local resources which meet their specific need. It is intended to serve as a one-stop connection – listing the names and contact information of individuals who can provide access or detailed information about their specific programs and services.

When individuals or families are in need – they often turn to trusted assets in the community – whether that is a case manager, a pastor, physician’s office, legislative office, a local library or many others. This Directory is intended to be used by these various key organizations, and will be distributed more widely to those groups over the coming months.

The Jefferson Directory is not designed as a replication of other community guides. Programs and services which were selected for inclusion have a site or an established base of services in the South Hills or lower Mon Valley area.

Members of the Jefferson Community Collaborative assisted in outlining key service categories and in identifying community resources to be included. An Advisory Committee provided additional guidance in the design and direct assistance in verifying content. We are grateful for their involvement.

The Jefferson Community Collaborative asked that the design of the Jefferson Directory be local, personal, and user-friendly with the following features: • A quick listing of all of the organizations included in the Directory with key information. • A detailed program information section divided into service categories. Each service category is marked by its own icon and color to facilitate ease of use. Programs are listed alphabetically within each service category and include the name of a program contact and contact information to ensure a personal connection is made for Jefferson residents.

• Information about language access is included only if the program has bi-lingual staff or offers materials in other languages. All programs are encouraged to provide language access services (i.e. see the listing for Center for Hearing and Deaf Services PLAN program which offers language interpretation and other services). • A notes section is included below each program so the user can record updates or additional details. • A listing of the school districts serving the Jefferson area is included. • A general resources page serves as supplemental information or when local resources are not available.

While the Directory highlights the many strong services in the Jefferson area, it also will serve as a tool to identify resources which are lacking or underdeveloped in our area. The Jefferson Community Collaborative, Jefferson Hospital, and the Jefferson Regional Foundation are committed to building on the strengths of the local community.

The Jefferson Directory was designed with the best of intentions and an attempt was made to verify all the information provided – but it certainly may include some errors. Please understand these were not intentional.
