THE UNIVERSITY OF HONG KONG INTERNATIONAL ADMISSIONS SCHEME FOR UNDERGRADUATE ADMISSION IN SEPTEMBER 2011 GENERAL INFORMATION AND INSTRUCTIONS 1. The procedures and information in this document apply to those applicants who require a student visa / entry permit to study in Hong Kong with overseas qualification. 2. Applicants applying through this International Admissions Scheme will not be considered for undergraduate admission via other schemes / means in the same admission year. Duplicate applications will not be considered by the University and the application fees paid are non-refundable. Closing Date and Submission of Application 3. You can apply for admission online or by paper form. In case of paper application, the completed application package should be returned by post / in person to: Academic Services Office Room UG05, Upper Ground Floor, Knowles Building The University of Hong Kong Pokfulam Road, Hong Kong Office hours: Weekdays : 9:00 a.m. - 5:30 p.m. Saturdays : 9:00 a.m. - 12:30 p.m. Closed on Sundays, Public Holidays and University Holidays ( i.e. Christmas Eve, New Year’s Eve (p.m.), The day preceding Lunar New Year (p.m.) and Foundation Day (March 16)). The closing date for application is December 30, 2010. An acknowledgment letter / e-mail will be sent to you normally within two weeks after receipt of your application. If you do not hear from the University after four weeks, please check with the Academic Services Office by phone at (852) 2859 2433 or by e-mail <[email protected]> regarding your application. Application Fee 4. All applicants are required to pay an application fee of HK$300 ( for online application ) or HK$700 ( for paper application ) and the fee is non-refundable. Please refer to the "Payment of Application Fee" sheet for details on the payment methods. Application Form 5. The application form should be completed and submitted as follows: Part 1 (Pages 1-4) To be completed by the applicant. Part 2 (Pages 5-6) Academic Referee's report or reference letter, to be completed by your school principal or teacher, or by an academic referee. The completed confidential report should be returned as soon as possible to the Academic Services Office, Room UG-05, Knowles Building, The University of Hong Kong, Pokfulam Road, Hong Kong either directly by the academic referee; or if the principal / teacher / academic referee prefers, it may be placed in an envelope addressed to the University, sealed, and given back to the applicant for him / her to return it to the University with other application documents. Applicants who have already left school or college and cannot obtain an up-to-date report from a school principal or teacher may leave the report blank. 6. You are also required to provide the following supporting documents in the sequence listed below: Part 3 A photocopy of your HKID card / passport / National identity card / birth certificate. (For applicants applying on the basis of "mature age", an officially certified copy of the document is required.) Part 4 Academic transcript (s ) / public examination results in detail, e.g. GCSE, HKCEE, including in the case of GCE AL/AS level, the Statement of Results which shows the numerical scores achieved ; and the predicted grade / point issued by the school. Part 5 (optional) A summary of not more than 2 A4-size pages of your extra-curricular activities / personal achievements / working experience, and any other supporting documents . Photocopy of academic transcript(s) / public examination results / personal identification in the case of mature student is acceptable for preliminary consideration of your application but officially certified documents must be submitted before an offer can be made. Please note that documents once submitted will not be returned, and will be destroyed if the application is unsuccessful. Choice of Curriculum 7. You should only submit ONE application for admission in any one academic year, but in that one application you are permitted to indicate up to 3 choices of curricula. Check the entrance requirements of each Faculty from the Undergraduate Prospectus 2011-2012 (online at <http://www.hku.hk/acad/ugp>) to make sure that you are eligible for the curricula you wish to choose. Interviews and Results 8. You may be called for an interview or a test at any time after the closing date for application, and you should be prepared to come at very short notice. 9. Under normal circumstances, you will be informed by post of the result of your application as soon as possible and no later than September 9, 2011. If you are offered a place, you must pay a deposit of HK$10,000 when you accept the offer by the date specified in the letter or the offer will be withdrawn automatically. You are advised to call the Academic Services Office (28592433) if you have not received the result of your application by September 9, 2011.. You can also view your application status online at <http://www.hku.hk/admission/nonjupas.htm> starting from February 2011. Composition Fees and Financial Assistance 10. The composition fees for local and non-local students are different. According to the definition laid down by the University Grants Committee, non-local students are persons entering Hong Kong for the purpose of education with a student visa / entry permit issued by the Director of Immigration. The composition fee for the academic year 2010-2011 is HK$42,100 for local students. The fee for 2011-2012 has yet to be decided pending Government’s advice, but we do not expect it to differ significantly from the current fee. For non-local students, the composition fee for the cohort joining the University in 2011-2012 will be HK$119,000 per year during the period of their undergraduate studies. 2 INSTRUCTIONS ON COMPLETING THE APPLICATION FORM Complete pages 1-4 by typing or by writing clearly in black or blue ball point pen. Please follow the instructions below. 1. PERSONAL INFORMATION FAMILY NAME and GIVEN NAME(S) Write your family name and given names in the boxes, without the title of address ( i.e. Mr. or Miss or Mrs. ). Leave one box between each word. Your name should be identical to the one stated on your Hong Kong Identity Card / Passport and used for any public examination(s). Example: CHAN TAI MAN FAMILY NAME GIVEN NAME(S) C H A N T A I M A N NAME IN CHINESE AND C.C. CODE ON HKID CARD Write your name in Chinese characters in the upper boxes, and the C.C. Code of each of these characters in the lower boxes. (Please refer to your H.K.I.D. card for the C.C. Codes of the characters of your name in Chinese) . Leave this column blank if you do not have a Chinese name. If you have a Chinese name but do not have a H.K.I.D. card and therefore do not know the C.C. Codes, you may enter your Chinese name but leave the boxes for C.C. Codes blank. Example : 陳 大 文 1 2 3 4 5 6 7 8 9 1 2 3 DATE OF BIRTH Write day, month and year in numbers. Example: May 1, 1992 0 1 0 5 1 9 9 2 DD MM YYYY SEX Write in the box " M" for male and " F" for female. HKID CARD Write the alphabetic prefix in the first box. Be very careful that you have written your HKID number correctly, including the check digit indicated in ( ). If you do not have a HKID number, leave all the boxes blank. Example : K 0 0 1 2 3 4 ( 5 ) 3 PASSPORT NUMBER For applicants without a HKID card, please enter your passport number in the boxes. If you do not have a HKID card nor a passport, please put “NIL”. NATIONALITY Please indicate your nationality by entering the correct code after referring to Code Table 1 on page 8 of these Instructions . COUNTRY OF RESIDENCE Please indicate your country of residence by entering the correct code after referring to Code Table 3 on page 11 of these Instructions . DO YOU REQUIRE A STUDENT VISA / ENTRY PERMIT TO STUDY IN HK? Check the box “Yes ” or “No ” as appropriate. If yes, you are classified as a non-local applicant and will be required to pay the composition fee for non-local students upon admission. UNIVERSITY NUMBER If you were a student at this University previously, please write your University Number in the boxes provided. CORRESPONDENCE ADDRESS Give an address at which you can be contacted during September 2010 - September 2011. Write in the boxes as if you were addressing an envelope. Leave one box empty between words. Please write your address carefully and correctly as all future correspondence will be sent to the address provided. Inform the Academic Services Office in writing of any subsequent changes to the address. Example: Room UG05, Knowles Building, Pokfulam Road, Hong Kong. If you are not in Hong Kong, you should, if possible, give the telephone number of a person in Hong Kong through whom you can be contacted at short notice.
