JOB DESCRIPTION

INTRODUCTION

This is an appointment to the Gastroenterology Department in the Specialist Medicine Directorate, which provides clinical services at both the Maidstone and Tunbridge Wells Hospital sites. This is a new post, joining a department of 8 other gastroenterologists in the Trust. The new consultant will have responsibilities for helping deliver a first class Gastroenterology and general medicine Service at the Trust.

MAIDSTONE & TUNBRIDGE WELLS NHS TRUST

The Trust was formed in April 2000 by the merger of the Mid Healthcare Trust and the Kent & Sussex Weald NHS Trust. It provides general hospital services to a population of approximately ½ million in West Kent and North East Sussex, and Specialist Cancer services to a considerably larger population. Its major hospitals are the Tunbridge Wells Hospital at Pembury and Maidstone Hospital. The Trust’s Headquarters are at Maidstone Hospital. The Trust’s Chairman is Mr David Highton, the Chief Executive is Mr Miles Scott and the Medical Director is Dr Peter Maskell. Clinical services are organised into nine Directorates. The chief of service is Dr Laurence Maiden, the Clinical Director for Medicine, which encompasses gastroenterology, is Dr Paul Blaker and the Lead Clinician for gastroenterology is Dr Hemant Sharma.

MAIDSTONE HOSPITAL

Maidstone Hospital is a modern 410 bed hospital situated at the western edge of Maidstone, two miles from junction 5 of the M20. It provides general hospital services to a catchment population of around 220,000. The Accident & Emergency Department has approximately 50,000 attendances per year. The hospital has a full range of diagnostic services, including multi-slice CT and MRI scanners. An upper GI cancer service for the Kent and region is commissioned from Guy’s and St Thomas’ NHS Foundation Trust. The Regional Hepato-Biliary Cancer Service is delivered at King’s College NHS Foundation Trust. In an attempt to improve local delivery of care within these networks, advanced procedures such as oesophageal and pancreatic endoscopic ultrasound, and radio frequency ablation of hepatic metastasis is performed at Maidstone for much of the patient population in Kent. Access to patients from across Kent by road links, is very strong and additionally, the Kent Cancer Centre is based here. This long established Cancer Centre is the fourth largest in the country and manages patients from around the county.

THE TUNBRIDGE WELLS HOSPITAL

The Tunbridge Wells Hospital is located in Pembury adjacent to the A21. After a three year building programme, costing £230 million, the Tunbridge Wells Hospital at Pembury welcomed its first patients in January 2011. It is the first NHS hospital where all main wards are comprised entirely of single en-suite bedrooms. These enable patients to have greater privacy, sleep and reduce the chances of infection. Most rooms have plenty of natural daylight and views over the surrounding forest, which further aid a good recovery.

THE GASTROENTEROLOGY DEPARTMENT

There has been major investment in the Trust’s Gastroenterology Department with JAG- accreditation, 3 room Endoscopy Suites at the Maidstone Hospital and The Tunbridge Wells Hospital sites which make up a team of 8. Recent successful business cases have seen the additional introduction of high resolution endoscopic equipment and the appointment of Nurse Endoscopists (5 posts) and IBD Nurse Specialists (3 posts). This marks an exciting point in the development of the Gastroenterology Department and the Trust expects the successful candidate to reflect this dynamic and productive unit. The successful candidate will join a team of eight other Gastroenterologists providing local services across the Trust.

The post-holder will join Drs Maiden, Blaker, Sharma, Hanumanthraya, Bird, Baburajan, Barnardo and Dr Bell on either sites to support the care of patients on the Gastroenterology Ward. The clinical work can include endoscopy and outpatient sessions at either the Maidstone Hospital or the Tunbridge Wells Hospital or Hospital sites. The Gastroenterology service at MTW is comprehensive with – 1- IBD- 2 weekly IBD mdm’s with support of 2 IBD surgeons and 3 IBD nurses and 1 co- ordinator. There is also an existing link with St Thomas’s hospital IBD meeting for complex cases. 2- Liver- Regional viral hepatitis centre with 2 consultants managing this group and 1 specialist nurse. Once a month benign HPB mdm meetings. There is also a link to the tertiary centre benign liver meetings for complex cases. 3- Nutrition- There are weekly nutrition ward round for enteral and parenteral nutrition patients and an established nutrition steering group. The trust is also one of the very few centres where Flamingo buried bumper removal endsocopically is taking place. The nutrition team is well supported by a pharmacist, dietitian, catering team and senior nurse. 4- Endoscopy- A fully functional 24/7 gi bleed service is in place at the trust. This service is performed at the Tunbridge Wells site and has the back up of the surgical services and interventional radiology during working hours. Out of hours interventional radiology is provided by the neighbouring trusts if needed. The trust is providing all general endoscopy and also bowel cancer screening along with ERCP, EUS, Capsule endoscopy and enteroscopy. We intend to develop upper gi physiology which will complement an existing lower gi physiology studies. The trust has also been chosen for colonic capsule provision 2

and is also in collaboration with the CCG for using screening tools like QFIT and faecal calprotectin regularly which is being done successfully. 5- Cancer- There are existing upper, lower gi and NET cancer pathways with dedicated MDM for each of them. There is a direct link with St.Thomas’s and King’s hospital for the same. This service is ably supported by multiple nurses and co-ordinators and is a streamlined services which has been running over a number of years. 6- DDU- The trust is keen to amalgamate the gastroenterology and surgical specialities to one site at Tunbridge Wells. The process has begun with acute and specialist surgery moving to one site and plans of moving acute gastroenterology to Tunbridge wells as well. The intention is to develop a state of the art Digestive diseases unit with most of the work done in house as opposed to being outsourced. This service provision is in progress and is subject to stakeholders and consultation. The intention in the long term is for gastroenterology to relinquish general medicine and manage acute and complex gastroenterology patients with close proximity with the surgical teams. 7- The trust has recently approved a further 22 consultant posts in various specialities and is committed to a clinically led service. We hope to fill these posts in the next few years to increase the consultant body substantially. 8- The Acute Medical Unit- at Tunbridge Wells is staffed by 4 acute physicians who independently manage the unselected medical take between 08:00 to 18:00 and who complete morning post take ward rounds for patients in the A+E department or on the Acute Medical Unit admitted out of hours. This activity is support by a 1:14 Medical Consultant Rota. 9- The Maidstone site has a fully developed Acute Medical Unit staffed by 4 Consultant physicians who independently manage the acute take from 8:00 am to 9:00 pm, Monday to Friday. Weekend and evening takes are managed by a 1:16 Consultant Rota that currently includes the three Consultant Gastroenterologists.

SUPPORT FACILITIES

The post-holder will have facilities such as internet, intranet and EPR access to support clinical, administrative, teaching, CPD and audit activities at both sides of the Trust. The appointee will have appropriate secretarial support. Access to an experienced Consultant to act as a mentor to the appointee will be available if required.

JOB DUTIES AND RESPONSIBILITIES

1. We Aim to develop a GI physiology unit and expand of the EUS service which has been running at the trust. The post holder would look to develop either of the 2 services and would be fully supported by the trust for any additional training if it is required. If they have any other special interest in any other sub-speciality of gastroenterology this will be encouraged.

2. To provide a high standard of care to any patient for whom you have clinical responsibility. This will include any patients referred to you and cared for by you personally or by members of your team and, in addition, any patients of consultant colleagues when you are covering for leave.

3. To participate in clinical and other service activities and developments with the aim of ensuring a high standard of patient care across the Trust.

4. To work in close co-operation with other medical, professional and managerial colleagues, both within and outside the specialty, to provide high quality health care to the Trust’s patients, and to make a contribution to future thinking about the Trust’s strategic direction.

5. To participate fully in and share leadership of the clinical governance of the specialty covering its Trust-wide activities. To attend clinical governance half-days as detailed in the Trust’s Guide to Job Planning unless on agreed leave or while attending emergencies.

6. To participate actively in departmental audits, sharing co-ordination of audit activities within the department and implementation of change.

7. To take an active responsibility for undergraduate and postgraduate teaching, training and supervision.

8. To assume responsibility, both personally and corporately with consultant colleagues, for the management of junior medical staff. You are expected to be involved in their professional development, both clinical and personal as per guidance from the Deanery.

9. To ensure that all doctors in training for whom you have supervisory responsibilities undertake delegated clinical tasks that are within their level of competence and knowledge.

10. To ensure at all times that you comply with the General Medical Council’s published guide “Good Medical Practice” including the Duties of a Doctor.

11. To take personal responsibility for risk management in your work and undertake to review practices and learn from mistakes.

12. To provide prospective cover, for advice and to deal with clinical emergencies, during colleagues’ annual/study leave and for other absences as far as is practicable and clinically safe.

13. To ensure that you respond promptly and efficiently to all complaints related to any patient care you or your staff have been involved with, assisting the Trust’s corporate responsibility to reply to complaints within a specified time frame. 7

14. To ensure that you develop and maintain effective working relationships with those agencies concerned with the provision of health and social care services and those representing the interests of patients.

15. To undertake an annual appraisal leading to a personal development plan as outlined in the Trust Appraisal Policy. You must collect and maintain sufficient evidence to support the appraisal process and your GMC revalidation. The appraisal will inform the study leave and job planning processes.

16. To undertake an annual job plan review as outlined in the Trust Guide to Job Planning.

17. You are required to take reasonable care for the health and safety of yourself and others who may be affected by what you do while at work. You must adhere to Trust IRMER Regulations.

18. Specialty out-patient clinics as appropriate for the post-holder’s special interests

19. Involvement in the development of clinical management protocols shared with the community and other consultant colleagues.

20. Participation on the management and future development of the gastroenterology service.

JOB PLAN

Please note that this job plan is for illustrative purposes only and there may be a variation of the job plan subject to the mutual agreement of the post holder and the Divisional Director after appointment. This job plan below is subject to change when a GOW (gastroenterologist of the week) is put in place. The post holder initially will have a combination of both general medicine and gastroenterology inpatients with a view to moving towards a pure gastroenterology bed base care provision. This is subject to the DDU set up coming to fruition. The template below is an illustration of the likely work pattern prior to the conceptualisation of DDU and a more person specific plan will be developed keeping in mind the successful candidate.

The time commitment will be 10 PAs.

SPA time can be used for activities supporting professional development and the service which include teaching, audit, mandatory training, clinical trials recruitment and additional responsibilities such as a clinical lead role. It is anticipated that any new consultant to the Trust will take on more managerial, educational and other responsibilities with time and hence it is anticipated that 2.5 PA will be developed on the job plan within two years of appointment.

The post will have 1PA for admin. There is also admin time incorporated in the clinic. The clinic templates would be 30 minutes for new and 15 minutes for follow up patients.

The post holder will be on the on call rota for both General medicine and GI bleed on call. This has an allocated 1.28 PA’s. The GIM on call will 1 in 5 Wednesdays. This will require an evening ward round of the admissions from 17:00 to 21:00 hours and 1 hour the next day morning. The weekend General medical on call is 1 in 52 weekends and has 5 ward rounds on the weekend and a Monday morning 1 hour post take ward round. The 5 ward rounds during that weekend would be the Friday evening ward round of 4 hours and 4 hour ward round on Saturday and Sunday morning and evening.

The GI bleed on call is 1 in 9 for out of hours endoscopy. This runs in sets of Monday to Thursday weekdays and Friday to Sunday weekends. In the circumstance of overnight endoscopy the next days endoscopic clinical activity will be back filled by other consultants or nurse endoscopists. The outpatient clinic activity will cancelled at a short notice and the ward will be covered by other gastroenterologists.

The post holder will get in lieu time off for working on bank holidays if he needs to be present in the hospital providing direct clinical care.

The gastroenterology ward is ward -12. The post holder will be responsible for 8 inpatient. Gastroenterology also covers 2 outlying wards and those patients will be divided up between all

9 the gastroenterology consultants. The ward round have dedicated 4 hours allocated to it. The post holder will provide 2 full ward rounds a week at least.

The post may be required on occasions to work cross site. A ½ hour provision as travel time will be given for the same. Although cross site working of half day sessions is not anticipated.

There are many multi-disciplinary meeting which happen at the trust and depending on the post holders sub specialty interest additional SPA will be allocated for their presence in these meeting. The meeting range from IBD, benign liver, upper and lower gi cancer MDM’s.

The job plan including the use of SPA time will be subject to annual review.

CLINICAL GOVERNANCE AND AUDIT

The Trust strongly supports clinical governance and risk management and expects all senior medical staff to contribute fully to these activities, and actively work towards and to maintain high quality clinical care. Full involvement of individual patients in decisions about their care is expected, as is the need to involve patients and the public in service change. Guidance and support is given to clinical directorates in the development of their programmes. There is an active clinical audit department.

You are expected to take part in the processes for monitoring and improving the quality of care provided to patients, including risk management and clinical audit. Team and individual performance data are being developed and you are expected to review your practice accordingly. The commitment of all staff to clinical governance is assisted by a monthly compulsory Trust-wide half-day for structured clinical governance activities. Consultants are expected to attend and lead clinical governance activities during the half-days and you will be held accountable for this at your annual appraisal.

EDUCATION AND RESEARCH

You are expected to maintain a proactive interest in life-long learning and engage in continuing professional development. You are required to maintain your CME status as mandated by the Royal College of Physicians and maintain competence in your specialist fields of interest.

Postgraduate centres at Maidstone and Tunbridge Wells Hospital are under the leadership of the Director of Medical Education, Dr Garth Somerville, and the clinical tutors, Dr Bet Mishra (Maidstone) and Dr Derek Harrington (Tunbridge Wells). The centres provide excellent accommodation for a range of postgraduate activities. There are good medical libraries with Internet access.

Clinical research is encouraged. If you engage in clinical research you must follow Trust protocols and ensure that the research has had local ethical approval. There is an active Research and Development Committee.

JUNIOR STAFF TEACHING AND SUPERVISION

You are expected to contribute to the teaching and supervision of all training and career grade junior staff and undergraduate medical students within the department and also provide teaching to other professional groups within the Trust. You are expected to be involved in the professional development, both clinical and personal, of staff for whom you have responsibility. With consultant colleagues you are expected to assume responsibility, both personally and corporately, for the management of junior medical staff. You must ensure that all doctors in training for whom you have supervisory responsibilities undertake clinical tasks that are within their level of competence and 10 knowledge. You will be expected to complete the KSS Deanery Certificate in Teaching and Certificate in Educational Supervision, or equivalent qualifications, within 18 months of taking up your post.

APPRAISAL

It is a contractual requirement that each consultant working within the Trust is appraised on an annual basis. Appraisal is a positive exercise designed to assist doctors in their educational and professional development and aims to improve and consolidate existing good practice. Sufficient evidence must be collected and maintained to support the appraisal process and your GMC revalidation. The appraisal system for consultants is as outlined in the NHS Advance Letters (AL(MD)6/00 and AL(MD)5/01, and as detailed in the Trust’s Annual Appraisal Policy and Procedure (2008).

LEAVE MANAGEMENT

You must co-ordinate your leave arrangements with those of your colleagues to provide a clinically safe environment. In respect of clinical cancellations etc this means giving at least six weeks’ notice of intended annual and study leave. The Clinical Director must agree all leave. Leave should be booked with Division management and taken in accordance with Trust Policy. The Trust supports study leave with expenses for CME activities and within agreed financial limits. Leave management within the specialty should ensure adequate and safe cross cover arrangements with colleagues. Special professional leave can only be taken with the prior permission of the clinical director and with due notice and cover arrangements.

INFECTION CONTROL

As an employee of the Trust you are required to be familiar with, and comply with, Trust policies for infection control and hand-hygiene in order to reduce the spread of healthcare-associated infections. For clinical staff with direct contact with patients this will include compliance with Trust clinical procedures and protocols, including the uniform and dress code (‘bare below the elbows’), the use of personal protective equipment policy, safe procedures for using aseptic techniques and safe disposal of sharps. All staff are required to attend mandatory training in Infection Control and be compliant with all measures known to be effective in reducing healthcare-associated infections.

Trust Consultants in their position as clinical leaders are expected to lead on infection control within their clinical areas and to set an example for other Trust employees and the public.

TRUST POLICIES AND MANDATORY TRAINING

All Trust employees should be familiar with and comply with Trust policies. These are available on the Trust intranet. Consultant staff are required to remain up-to-date with statutory and mandatory training. The Director of Medical Education and the Head of Learning & Development will provide information and organise training much of which can be completed online.

SAFEGUARDING CHILDREN

Everyone employed by the Trust regardless of the work they do has a statutory duty to safeguard and promote the welfare of children. When children and/or their carers use our services it is essential that all child protection concerns are both recognised and acted on appropriately. You have a responsibility to ensure you are familiar with and follow the child protection procedures and the Trust’s supplementary child protection guidance which is accessed electronically on the Trust's Intranet site. You have a responsibility to support appropriate investigations either internally or externally. To ensure you are equipped to carry out your duties effectively, you must also attend child protection training and updates at the competency level appropriate to the work you do and in accordance with the Trust's child protection training guidance.

SAFEGUARDING ADULTS

Everyone employed by the Trust regardless of the work they do has a duty to safeguard and promote the welfare of vulnerable adults. When patients and/or their carers use our services it is essential that all protection concerns are both recognised and acted on appropriately. You have a responsibility to ensure you are familiar with and follow Trust policies in relation to safeguarding vulnerable adults. You have a responsibility to support appropriate investigations either internally or externally. To ensure you are equipped to carry out your duties effectively, you must also attend vulnerable adult protection training and updates at the competency level appropriate to the work you do and in accordance with the Trust's vulnerable adult protection training guidance. 12

TRUST MEDICAL MANAGEMENT

As a senior member of staff, you will be expected to contribute to the management of the Trust. The main operational decision-making body of the Trust is the Division Operations Committee which meets monthly, chaired by the Chief Operating Officer. The Divisional Directors are members of this committee and actively contribute to the Trust’s management. The Medical Staff Committee, a meeting of all Trust consultants and associate specialists, is held monthly and provides a forum for discussion of a wide range of work related matters.

IT SKILLS

The Trust uses Microsoft Office and NHS.net and uses email as a means of cascading important information and for internal communication. It is expected that consultant staff should have basic IT skills and be familiar with the use of emails, Outlook, Word, Excel and Powerpoint.

NOTICE PERIOD

You are entitled to 3 months notice and have to provide the Trust with 3 months notice of any termination of contract.
