Cwmbran House Care Home

Provides residential care for 56 older persons (aged 60+) in 3 separate living areas. Two areas provide accommodation for physically dependent older people, and one area is specifically for those with dementia care needs.

Cwmbran House also offers both respite and long term accommodation to adults who have been assessed by a Care Manager of Torfaen Social Services as needing residential care.

The home is not registered to provide nursing care.

Our staff and model of care

At Cwmbran House, we provide an excellent 24 hour care package through a combination of a well motivated, trained staff who will confidently assist to meet the care needs of our residents.

Staff training is a high priority for Cwmbran House and we ensure that all our workforce have the skills, values and behaviours to deliver a professional and dedicated service to those in our care.

Hafod Care are working with Helen Sanderson Associates in developing a person centred model of care across all its services with a focus on personalised care and support which maximises wellbeing outcomes. We are working with our residents, relatives and staff to design one page profiles detailing what people appreciate about me, what’s important to me and how best to support me. Knowing this information, we are able to deliver care in a much more personalised way.

For each resident, a full assessment of needs is undertaken in conjunction with statutory services or completed privately, to develop a personalised plan of care to ensure that their needs are identified and met.

Accommodation Lounges and Dining Rooms There is a communal dining area and lounges situated in the centre of Cwmbran House with generous space for activities and a day room. Two areas have their own facilities, lounge/dining areas and kitchen. One area contains 3 smaller kitchenettes/dining areas. All lounges have comfortable seating with access to TV, DVD and radio. Gardens There is a central court yard with space to sit & relax, plus an outside sitting area over looking the pond. Comfortable Bedrooms The majority of bedrooms at Cwmbran House have en- suite facilities and are fully furnished with a wardrobe, lockable chest of drawers, bed and bedside cabinet. Each room is fitted with room call systems and each resident can be given a neck pendant if they feel they want to wear one. Other Facilities Each area has a specially adapted bedroom with an overhead hoist and integrated bathroom, domestic laundry rooms and generous storage space.

Accommodation Catering Meals are prepared in our high-tech kitchen to a consistently high standard and a wide choice of food is available at each meal time, ranging from hot food to salads and snacks. Menus reflect seasonal trends and are published each fortnight. Individual and specialist diets can be catered for by our experienced and professional kitchen staff.

Additional Services Cwmbran House staff can arrange visits from doctors, nurses, chiropodists, dentists, solicitors, hairdressers and beauticians as well as other professionals. Some of these may incur additional costs to service users. Daily newspaper deliveries can also be arranged.

Activities

Cwmbran House is complimented with an activities organiser who arranges group and bespoke activities for our residents that aim to achieve greater stimulation and wellbeing.

There is a regular programme of planned activities available to all residents, including: • social evenings, quiz nights, singalongs, barbeques, cards & games • interest groups and reminiscence sessions • Local artistes also visit and perform

Hafod Care is a charitable, not for profit association which provides a wide range of housing, care and support to over 1000 people each year across South Wales.
