Madrid is perfect for walking. Some of its parks are true green lungs, like El Retiro and Casa de Campo, while others are almost hidden and secret, such as El Capricho, Quinta de los Molinos and Cerro del Tío Pío, the latter with one of the best views over the city. Download by clicking on http://www.esmadrid.com/sites/default/files/documentos/mapabloc_mayo_2016. pdf Transport from the airport to city centre Adolfo Suarez Barajas Airport (MAD) is one of the world's most busiest Airports (50 million passengers a year) and over the years, has become an important hub between Europe and South America. Despite that, Madrid-Barajas is easy to navigate and PAGE 1 pleasantly efficient. The Airport is 12km away from the city center but well connected by metro, bus, commuter trains and taxi. Barajas Airport consists of four terminals of which three are linked together through mechanical corridors. However, the new Terminal 4 is 2 kilometers away. Anyway, there is a free shuttle bus (24/7) connecting the different terminal buildings. Taxi Madrid airport disposes of several public taxi ranks, distributed over all airport terminals. They are located at: • Terminal T4 floor 0, Arrivals area, • Terminal T4 floor 2, Departures area, • Terminal T1 floor 0, Arrivals area, • Terminal T1 floor 1, Departures area, • Terminal T2 floor 0, Arrivals area, • Terminal T2 floor 2, Departures area, • Terminal T3 floor 0, Arrivals area. Fares The final price for your ride should be indicated by the taximeter. We therefore recommend that you make sure prior to the beginning of your ride that the driver has switched the taximeter on, indicating the basic fare. You can always ask for an invoice, once you arrive at your destination. The price for a taxi ride from the Barajas airport to the centre of Madrid is a flat rate 30 € one way, as taxi fare 4. The luggage is included for any fares which apply. Basic fares Fares 4 (flat rate from the airport): 24 hours every day/year for services from / to the airport and from / to any destination within the boundaries of 30st street (bypass road M-30), including those rides that were contracted via telematic means. Fare 1: Monday to Saturday, from 07:00 a.m. to 09.00 p.m.. Fare 2: every day, from 09:00 p.m. to 07:00 a.m, and for Saturdays, Sundays and public hollidays from 07:00 a.m. to 09.00 p.m.. Fare 3 or minimum fare to the airport: 24 hours every day/year for routes to a destination under 10 km from any airport terminal. Once exceeded the 10 km (or equivalent initial time), the taximeter will continue counting at the corresponding fare rate. For journeys from the airport that have been booked online or by phone, this fare will not be applied. Buses Airport express. It has direct connection between Atocha (Cibeles at night) and Barajas Airport 24 hours a day 365 days a year. Buses run every 15-20 minutes (daytime service) and every 35 minutes (night time service). Madrid Underground Madrid's Barajas Airport has Metro stations in Terminal T2 and the newer Terminal T4. Trains leave every 5 minutes from 6.00 am to 2.00 am. Line 8 goes straight to Nuevos Ministerios Metro station in the centre of Madrid, with a journey time of just 12 minutes. You can buy a single journey ticket to/from the airport which includes a supplementary airport charge. PAGE 2 Consider buying the Madrid Tourist Travel Pass, which allows you to travel on any Metro, bus or suburban train, making it a great way to save money on public transport. With this Travel pass, it is not necessary to pay the airport surcharge. It is also possible to purchase a 10-trip "Metrobus" ticket, valid for both the Metro and city buses, since you will undoubtedly be using the Metro again during your stay. This may be purchased at newspaper outlets, "estancos" or tobacconists and Metro stations. Note that you will still need to purchase the supplementary ticket for any journey to/from the airport. STEP 1: Complete Form CBM-DCL for both the projected actual and budget years in the following manner: (A) Calculate the cost-to-charge ratio for the respective year by entering the total gross patient revenues (CBM-5, line 1, column A) in the first cell of the CBM-DCL template, and the total operating expenses (CBM-5, line 6, column A) in the second cell of the template. The template will calculate the hospital’s cost-to-charge ratio in the third cell. These revenue and expense amounts must match the respective CBM-5 forms; (B) Calculate the hospital’s volume threshold for each year by entering the respective total nongovernmental discharges (CBM-1, Nongovernmental Column, Line 3 or Line 6) in the template cell labeled “Inpatient”, and the total nongovernmental visits (CBM 1-B, Nongovernmental Column, Line 9 or Line 18) in the template cell labeled “Outpatient.” The template will calculate the hospital’s five (5) percent volume threshold.
