Subject: 10CFR21 Repertable Data .

Reference: Phone conversation of 8/18/83

Dear Mr. Gregor: In accordance with 10CFR21, VICT0REEN, INC. shall use this letter to formally document our phone co'nversation of 8/18/83 as well as transmit our written report. On 8/18/83, your department was notified by phone . of a potential defect involving the marking of the 8-Bit (MPU's) Family. The defect involves the letter designa- tion utilized on the Hi-Re1 products which was changed to indicate tem- perature classifications of the microprocessor. What follows is the written report required as per 10CFR21 Paragraph 21.21 b2 and b3.,

At the time this letter is being generated, a full and complete review has not been performed because of time requirements involved in complying with 10CFR21. To the best of our knowlei :e and belief, as a result of our investigation th'us far, only the following facilities have

received the products in question. . Construction Firm Facility

Bechtel, Inc. Midland Units 1 & 2 Consolidated Edison Indian Point ,

The firm witch supplies the M6800 products is: Motorola 3501 Ed Bluestein Blvd. Austin, Texas 78721

. Ref: B3.iv) The nature of the potential defect involves the changing of part number designations by Motorola, in September 1980. This change was effective with code date 8035. In the 1981 version of the Motorola Microprocessor handbook the version of the 6800 that VICTOREEN wanted to use was MC6800BQCS. This was a IM Hz 8-Bit Microprocessor tested in accordance with Mil Standard 883 with a temperature range of -55* C to +125* C. According to the Motorola Semiconductor Master Selection Guide and Catalog, also dated 1981, a MC6800BQCS was a 8-Bit IM Hz Microprocessor with a temperature range of O' C to 70* C. This type of part number six up is also present in 6850 and 6820 chips that are also used in our Digital Radiation Monitoring System.

As a result of conflicting information put out at the same time for the same part number, VICTOREEN utilized what we though6to be MC6800BQCG components rated at -55* C to +125* C, but are in actuality rated O' C - to +70* C. Due to the temperature extremes involved, premature failure or complete failure could result. This failure could potentially create a condition in which releases of radioactive materials may go unde- - tected. !

Ref: B3.v.) VICTOREEN, INC. received~ documented information from Motorola on August 16, 1983 which led to our investigation of the potential defect. .

Ref: B3.vi.) Please refer to Paragraph B3.ii) of this letter which states which facilities and number of units may contain the potentially defec- tive devices. What follows is a list of VICTOREEN, INC. model numbers ! which utilize the Motorola devices.
