Student Health Requirements
All students (new and transfer), entering St. George’s University must submit a completed health form and required tests and vaccines prior to first-term registration.
The following documentation is required as a condition of matriculation and must be kept up-to- date:
For MD Students
History / Physical exam within 6 months prior to registration Tuberculosis Skin Test (TB) - TB Screening within 6 months prior to registration Tdap (Tetanus, Diphtheria Acellular Pertussis) - Proof of Tdap specific vaccine received within 5 years prior to registration Measles, Mumps and Rubella (MMR): Serum IgG titers for Measles, Mumps, Rubella and Varicella are required . Any non- immune results require additional vaccine(s) as indicated . Copy of personal immunization record should be submitted Completion of the Hepatitis B series (3 vaccines) . Students must complete the first 2 vaccines prior to registration . The 3rd vaccine may be completed at the Student Health Center on campus . Students must have completed a Hepatitis b surface antibody test upon completion of the 3 vaccinations; alternately, a student may provide evidence of a positive Hepatitis b Core antibody test (if appropriate) Meningococcal Meningitis Vaccine . Students may review information regarding this vaccine at www.cdc.gov/vaccines/vpd/mening/index.html . Students must submit proof that they have received the meningococcal vaccine within the 5 years prior to 1st term registration . Completed health form to be submitted to the designated Term One email account in the following manner:
o Scan health form and all supporting documents into PDF files and email to: [email protected]
o Do not send any health documents to your Admission Counselor or other SGU representative If a student has any questions related to completion of the health form, not answered in the informational memo sent email Susan Conway RN at [email protected]
For DVM Students
History / Physical exam within 6 months prior to registration Tuberculosis Skin Test (TB) - TB Screening within 6 months prior to registration Tdap (Tetanus, Diphtheria Acellular Pertussis) - Proof of Tdap specific vaccine received within 5 years prior to registration Measles, Mumps and Rubella (MMR) - Proof of 2 complete vaccinations for Measles, Mumps, Rubella Varicella: Students may satisfy this requirement by submitting one of the following: . MD statement that they have a history of Chicken Pox . Lab copy of IgG titer for Varicella indicating immunity Completion of Pre-Exposure Rabies Series . This must be completed before the end of Term 1 in Grenada . Proof of continued Rabies immunity by Rabies Serology showing immunity or proof of additional Rabies vaccination every 2 years . The pre-exposure Rabies vaccinations are available at the University Health Services clinic on campus . Students who are participating in the SGU sponsored WEA insurance plan can receive the Pre-Exposure Rabies vaccine series at the University Health Services clinic on campus at no cost Meningococcal Meningitis Vaccine . Students may review information regarding this vaccine at www.cdc.gov/vaccines/vpd/mening/index.html . Students must submit proof they have received the Meningococcal Meningitis vaccine within the 5 years prior to 1st term registration Completed health form to be submitted by scan and email to [email protected] Any questions related to completing form should be sent to Susan Conway RN at [email protected]
For SAS Students
History / Physical exam within 6 months prior to registration Tuberculosis Skin Test (TB) - TB Screening within 6 months prior to registration Tdap (Diphtheria Acellular Pertussis) - Proof of Tdap specific vaccine received within 5 years prior to registration Measles, Mumps and Rubella (MMR) - Proof of 2 complete vaccinations for Measles, Mumps, Rubella Varicella: Students may satisfy this requirement by submitting one of the following . MD statement that they have a history of Chicken Pox . Lab copy of serum IgG titer for Varicella indicating immunity Meningococcal Meningitis . Students may review information regarding this vaccine at www.cdc.gov/vaccines/vpd/mening/index.html . Students must complete the Meningococcal Meningitis vaccine within the 5 years prior to Grenada arrival. Completed health form to be submitted by mail to: St. George’s University School of Arts & Science St. George’s Grenada, WI Alternately, health form and related documents may be submitted in person upon arrival in Grenada
