Welcome to the Cross Countes Healthcare Primary Care Network Health and Wellbeing Team newsleter. Primary Care Networks (PCNs) are a natonal initatve set up in July 2019 to create groups of GP practces in local areas allowing them to work more closely together and with the other health, social care and community groups in the area. This collaboratve working will produce care that is more pro- actve and accessible, whilst address gaps in services. Your local PCN health dew wellbeing team is made up of social prescribing and health and wellbeing coaching. What is a Health Coach?

Health coaching is a patent-centred process that provides personalised lifestyle support, helping patents to increase actvity levels, improve eatng habits and maintain the long-term changes they need to improve their health and wellbeing through developing accountability in health behaviours.

What are the benefts of health coaching?

Following a period of health coaching, clinicians report patents are more able to self-manage their lifestyle and health behaviours, long term health conditons, and medicatons. People may also show improved outcomes in mild mental health difcultes, adherence to medicaton, and positve efects on other health issues including falls.

Who is it for?

Health Coaching is for individuals who require help with making lifestyle changes and those who could beneft from making healthier lifestyle choices. This person may feel unmotvated or may have atempted to make lifestyle changes in the past but has been unsuccessful. Lifestyle changes may include: · Healthy eatng · increasing actvites · reducing smoking or alcohol use · managing stress Page 5 of 8

Health and Wellbeing Support

Wellness Exercise Zoom Session/Wellness Walks with HDC Council has a number of Zoom sessions available to support us with our physical and mental wellbeing. They involve some gentle stretching, exercise and mindfulness. They are currently collectng interest for the idea of a Wellness Walk in Harborough. The aim of this walk is to improve our health and mental wellbeing by walking outdoors in the fresh air with others. It will be run by a qualifed walk instructor from Harborough District Council. This is aimed at anyone over the age of 18yrs who may be feeling isolated, lonely, with low level mental health needs. It is an inclusive walk, wheelchair friendly and everyone is welcome. Winter Wellness Sessions on Zoom
