We reached another milestone, celebrating the giving of our 10,000th grant GAYLE WOODIS Executive Director in September. The Foundation couldn’t be more pleased, or proud. The Foundation has also implemented many new outreach programs to Legal Counsel “spread the word” so we can help even more children – with a goal of awarding and Secretary 2,100 grants nationwide in 2016.

A special thank you to everyone who contributed to the Foundation during the Giving Campaign. We couldn’t do this without your support. Over 9,000 employees across the UnitedHealth Group enterprise raised nearly $1.7 million for the Foundation. Matched dollar-for-dollar by UnitedHealth Group, the campaign will fund more than $3.3 million and will generate more than 1,000 UHCCF grants for children and families in 2016.

With great support from our regional and local markets, UHCCF hosted 13 regional awareness and fundraising events during 2015. Notably, our annual flagship Golf Classic at Hazeltine fundraiser was once again a huge success – raising more than $1 million in donations for UHCCF. Fundraising bike rides, golf tournaments and other events also contributed much-needed donations for our mission.

We are grateful to all the board members, employees, team members, volunteers, supporting partners and corporate sponsors who donated their time, talents and generosity to the Foundation. While we are so proud of all that we have done, We know the true heroes are the children who benefit from UHCCF medical grants. Together we DO change lives.

UHCCF.ORG | 3 Culture Ambassadors Participate in the 10,000th Grant Celebration To celebrate the 10,000th UHCCF grant, UnitedHealth Group Culture Ambassadors held more than 20 social responsibility events in September and October to donate “Little Book - Big Laughs Joke Books" across the country. One such event included more than 100 ambassadors who assembled kits including brochures and joke books. Volunteers delivered books to organizations nationwide such as Make-A-Wish, Ronald McDonald House, Read to Succeed, Project Sunshine and other children’s hospitals, public schools and clinics.

Volunteerism, children and smiles are what drive UHCCF's mission.

Facebook Likes Grow to More than 10,000! The UnitedHealthcare Children’s Foundation’s Facebook page has been an integral part of the organization’s communications. 2015 ended by surpassing 10,000 likes, which is great growth for the UHCCF mission. UnitedHealthcare Children’s Foundation reached a new milestone in September, awarding the 10,000th grant to a Florida family to help pay for updated hearing aids for their 5-year-old twin boys. The boys, Cole and Jake, were diagnosed with hearing loss at just over a year old and have been wearing hearing aids ever since. “We were thrilled to hear that we received a grant from the UnitedHealthcare Children’s Foundation that would help each of the twins. The grant has enabled us to purchase hearing aids with more updated technology, and we are already seeing the boys respond to sounds they weren’t hearing previously. We can’t wait to watch as they continue to enjoy the activities they love so much – soccer, the beach, playing with their older brother – but with better access to the sounds around them.” Said parents Peggy and Eric.

The UnitedHealth Group Employee Giving Campaign reached employees across the country in 2015. Over 9,000 UnitedHealth Group employees pledged more than $3.3 million to UHCCF, which includes the UnitedHealth Group match.

UnitedHealth Group employee donations count for nearly two-thirds of UHCCF's grant funds each year, proving that UnitedHealth Group employees are vital to maintaining and growing the Foundation.

The UnitedHealth Group Giving Campaign also allows the Foundation to share UHCCF's story and make a direct connection with the greater UnitedHealth Group family. We are incredibly grateful for the ongoing generosity of UnitedHealth Group employees. Together we DO change lives.

JACKSON // AGE 6 // NEWARK, OH Jackson loves the great outdoors. He also has spinal cord neuroblastoma which causes decreased motor function – but he doesn’t let that slow him down. His grant provided intensive therapy, making it possible for him to get out of his wheelchair and enjoy nature with his walker.

6 | ANNUAL REPORT 2015 TOGETHER WE DO CHANGE LIVES To encourage early reading and healthy living among children, UnitedHealthcare donated UHCCF-branded reading stations, Oliver & Hope books and joke books to community-based organizations and clinics in California and Nevada. A group reading event highlighted each reading station donation. UnitedHealthcare employees joined children and families, community leaders, local elected officials and UnitedHealthcare mascot Dr. Health E. Hound for a reading circle featuring the UHCCF Oliver & Hope books.

In December, UnitedHealthcare also donated UHCCF books to more than 100 Las Vegas–area schools as part of an initiative to encourage early literacy and spur students to continue reading Oliver & Hope were on hand for reading stations, live book readings and other literacy events. over the holiday break.

UHCCF.ORG | 7 “Cheers” to Fashion and New Friends In 2015 UHCCF held events that catered to audiences across the country. More than 1,200 UHCCF supporters gathered at downtown luxury hotels in Chicago, Boston, Minneapolis and Austin to enjoy fine wine and hors d’oeuvres, local vendors, silent and live auctions, and shopping marketplaces that included Max Martin designer shoes. Haute couture fashion shows were presented by Tracey Tarantino of Zzazz Productions and featured fashions from Project Runway designer Peach Carr, among many other designers and special guests. “It was simply fabulous,” said many of the event attendees.

8 | ANNUAL REPORT 2015 UnitedHealthcare Pro Cycling Team Rolls for a Good Cause

The UnitedHealthcare Pro Cycling Team has been an ongoing partner of UHCCF to help raise funds and awareness for the Foundation. Each year the team dedicates one important race to honor a UHCCF grant recipient and donates the prize money earned to the Foundation.

The 2015 race was the Littleton Criterium in Littleton, Colorado, and featured two UHCCF grant recipients, brothers Daniel (11) and Tristan (9), who were both diagnosed with genetic hearing loss. UHCCF was able to assist with the expense of hearing aids for both boys. “Pros for the Day” Daniel and Tristan.

In addition, the team contributed a total of $60,000 in cash and in-kind donations to UHCCF in 2015, participated in the UHCCF Velo & Vines Century Ride in Healdsburg, California, and the Minneapolis Century Ride in Eden Prairie, .

ABBY // AGE 2 // WHITE BEAR LAKE, MN Abby has a personality as colorful as the pictures she draws. She is also a brave girl who endures blood transfusions for Diamond Blackfan Anemia, and faces the challenges of having developmental hip dysplasia. Abby’s grant helped her transition from wearing a brace to now being brace free. According to her parents, Abby runs around today like every 2-year-old should.

UHCCF.ORG | 9 CYCLING THROUGH THE YEAR Smooth Ride at 1st Annual Velo & Vines Century Ride 28 cyclists met at the Arnot-Roberts Winery in Healdsburg, California

in June to embark on the 100-mile Velo & Vines Century Ride. The riders were joined by supporters and sponsors from across the nation along the rolling hills of the Alexander, Dry Creek and Russian River area for the inaugural tour. The ride, hosted by Three Rivers Provider Network and sponsored by Arnot-Roberts Winery, raised approximately $45,000 to fund future UHCCF medical grants for children. At each rest stop along the route, riders experienced artisan foods and local music. The cycling tour ended with a post-ride awards ceremony and marketplace featuring local vendors, wine tastings and a silent auction.

Healdsburg resident and UHCCF Board Member Jennifer Roberts hosted the event and was joined by such notables as Lucas Euser, a UnitedHealthcare Pro Cycling Team rider. Lucas helped make the day a genuine treat for cycling enthusiasts and race fans. We look forward to the 2016 Velo & Vines ride which will be held on June 11 in Healdsburg, California. 10 | ANNUAL REPORT 2015 4th Annual Minnesota Century Ride Wheels in Nearly $300,000 Heat and humidity didn’t stop dedicated cyclists from completing the 4th Annual UHCCF Century Ride in Minnesota. Bicyclists from UnitedHealth Group, UnitedHealthcare, , sponsors, other business partners and UHCCF supporters rode the 100-mile trek to raise money for UHCCF. Beginning with only eight riders in 2012, this event has grown to more than 75 cyclists in 2015. Together they raised a record amount to help support children and families in need of medical grants. That’s up more than $100,000 from the previous year! Special thanks to Jeff Meyerhofer and the Minnesota planning team for making this event something we look forward to each year. Congratulations riders, and well done!

This year’s 5th annual ride will be held on Sunday, July 24, 2016 in Eden Prairie, Minnesota. The goal is to raise substantially more than last year! Are you up for the challenge?

12 | ANNUAL REPORT 2015 Grins2™ Program Delivers Reasons to Smile As part of the 2015 Holiday Fundraising campaign, UHCCF heavily promoted its Grins2 program in Where are November. The program has launched at hospitals across the country and simultaneously benefits two children with each and every bag that is distributed. The program will expand to include Ronald McDonald Houses in 2016. One child receives a dose of laughter and inspiration through the gifts, and a second child is impacted by making a UHCCF medical grant a reality. In 2015 the number of bags purchased nearly doubled compared to 2014. More than 330 bags were distributed to children in need of a smile at 110 hospitals nationwide during December. UHCCF is grateful to all who participated in this program and reminds everyone: This program is not seasonal; a smile is a welcome gift any time of the year! UHCCF.ORG | 13 Kids Benefit from Four UHCCF Golf Classic Events UHCCF supporters and corporate sponsors at Hazeltine National Golf Club (Chaska, MN), Manor Country Club (Rockville, MD), TPC Boston (Norton, MA), Lake of Isles (North Stonington, CT), Florida and should be very proud of their contributions to UHCCF and the many children who have benefitted. Together these events raised more than $1.5M for the Foundation. Each event was well-attended by golfers enjoying the legendary courses, camaraderie, food and special activities. Special grant families were present at each event to share their stories as well.

14 | ANNUAL REPORT 2015 Meet a Hero! ZI // AGE 6 AUBURNDALE, FL “Zi” is short for Elizabeth, but this young girl is not short on creativity and energy! She loves to play chef in her pretend kitchen and always has a recipe for fun when playing with dolls and stuffed animals with her siblings.

Diagnosed with anorectal malformation, Zi struggled with bowel control. But a UHCCF grant allowed her to receive specialized therapy at a bowel management clinic and today Zi no longer requires diapers.

Zi’s improvements have helped her to live a more normal and confident young life. She’s very enthusiastic about school and talks about one day becoming a doctor, which would make the world a healthier, happier place.

Fishing Tournament Eager anglers gathered under blue skies for the first annual Florida Fishing Tournament. Sixty generous attendees participated at Sanibel Harbor in Fort Myers on May 1. In addition to catching fish, the crew reeled in $60,000 toward future UHCCF medical grants. Thank you to the generous volunteers who helped make the tournament a success – especially Patrick Stamm ( of UnitedHealthcare Shared Services) whose energy and leadership was instrumental in planning the event. Also, congratulations to the tournament champions: Team Platfora! UHCCF looks forward to another great fishing event in 2016. Registration for the 2016 event is open at UHCCF.org.

Kickin’ It for Kids The 3rd Annual UnitedHealthcare “Kickin’ It for Kids” Kickball Tournament in Seattle, Washington was a huge success. This wellness-focused event held on July 15 brought various UnitedHealthcare departments, partners and communities together. More than $10,000 was raised for UHCCF. Thank you to the Washington planning team connecting supporters in such a fun and creative way!

New York Rooftop Event a Hit! On a beautiful evening in May, the UnitedHealthcare Employer & Individual New York Healthplan hosted their first Rooftop Event on the 54th Floor of New York’s Hyatt . Proceeds from the ticket sales support UHCCF programs, making Tickets for Hope a sustainable component for the Foundation’s fundraising strategy. Generous ticket donors purchased tickets to events nationwide and grossed more than $26,000 in donations for UHCCF in 2015. BRYNN // AGE 2 // SARTELL, MN When Brynn’s parents learned that she had infant hip dysplasia, they were very stressed. Brynn needed hip ultrasounds and a pelvic harness – which represented both an emotional and financial burden for her folks. But Brynn’s UHCCF grant helped ease that burden, and allowed mom and dad to enjoy being happy parents to a newborn. Today Brynn is doing great. She loves going on long walks with her dog Mace, and can’t get enough of swimming, biking and running through sprinklers.

UHCCF.ORG | 19 Oliver & &Hope Hope® Storybooks® Storybooks – – The Adventures Continue… First came "Oliver & Hope’s Amusing Adventure." Then, in 2014, the friends returned in their second award-winning storybook "Oliver & Hope’s Adventure Under the Stars." This heartwarming tale received critical acclaim, including a Mom’s Choice Silver Award. In 2015 "Oliver & Hope’s Superhero Saturday" joined the series and showed readers that anyone can be a hero. The Oliver & Hope 3-book series has led to many other products from UHCCF, including plush animals and a Spot it! game. Proceeds from these products have raised thousands of dollars toward new UHCCF medical grants. UHCCF has released this series in paperback format in 2016. What Has a Spine, but No Bones, and Will Make You Laugh? Answer: Two New “Little Book - Big Laughs” books! These creative comedy collections will entertain readers with hundreds of funny jokes and riddles. Both the “Little Book - Big Laughs Riddle Book” and “Little Book - Big Laughs Knock-Knock Book” feature humor submitted by kids for kids. Their jokes will hit the funny bones of grownups and kids alike.

The “Little Book – Big Laughs” books were compiled by Meg Cadts, author of the award-winning Oliver & Hope storybooks. The new riddle and knock-knock books are two of three in the “Little Book – Big Laughs” series. All are available for purchase on Amazon for $5.99 each and will be available in a boxed set in 2016. UHCCF invites customers to make an even bigger difference by ordering several copies to give as gifts, because everyone enjoys receiving the gift of laughter.

“It was the greatest day of our lives when we were blessed with Nicholas’ grant for hearing aids. His hearing loss is 55% bilaterally, so they were necessary for his success in school, Boy Scouts and sports – which he loves. When he grows up, Nicholas plans on becoming an engineer. One of the things he wants to build is a roller coaster!”

“Our son Kasper experienced hearing loss resulting from his cancer treatment. The costs of the hearing aids and FM system he needed weren’t covered. So receiving a UHCCF grant for Kasper’s hearing aids was a gift. Today he’s able to learn and participate in school in ways that wouldn’t have been possible without the gift of hearing. Thank you UHCCF for stepping in when no one else would.”

“Treston has Down Syndrome. Our insurance wouldn’t cover the orthotics he needed for his feet, and Treston also needed physical, occupational and speech therapy to ensure his progress. We had some money to pay for a 5-day therapy camp, but needed assistance there too. Since receiving his grant, Treston walks better, is more confident and he is thriving! He loves books, music and swimming with his big brother. Those two are inseparable!”
