FORMS 1. Family Sign-up Form: Required for every family On this form, list each family member participating in the Ski/Snowboard Program, along with the dates they will participate and the Option # and Skill # for each person. This form allows Schweitzer and LCA to collect the information needed each week to have your lift ticket and instructor ready for you. You can always make adjustments to the weeks you participate. 4. Schweitzer Rental Form: Required for each person renting equipment Every participant who is renting equipment (boots, /snowboard, poles, and/or helmet) must complete this form.

PARENTS PLEASE READ THIS IMPORTANT INFORMATION

FAST FACTS ● Ski/Snowboard Program Coordinator: LouAnn Henneberg, 208.661.9825. You may call or text LouAnn at this number. ● We will ski/snowboard on Thursdays: Feb 11, 25, Mar 4. If a Winter Sports date is cancelled, it will not be made up. ● Schweitzer’s hours of operation on Thursdays are 9AM to 3:30PM. ● Please visit www.schweitzer.com for more Schweitzer information, including current conditions. The Ski/Snowboard Program Coordinator will not be available to take calls regarding weather conditions while in the midst of handing out lift tickets and checking students into lessons. Thank you for understanding! ● All students in grades 5th, 6th, 7th, and 8th are required to ski/snowboard for the 1st two Thursdays (they must attend Feb 11 and 25, no exceptions). The cost for these two weeks is already included in your tuition for these grades. Additional ski/snowboard days will be charged in addition to tuition. ● Younger students, age 6 through grade four, must be in the care of an adult other than school staff in order to be in the ski program. This is usually a parent or family friend. Note: You can get free tickets for children six and younger at the Schweitzer ticket window.

LESSONS All students are required to take a lesson. Students that skip a lesson (for any reason) may not be allowed to participate in the winter sports program. Schweitzer has given us discounted rates based on the understanding that ALL (adults included) who sign up for this program will be participating in the weekly lessons. Parents, if you would rather not take lessons, you may still come with us, but must purchase your ticket directly from Schweitzer.

Schweitzer has assigned us a lesson time of 9:00am this year. We are trying to get this moved back to 12:00pm but we need to know exact number of students participating before the change can be made. Lessons are 1 hour 30 minutes. Please arrive 10 minutes early for your lesson at the Group Meeting area near the top of the Magic Carpet area.

See the attached page to select your skier/snowboarder skill level. Please turn in skill levels for each skier/snowboarder you are signing up on the Family Sign-Up form so that Schweitzer instructors can place each of you into your lesson based on your skier/snowboarder skill level. Any adjustments after the first week can be made with Schweitzer staff out at lesson time (please arrive early to allow time for this).

TRANSPORTATION Parents must arrange transportation to/from Schweitzer Mountain Resort. LCA provides limited transportation by reservation only. Sign up in the school office by Tuesday of each week. Please be at LCA by 6:30AM if you need this transportation option.

PAYMENT INFORMATION ● Credit cards CANNOT be accepted. ● Payment is due prior to receiving your lift ticket. You may pay all in one payment or you may pay each week as you go. ● Payment can be given to the school office or to LouAnn Henneberg at Schweitzer when you pick up your lift ticket. Check or exact amount of cash is required if paying upon arrival at Schweitzer. No change can be given.

If a refund is due to you, it will be refunded to you after accounts have been totaled at the end of the program. If you have questions about your refund, please contact the school office.

STEPS FOR A GREAT DAY: 1. Everyone MUST have proper clothes for the winter weather conditions such as hats, gloves (no cotton gloves), 1 pair of socks to wear as well as extras to change into (preferably wool), goggles or sunglasses, and warm, -resistant pants and coats. 2. First, please pick up your lift ticket no later than 9:30AM from LouAnn inside the Lakeview Lodge at Schweitzer, main level, north side. If you are unable to pick up your ticket by 9:30AM, please call or text LouAnn that you are still coming and she will leave your tickets with the teacher on duty in the lodge. If you have NOT canceled for that day and do not show up, a ticket will have already been purchased and no refunds will be provided. 3. Next, check in with LouAnn to let her know you are present for the day and will attend your lesson. 4. Then, if needed, go to the rental shop to get your equipment. The first week will take more time to get your rental equipment, due to determining what equipment settings, etc. are best suited to your skier/snowboarder type and level. Going forward, Schweitzer will keep this information in their files so that your equipment is ready for you to pick up more quickly in the following weeks of our program. 5. A shoe check is provided for our group in the rental shop for those renting and needing a place to put shoes while .

COURTESY REQUEST LCA students need to remain on the main level of the lodge. The Lakeview Lodge manager has also requested that bags and gear NOT be left on or under the tables. You may hang bags from the poles on the main floor of the Lakeview Lodge. We have been given a designated area this year for our school on the north side of the main level. It looks out onto Musical Chairs. If there are no places to hang your bags, please line them up against the wall when you are out on the hill.

Furthermore, please remember that as a Christian school we are representing Christ. We need to behave respectfully and courteously and we also need to keep our area spotless - not just when we leave for the day, but throughout the day while we are eating and visiting.

I recognize that there is a certain amount of risk and potential for accidental injury inherent in auto travel, vigorous physical activities and the mountain environments that are part of the LCA Winter Sports Ski/Snowboard Program. SCHOOL GROUP LEVEL INFORMATION

Skill Number Skier Levels

#1 1st Time Never been skiing Lesson Focus: Stopping, basic turns, easiest terrain.

#2 Beginner Skiers learning to turn to a stop to skiers that can turn both ways to a stop. Have ridden a lift. Lesson Focus: Be able to link turns comfortably and consistently on easy Green runs and gain skills for Blue runs.

#3 Intermediate Skiers who can sometimes on easier terrain. Lesson Focus: Confidently turning and controlling speed on Blue runs. Low to moderate speed.

#4 Advanced Skiers who always parallel turn on more difficult terrain. Lesson Focus: Mastering skills in order to ski all runs and conditions on the mountain.

Snowboard Levels

#5 1st Time Never been . Lesson Focus: Equipment, balance, stopping, and side sliding. Will use easiest terrain.

#6 Beginner Riders learning to side-slip and turn to a stop to riders who can turn to a stop and side-slip directionally on one edge. Have ridden the lift. Lesson Focus: Learn basic turns in both directions.

#7 Intermediate Riders who can link toeside and heelside turns on easier terrain. Lesson Focus: Smoothly link turns and begin riding on Blue runs. Confidently turning and controlling speed by turn shape. Low to moderate speed.

#8 Advanced Riders who always link toeside and heelside turns on more difficult terrain Lesson Focus: Mastering skills in order to ride all runs and conditions on the mountain.

I will be responsible for the replacement at full retail value of any equipment rented under this agreement unless the equipment is returned to Schweitzer Mt. Ski & Ride Center by the date agreed to above. Failure to promptly return leased property by the closing time on the date due may result in criminal prosecution and full retail charge. To avoid any additional charges the equipment will be retuned in clean condition. I agree to check the equipment before each use and will not use the equipment if any parts are worn, missing or damaged; plus I will report any issue or damage of any equipment to the rental shop immediately. I am also financially responsible for any loss or damage to the equipment, other than reasonable wear and tear, which results from use of the equipment. • I expressly assume the risk of and legal responsibility for any injury to person or property that results from participation in the sport of skiing or snowboarding including any injury caused by the following, all whether above or below surface: variations in terrain; any movement of snow including, but not limited to, slides, sloughs or avalanches; any depths of snow, including tree wells, or any accumulations of snow, whether natural or man-made, including mounds; freestyle terrain; surface or subsurface snow or ice conditions; bare spots, rocks, trees, other forms of forest growth or debris, lift towers and components thereof; utility poles, and snowmaking and snowgrooming equipment which is plainly visible or plainly marked. I agree to RELEASE, HOLD HARMLESS, AND INDEMNIFY SCHWEITZER MOUNTAIN SKI OPERATIONS LLC, (“SCHWEITZER”), ITS OWNERS, AFFILIATES, AGENTS AND EMPLOYEES from all claims for any cause, including negligence, which arise out of participation in skiing, snowboarding and all related activities; including, but not limited to, Schweitzer Snowsports School programs and travel to and from Schweitzer Mountain, or travel to and from Schweitzer Snowsports School. I understand that the ski-boot-binding system which I have rented will not release or retain at all times or under all circumstances, nor is it possible to predict every situation in which it will release or retain. • I understand in the case of snowboard, snowshoe, and equipment, the boot-binding system is not designed to release and may or may not release under all circumstances. • In the case of a helmet rental, I understand that no helmet can protect the wearer against all foreseeable impacts and injuries to the , that mountain sports can expose the user to forces which exceed the limits of protection offered by the helmet, and will not protect against neck, cervical, spine, or any other bodily injuries. I understand that a helmet rental is only for the person that it is fitted to and not be used on anyone else. • I understand and agree that the binding systems, helmets and accessories are therefore NO GUARANTEE FOR MY SAFETY. I therefore RELEASE AND HOLD HARMLESS and agree to defend and indemnify Schweitzer Mountain Ski Operations LLC, (“Schweitzer”), Smith Sport Optics, their owners, affiliates, directors, officers, agents, instructors, volunteers, contractors and employees, the equipment manufacturers, distributors, and their owner, agent and employees from any and all liability for damage, death, an injury to myself or to my person or property resulting from negligence, the selection, adjustment, and use of this equipment and facilities, accepting myself the full responsibility for any and all such damage, injury or death of any kind which may result. • I have made no misrepresentations to Schweitzer in regard to my height, weight, age, or skier type. • I agree that if any portion of this agreement is held to be invalid, the remaining terms shall continue to be in full force and effect. This agreement shall be binding upon any other persons, including my heirs, assigns, and representatives. This release is intended to be a comprehensive Release of Liability to the fullest extent allowed by law. • If I am signing on behalf of a minor, I accept full responsibility for all medical expenses incurred as a result of the minor’s participation in skiing, snowboarding and all related activities, including Schweitzer Snowsports School programs and/or travel to and from Schweitzer Mountain and the use of the rental ski/snowboard/snowshoe equipment, facilities, helmet and/or accessories listed on this form. • I understand my child may ride the lift alone, with another child or with an adult other than his coach. I AGREE TO RELEASE, HOLD HARMLESS AND INDEMNIFY SCHWEITZER, SCHWEITZER SNOWSPORTS SCHOOL, THE EQUIPMENT MANUFACTURERS, DISTRIBUTORS, AND THEIR OWNERS, AGENTS, AND EMPLOYEES FOR ANY CLAIMS BROUGHT ON BEHALF OF THE MINOR. I, THE UNDERSIGNED, HAVE READ AND UNDERSTOOD THE TERMS OF THE ABOVE RENTAL AND RELEASE AGREEMENT. • In case of an emergency if I cannot be reached, Schweitzer has my permission to obtain any medical and/or First Aid treatment necessary for my child, and I agree to be financially responsible for any medical services deemed necessary. • I hereby grant Schweitzer Mountain Resort, their representatives or assigns (including any agency, client, partner or publication) irrevocable permission to publish photographs of me or my child. These images may be published in any manner, including websites, newsletters, advertising, periodicals and publications of any sort. Furthermore, I will hold harmless Schweitzer Mountain Resort, their representatives and assigns from any liability, by virtue of blurring, distortion or alteration that may occur in producing the finished product, unless it can be proven that such blurring, distortion or alteration was done with malicious intent to me.
