Outstanding Instructor Nomination Form ======The Honor Society of Phi Kappa Phi – Louisiana State University Chapter Award for Outstanding Instructor is presented to an exemplary educator at the rank of instructor at Louisiana State University. This award is co-sponsored by the LSU Alumni Association.

I am writing to invite your nominations for these prestigious awards. The recipient will be honored with a cash award and certificate.

This Instructor award is:

1) $900 PLUS Membership in Phi Kappa Phi (for Non-Members)

OR

2) $1,000 (For current Members of Phi Kappa Phi)

Selection of the award recipient is made by a committee of faculty members. Nominations must include the following information:
