PRODUCT REVIEW REQUEST , WINE + CIDER

SECTION 1

1. Please state the name of the company and/or parent companies you are responding for.

2. Soybean, beets, cottonseed, canola, corn, alfalfa, papaya, zucchini and yellow squash are all heavily genetically engineered crops. Common ingredients derived from these crops are amino acids, ascorbic acid, sodium ascorbate, sodium citrate, ethanol, natural and artificial flavors, lactic acid, maltodextrin, high-fructose corn syrup, dextrin, glucose, molasses and YEAST, as well as many more. If the products contain any of these ingredients or are sourced from any of these crops, please provide a copy of your non-gmo statement. If you do not have an official statement, please advise if your ingredients are sourced from non-GMO crops.

3. Are supplemental enzymes added to accelerate malting, suppress off-flavours or degrade residues or for any other purpose? If supplemental enzymes are used, can you confirm they are from non-GMO sources?

4. Do you add fining agents? If so, what are they sourced from (egg whites, , casein, , or another ingredient)?

5. Do you use a chemical filtration process? Yes No

6. Is your product free of all artificial colors, flavors, preservatives or synthetics? Yes No

7. If caramel coloring is used, can you advise if it is made naturally by heating , or synthetically by reacting with ammonia and sulfites?

8. If your product contains , is it produced from Red or Irish Moss ( crispus)? Can you verify what food grade it is?

9. If the products contain dairy/milk ingredients, are they from livestock not treated with rBGH/rBST and sub-therapeutic antibiotics? PRODUCT REVIEW REQUEST BEER, WINE + CIDER

10. If your products contain honey, please indicate the Country of Origin.

SECTION 2: WINE + HARD CIDER PRODUCERS ONLY

1. If you are an organic or biodynamic wine and/or hard cider producer, do you add sulfites? If so, please list the ppm level of sulfites in the products.

2. If you are an organic or biodynamic wine producer and/or hard cider producer and you do not add sulfites, please list the ppm level of sulfites in the products.

3. If yeast is used in the wine making process, please provide documentation that it is not genetically modified.

SECTION 3: ALLERGEN INFORMATION

1. If the company produces any gluten free items, does the company consider gluten to be derived from wheat, rye, barley, oats and any derivatives of these four grains?

2. What products manufactured and/or produced by your company does the company consider “gluten free?”

3. Are these “gluten free” products tested to meet the proposed FDA standard of less than 20 ppm gluten? If not, how is gluten free status determined (i.e., verification from ingredient suppliers, gluten free facility, etc.). If the facility is not gluten free, how is cross contamination addressed?

4. If you are an organic or biodynamic wine producer, are your barrels sealed with a paste of flour and or any other gluten containing substance? PRODUCT REVIEW REQUEST BEER, WINE + CIDER

5. If the products are aged in barrels, is cross contamination from these barrels addressed (i.e., were they used to age whiskey, bourbon, etc.)? Yes No

6. If fining agents are used, can you confirm they are gluten free? Yes No

7. Are any of the products manufactured by your company vegan? Is cross contamination with dairy ingredients a risk?

8. Are the products produced and packed in peanut free facilities? Yes No
