3. Collision of 9167 Up Sabarmati Express and a Tractor-Trolley at unmanned level crossing gate No. 95/C between Chachura-Binaganj-Kumbharaj stations of Division of West Central Railway on 06.05.2009.

4. Unusual occurrence of Flashing o Pantagraph resulting in sparking in N-4 Up Suburban local train between Shahad and Kalyan stations of Mumbai Division of Central Railway on 12.05.2009.

5. Unusual occurrence of injuries to passengers of B 0346 Up Borivali-Churchgate EMU train on Up through line between Vileparle and Santacruz stations of Mumbai Central Division of Western Railway on 29.06.2009.
