1.0 PROJECT OBJECTIVE

Local Government bridges located within the geographical limits of the Department of Transportation District Six, herein after called the Department, are to be inspected in compliance with Federal and State regulations. Reference should be made to the following when applicable; National Bridge Inspection Standards, Code of Federal Regulations (CFR) Title 23, Part 650, Subpart C; Section 335.074, Department of Transportation Rules, Chapter 14-48, Florida administrative Code (F.A.C).

2.0 GENERAL DESCRIPTION

The Consultant shall perform bridge inspection services, as described in this scope of services, and present inspection findings in the BrM format to an estimated total of 364 Bridges (see Attachment A). The consultant is responsible on their own expenses for procuring training in BrM for its employees. The Bridges are classified as On-System (Federal Aid or FA) and Off-System (Non-Federal Aid or NFA). The type and frequency of inspection shall comply with Florida Statutes and the Manual for Bridge and Other Structures Inspection and Reporting Procedure 850-010-030.

2.1 The following portions of the work are dynamic, therefore, the quantity for each work item is estimated:

2.1.1 Bridges not listed in Attachment A will require initial inspection and application for new bridge numbers, when necessary, since prior inspections of these newly constructed or newly inventoried structures have not been performed.

2.1.2 All bridges that are newly constructed, rehabilitated, inventoried, having significant deterioration or not having a proper load rating analysis will required to have a new load rating calculations when approved by the Department.

2.2 The service period shall be twenty-four (24) months and shall begin from the date of the written notice to proceed issued by the Department.

2.3 For the purpose of this agreement, a structure is considered to be a bridge as defined in National Bridge Inspection Standards 23 CFR 650 Subpart C and it is located on public roads.

Stage I A-1 2.4 All services to be performed by the Consultant shall conform to the current National and State publications covering bridge inspection standards of practice. The following are references to be used during the contract terms:

3.0 PERSONNEL REQUIREMENT

3.1 A qualified individual, Lead Inspector, must be at the bridge site to supervise inspection activities at all times. To be qualified, an individual must be registered as a Professional Engineer in Florida and experienced in the inspection of bridges according to the National Bridge Inspection Standards or be confirmed officially by the Department as a Certified Bridge Inspector. Qualifications for certification as a bridge inspector are stated in the Rules of the Florida Department of Transportation, Chapter 14-48.0011, F.A.C.

3.2 An organizational chart shall be submitted to the Department for approval. The organizational chart shall include the names and registration numbers the proposed personnel, team size, and which individuals are assigned to the various work tasks.

3.3 The Consultant’s work shall be directed and performed by the personnel identified in the Stage I A-2 technical proposal as presented by the Consultant. Any changes in the indicated personnel shall be subject to review and approval by the Department, prior the person performing any work on this contract. The replacement of key personnel shall be done with personnel with similar qualifications. A minimum of seven (7) calendar days should be provided for the Department’s approval of a personnel change.

3.4 Each final Bridge Inspection and Load Rating Analysis Report shall be signed, dated, and sealed in accordance with Florida Statute 471.025 by the Professional Engineer who confirms the accuracy and completeness of the document. Lead Inspectors shall initial each Final Bridge Inspection report.

3.5 Due to the nature and scope of the required services, it may be desirable for the Consultant to subcontract portions of the work (i.e., underwater inspections, mechanical inspections, electrical inspections, load ratings analysis). The Consultant will be authorized to subcontract these services under the provisions of this agreement.

3.6 Subcontracting firms and the work they will perform shall be identified in the original proposal and organizational chart. The subcontracting firms must be approved in writing by the Department prior to initiation of any work. Any changes in the subcontracting firms or the work they will perform as indicated in the Consultant’s proposal are subject to review and approval by the Department as required in section 3.3 of this document.

3.7 The Consultant is responsible to keep informed all of their personnel, working under this contract, of changes that have been made in inspection and/or reporting procedures.

4.0 WORK ITEMS

4.1 Field Inspections

4.1.1 Field inspections must be conducted in accordance with the publications listed in Section 2 of this document. The Professional Engineer or Certified Bridge Inspector on site shall be responsible for identifying all elements within the structure; properly identifying all deficiencies; and assigning the proper element condition states.

4.1.2 The Lead Inspector is responsible for ensuring that the bridge numbers are in place and legible on each bridge. Painting of missing or illegible bridge number is part of every bridge inspection, not a separate pay item, during the term of this contract.

4.1.3 The removal of dirt and debris will be considered excessive when the effort requires more than one hour for the inspection team (2 staff) to perform. The Department should be consulted prior to any major cleaning operation. Bridge members and elements that cannot be inspected, prior to report publication, must be documented in the report as such, along with the reason for not inspecting them. The Consultant must perform proper planning in the event that the Department need to ask the bridge owner to perform cleaning operations, before the field inspection, to avoid a bridge inspection delinquency.

Stage I A-3 4.1.3.1 Terminology not commonly understood by non-inspection personnel should be minimized and undefined values or ratings shall not be used.

4.1.3.2 Where deficiency dimensions are documented, the actual width, length, depth, etc., and location on the member should be recorded. In cases where dimensioning every deficiency is impractical, the standard Department Table of Deficiency Dimension Classes, must be used in lieu of undefined general terms, such as narrow, fine, etc.

4.1.3.3 The probable cause of deficiencies should be identified. This serves two primary purposes; first, the most effective repair can only be determined if the source of the deficiency has been identified; second, when the same cause for a specific deficiency is documented repeatedly, a body of evidence becomes available to justify its elimination through a change in design.

4.1.3.4 Deficiencies described in previous bridge inspection reports must be addressed again in the Element Notes of the current report. A bridge inspection report must provide a comprehensive description of all deficiencies and state probable causes of the deficiencies, specify required corrective action, and contain an evaluation and picture of any previous corrective action noted if any.

4.1.3.5 Deficiency descriptions shall be in enough detail to allow rates of change to be monitored over consecutive inspections. This requires liberal use of size and location dimensions, sketches, and photographs. A sketch should illustrate only the essential features of a deficiency without distracting details. Sketches should be used only when a photograph is not practical or do not describe properly the deficiency. Photographs shall always be taken, reported and archived in color.

4.1.3.6 Gauge, mechanical clearances and electrical readings must be recorded in the Element Notes to be used for monitoring purposes. Previous readings should be compared to detect any inconsistencies with the subject element. This requirement will apply to those bridges with mechanical, electrical, or hydraulic components.

4.1.4 The Consultant shall not allow any bridge inspection team member to be a Team Leader on consecutive routine inspections on the same bridge. If this occurs, the Consultant shall re-inspect the bridge using another Team Leader at no charge to the Department.

4.1.5 If by field observation, deficiencies are sufficiently critical to warrant immediate and substantial traffic restrictions or closing of the bridge, the Department must be immediately verbally notified (no voicemail is accepted). Verbal notification must be confirmed with written notification to the Department within twenty-four (24) hours.

4.1.6 If one or more posting signs are missing from a structure that requires load carrying restrictions, the Consultant will immediately notify the Department in writing.

4.1.7 Traffic control procedures must be in accordance with the FHWA/USDOT Manual on Stage I A-4 Uniform Traffic Control Devices (MUTCD, current edition) and the Department’s Roadway and Traffic Design Standards (current edition). The Consultant is responsible for the coordination and implementation of the Maintenance of Traffic and to obtain all necessary lane closures approvals from the bridge owner when necessary.

4.1.8 The Consultant is responsible to coordinate with the bridge owner when permit access is required.

4.1.9 Color photographs and or sketches will be used to illustrate at least one typical example of a severe and/or common deficiency. When deficiencies are severe enough to warrant immediate repair, one photograph illustrating a typical example must be taken. When a photograph is taken, an object that provides a sense of scale such as a ruler should always be included in the photograph. The essential features of a deficiency shall be emphasized, by marking arrows or dimensions directly on the photograph. The date the photograph was taken shall always be marked on the front. Each copy of the report will have color photographs. Sketches or photos should be used to indicate the location of deficiencies. All deficiency photos will be included in the BrM report format. All other photo’s not relating to deficiencies will remain in the addendum.

4.2 Inspection Reports

4.2.1 The Consultant must develop, publish, and distribute electronic bridge reports for the Department and for the bridge owner, for any type of inspection performed. Each report will contain all documentation specified by this agreement and must meet the requirements of the Bridge and Other Inspection and Reporting Procedures 850-010- 030. All reports will be created in a BrM format, using most current release version. The Department will provide access to the BrM network. The Consultant is responsible for procuring BrM training for its employees. Each report will be presented in the BrM format, stored in EDMS and contain all documentation specified by this agreement.

4.2.2 Each bridge inspection report shall be prepared in letter size format (8 ½ in by 11 in). Addendums and sections of inspection performed by sub-consultants must be attached at the end of each report with proper signatures and seals.

4.2.3 Each bridge report must be submitted to the Department, for review, no later than forty- five (45) calendar days from the date of the field inspection. The Department will return records, within seven (7) calendar days, that do not conform to the specifications of this agreement or that are in error. Such revisions must be resubmitted to the Department within seven (7) calendar days. All reports shall be sign and seal and finally approved by the Department no latter that sixty (60) days from the inspection date.

4.2.4 After the acceptance of the report, the Consultant shall upload the final signed and sealed electronic copy to the Department’s EDMS system. The Consultant shall deliver, immediately, an electronic copy to its respective owner via certified mail, by hand or at scheduled meetings throughout the term of the contract. The Consultant shall obtain delivery signatures from the owner’s representative confirming the reports delivered on a digital media device. Stage I A-5 4.2.5 BrM Critical Deficiency Statement: It will be located on the first page of the BrM report. Written explanation will be provided, on the submittal letter and in the Structures Notes Section of the BrM report, under the heading of “CRITICAL DEFICIENCY”.

4.2.6 Traffic Restriction Statement: The BrM report should contain, in the Structures Notes Section, under the heading of “TRAFFIC RESTRICTIONS”, a statement describing the current posting requirements and the current field conditions.

4.2.7 Each Initial Bridge Inspection report transmitted to the bridge owner electronically and a paper copy must be top bound in a file folder as specified in the manual for Bridge and Other Structures Inspection and Reporting Procedure 850-010-030 with the bridge number printed on the index tab. The final signed and sealed Initial Bridge Inspection shall be uploaded to the Departments EDMS for retention.

4.2.8 The current Load Rating summary sheets for every structure in this contract will be provided to the Consultant. The Consultant will be responsible to use this sheet to verify and update the load rating data in BrM and to include this sheet at the end of every inspection report.

4.2.9 When a sub-consultant conducts any section of the field inspection, an addendum will be created by the sub-consultant and submitted along with the BrM report.

4.2.10 Evaluation of previous corrective action. The purpose of this section is to evaluate the effectiveness of repairs that were performed since the previous inspection. Repairs recommended in previous inspections but not completed shall be addresses in the “Recommended Corrective Action Section”. Each repair should be inspected carefully to determine if the repair effectively corrected the deficiency. A photograph must be included in the report for both cases, corrected and not corrected repairs.

4.3 Underwater Inspection

Underwater inspections shall be performed in accordance with the publications listed in Section 2 of this document. Also, underwater inspections are required, regardless of depth, on any portion of a bridge that cannot be adequately inspected without the use of divers or when scour problem exists. If no deficiencies are found, at least one general photograph must be included in the report of divers performing the inspection. At the discretion of the District Structures Maintenance Administrator, underwater inspections may be required for bridges undergoing significant scour after significant storms.

4.4 Fracture Critical Inspection

A qualified structural engineer must identify fracture critical members. Refer procedures 850-010-030. The documentation of this inspection shall be included in an addendum at the end of the BrM report with proper plans or sketches. Stage I A-6 4.5 Inspection of Gusset Plates in trusses

This inspection shall be performed according to procedure 850-010-030 and an addendum report shall be included at the end of the BrM report. This inspection will be performed along with the Fracture Critical inspection.

4.6 Channel and Scour Evaluation Data

Channel and scour evaluation should be performed according to Section 3.6.2.9 of procedure 850-010-030.

4.7 Load Rating Analysis

4.7.1 The Load rating analysis reports and documentations will follow the requirements established in the Department’s Procedure 850-010-030. An electronic signed and seal copy will be delivered to the Department and another copy will be delivered to the respective owner, after Department’s approval.

4.7.2 The Consultant will be responsible to update the load rating data in BrM once the analysis has been approved by the Department.

4.8 Bridge Owners Communication

4.8.1 The Consultant may be required to attend meetings, prepare and conduct workshops or any other kind of informational activities that could help the owner improve their Bridge Maintenance program. The Consultant must propose different informational activities for Department consideration. These activities will be discussed and negotiated during the Contract Negotiation period.

4.8.2 The Consultant should provide innovative ideas or approaches which would facilitate better sharing of information with the bridge owners, such as deficiencies, recommended maintenance alternatives, and any other strategies intended to preserve structural integrity of their structures under this contract.

4.8.3 At least one meeting per Bridge Owner may be scheduled through the contract term. The meetings may include a brief discussion of bridge inspection program, regulations, and objectives, an outline of the bridge owner’s responsibility to close or post bridges with recommended weight restrictions and to correct deficiencies that endanger the traveling public. The discussion of the bridge inspection program should cover as minimum the Department Rules Chapter 14-48, Bridge Inspection Standards, and the importance of notifying the Department when structures that are structurally or critically deficient has been repaired or replaced and when recommended weight restriction posting have been accomplished.

4.8.4 The Consultant will be responsible to update every Plan-of-Actions (POAs) for those Stage I A-7 scour critical bridges after each corresponding bridge inspection. The Consultant must encourage the local agencies to implement the POAs on their scour critical bridges. 4.8.5 The Consultant shall provide the bridge owners and the Department with the inspection schedule of their bridges. The information shall be transmitted with sufficient time to allow the bridge owner to plan their participation in the bridge inspection, if desired.

4.9 Emergency Response

The Consultant will provide the Department with services that include Emergency Inspections and Reports to structures, identified by the Department and listed on attachment “A”, after an emergency event occurs. This work will be paid under the Emergency task item included in this contract.

5.0 QUALITY CONTROL / QUALITY ASSURANCE

The Consultant shall have quality control and quality assurance procedures which shall be provided in written form to the Department for review and approval before the work begins. Quality control and quality assurance procedures are an integral part of this contract. The Department will perform quality assurance activities according to the District Six Structures Maintenance QA/QC Plan during the term of this contract.

5.1 Quality Reviews

5.1.1 The Consultant shall conduct quality reviews to make certain his organization is following the requirements cited in their scope of services. Quality reviews shall evaluate the adequacy of materials, documentation, processes, procedures, training, guidance, and staffing included under this contract.

5.1.2 The Quality Assurance Program should also have a second peer review level. The peer review can be conducted internally by the Consultant’s organization or by subcontracting the review to another consulting firm.

5.2 Quality Records

The Consultant shall maintain adequate records of the quality assurance actions performed by his organization (including subcontractors and vendors) on the services and products provided under this contract. All records shall indicate the nature and number of observations made, the number and type of deficiencies found, and the corrective action taken. These records shall be kept at the primary office site and shall be available to the Department for review during the contract term.

5.3 Quality Assurance Plan

The Consultant shall include a quality assurance plan in the proposal. The quality assurance plan should detail the procedures, evaluation criteria, and instructions to his organization to assure conformance with the contract. Significant changes to the scope of work and applicable manuals and procedures may require the Consultant to revise Stage I A-8 his quality assurance plan to keep it current with the scope of services. The plan shall include, but not be limited to the following areas: - Organization - Quality Reviews - Quality Records - Quality of Subcontractors and Vendors - Quality Assurance Certification

5.4 Quality Reviews by the Department

5.4.1 The Department will conduct quality assurance activities according to the District Six Structures Maintenance QA/QC Plan and the requirements cited in the scope of services. These quality reviews will be performed at different periods during the contract term. The Consultant will be notified prior to any of these quality reviews and shall be required to present all records and certifications upon request.

5.4.2 Subcontractors or vendors may be included as part of the quality review.

6.0 MATERIALS AND MANUALS

6.1 Materials to be Furnished by the Department

7.0 WORK SCHEDULE AND PROGRESS REPORTS

7.1 Kick-Off Meeting

7.1.1 The Department shall conduct a kick-off meeting before the Consultant start working on their first assignment. This meeting will include but will not be limited to the following personnel:

 Department’s Project Manager  Consultant’s Project Manager

7.1.2 The Department will explain the financial and administrative details of the contract, establish any ground rules upon which the work will be conducted, and transfer materials to be furnished by the Department.

7.1.3 The Consultant will provide the Department with the Consultant’s written quality control/quality assurance plan, a proposed work schedule and organizational chart. The list of major equipment shall indicate if the Consultant owns the equipment, and if not, his plans for obtaining the equipment.

7.2 Work Schedule

7.2.1 A proposed work schedule shall be submitted to the Department for approval. This schedule shall show how the Consultant anticipates the work will progress from month to month up to completion of this agreement. The work schedule shall be established in a manner that will ensure no bridge inspections will be delinquent. The Consultant is responsible to avoid any Bridge inspections delinquency. In the event that a bridge delinquency occurs, it will be penalized with Low Contract Grading on the Department’s Consultant Ratings system.

7.2.2 The Consultant shall perform periodical BrM reviews to avoid any delinquency event. The Consultant shall submit a verification schedule for the month to be inspected before the end of the preceding month. The supporting documentation review must include BrM Due Date reports.

7.3 Progress Reports

7.3.1 The Consultant shall submit on Mondays via e-mail weekly progress reports that show; work completed as measured against the proposed schedule. Weekly progress reports Stage I A-10 shall describe the work performed on each bridge during that month and to which task work order the work corresponds.

7.3.2 The Consultant shall meet with the Department on a monthly basis or as deemed necessary by the Department. The cost for this meeting is not a separate pay item.

8.0 ELECTRONIC DOCUMENT MANAGEMENT SYSTEM (EDMS)

8.1 The Consultant must follow the Department’s Electronic Document Management System (EDMS) Requirements.The purpose is to ensure that all records produce from this contract are legible, complete and retrievable once they are archived. All official documents will be stored as a PDF file (e.g..PDF). The Consultant shall use the official naming convection as directed by the District Structure Maintenance Office

8.2 The Consultant must obtain and maintain an active VPN connection which will be used to manage electronic documents in the department’s EDMS.

8.3 The Consultant must obtain and/or maintain the Enterprise EDMS software approved by the Department. The Department may provide a brief training of the application for the Consultant’s representative as requested.

8.4 In the event that Unacceptable Documents (a document that did not comply with the scanning/importing requirements such as legible, complete and retrievable) are identified, the document or relevant pages should be immediately re-scanned/re-imported and reviewed again to prevent document loss. No reports will be paid until they have been properly uploaded into EDMS and delivered to the respective bridge owner.

8.5 The Consultant shall provide personnel proficient in the use of computers to input Bridge Inspection Documents into EDMS. This will require familiarity with the Structures Management Document guidelines for EDMS posted on the Department’s webpage. Duties will include uploading/scanning, attributing (assigning information specific to the document) and retrieving documents that are archived electronically in EDMS.

8.6 The Consultant shall maintain an EDMS Log listing file upload name, date uploaded and brief description of the file contents. The EDMS Log should be shared with the Department on a monthly basis for QA/QC review.

8.7 The Consultant shall provide and have available the following hardware and software:

8.7.1 Computers with capabilities of efficiently work with BrM and EDMS and any other software needed to complete every task on this project.

8.7.2 Software:  Operating Systems compatible with FDOT software’s  Internet Browser & Add-ons compatible with FDOT EDMS

Stage I A-11 8.8 The Department shall provide instruction for obtaining VPN, EDMS and the use of the EDMS program.

9.0 MISCELLANEOUS

9.1 Legal Proceedings

The Consultant shall serve as an expert witness in any legal proceedings if required by the Department. The fee for these services shall be established if, and when, they are needed.

9.2 Errors and/or Omissions

The Consultant shall be responsible for the professional quality, technical accuracy and the coordination of all documents furnished by the Consultant under this contract. The Consultant shall, without additional compensation, correct or revise any errors and/or omissions (and approved corrections of same) that result from said firm’s substandard performance of the services described in this contract.

9.3 Contradictions

In the event of a contradiction between the provisions of Exhibit “A” and the Consultant’s technical proposal, the provisions of Exhibit “A” shall govern.
