............................................................................ 21 3 SERVICES ACO Partnering Service Description & Features Accountable Care Organization (ACO) Partnering provides the ability for ACOs to receive notifications and view clinical data. The service benefits ACOs by providing CCDAs to support analytics platforms, sending notifications of patient admissions and Emergency Room visits to ACO Case Managers, and giving Case Managers access to the clinical portal to review documentation across the ACO. Users of this service will receive a KeyHIE Clinical Portal account to view patient clinical data from other participating facilities. CCDA Data Quality Improvement Service Description & Features KeyHIE partners with its member organizations to improve CCDA data quality through scoring techniques performed on their respective CCDA data. This cooperative effort allows providers to see the quality of their electronic health record (EHR) generated CCDAs and to pinpoints areas where key data element improvements can be made. KeyHIE benchmarking tools allow KeyHIE participants to proactively test their CCDAs and assist the KeyHIE implementation team with validation testing in the KeyHIE portal. The results can be used for clinical quality reporting and to help drive quality initiatives. Improving the CCDA grades across our healthcare community will allow more clear and consistent clinical data sharing and CCDA document interoperability. Clinical Portal Service Description & Features The Clinical Portal provides an entry point to view patient-specific information including Demographics, Encounters, Allergies, Medications, Laboratory, Radiology, Problems and Procedures and Transcribed Documents via the web. The portal manages and maintains the authentication and authorization, privacy and security, and auditing of user actions. The Clinical Portal includes Clinical Document Viewer and Summary Viewer. 11 The Clinical Document Viewer enables the ability to view clinical summaries and documents via a laptop, desktop, or mobile web browser. Its features include viewing and accessing clinical laboratory test results, summary of care records, and specialist consultation documents. The Summary Viewer provides a high-level summary of clinical documents specific to each patient. For example, it provides a summary of clinical laboratory test results, summary of care records, and specialist consultation documents. The Medication Viewer offers providers a longitudinal view of a patient’s medication history, allowing the entire care team to easily reconcile a patient’s medications.
