Parents/Guardians of the above named student (on reverse) acknowledge that they will accept general liability for the participation of the child in after school program and hold harmless The Bear Creek School and Eastside Family Karate, their coaches, employees, directors and volunteers from claims, liability or suits arising from the students involvement while participating in all the program and related activities offered by The Bear Creek School. If transportation is pro- vided in a privately owned vehicle, the driver is responsible for his or her passenger’s well-being. The parent/guardian agrees to carry and supply proof of auto liability insurance prior to transporting any student on behalf of The Bear Creek School. The Bear Creek School does not purchase medical insurance for coverage of student injury or accident. Private insurance protection is strongly advised. Parent’s/guardians are responsible for securing medical insurance cov- erage for their student and for any costs of medical treatment that may be incurred as a result of the student’s after school program participation.
