1.0 INTRODUCTION

SKYCABLE CORPORATION is issuing a Request for Quotation (RFQ) for the proposed services (Semi Turn Key) of projects to all applicable areas. SKYCABLE to evaluate complete scope, costing and timeline from all the shortlisted bidders. If you are interested in providing these services, please submit a proposal for this works based on the information presented on this RFQ.

2.0 INSTRUCTION TO BIDDER

2.1 RFQ Terms & Conditions

2.1.1 Sky assumes no contract obligation in the issuance of the Request for Quotation (RFQ). The bidder shall shoulder any cost incurred in the preparation of the Quotation.

2.1.2 It shall be understood that these documents are confidential and property of . In accepting this RFP, the bidder agrees to safeguard the information contained herein.

2.1.3 SKY reserves the right to reject any or all proposal. Sky Cable will notify the bidder regarding acceptance of their proposal.

2.2 Important Dates and information:

2.2.1 Pre-Bid conference, clarification meeting will be scheduled.

2.2.2 Technical clarifications concerning the project may be emailed to SKY Cable procurement division.

2.2.3 Submission of bids will be finalized shall be through hard copy (green/red envelope)

3.0 SUBMITTALS Participating bidders shall submit the following documents as part of the proposal;

3.1 Commercial (for submission to Procurement Team)- 2 copies in Green Envelope 3.1.1 Price Proposal 3.1.2 Price BOQ 3.1.3 Financial capability 3.1.4 Proforma contract compliance checklist/statement 3.2 Technical (for submission to Technical Team)- 2 copies in Red Envelope 3.2.1 General Understanding of the Scope of Work 3.2.2 Engineering 3.2.3 ROW/Permitting/RTA 3.2.4 Construction 3.2.5 Quality Assurance( QA), Turn-over and Acceptance Program 3.2.6 Acceptance metrics 3.3 Incomplete submittal shall be ground for technical disqualification

3.4 The Sealed Envelopes Shall: 3.4.1 Bear the name of the Project to be bided. 3.4.2 Shall be addressed to the party designated to receive the Bid. 3.4.3 Indicate the name and address of the Bidder to enable the Bid to be returned un-opened in case it is declared “late”. 3.4.4 Be marked Do not open before (Date and Time of opening Bids.

Submitted proposal will be considered as Provisional Sum. All quantity/scope of work are re- measurable and subject to reconciliation once the plan are approved.

4.0 PROJECT SCOPE OF WORK

4.1 The CONTRACTOR shall be assigned to perform Semi Turn Key Sky projects.

4.2 The COMPANY shall provide the CONTRACTOR a work order, as the case maybe, on a per project basis. All work shall be performed and completed with the established standards, guidelines and timetable.

4.3 The CONTRACTOR shall be responsible for the necessary permits / authorization required to complete the implementation of the project .

5. A CABLING

5. A.1 COAXIAL CABLING (0.565, Figure 8) TOTAL REQUIREMENT : ______Coaxial Cabling & Installation of Grounding P______per strand km.

5. A.1.1 Laying-out of new cables and pole hardware as per design map. 5. A.1.2 Pullout of damaged/replaced cables. 5. A.1.3 Correction of sag cable / for pole/ alley arm attachment / for transfer of cable to new position on alley arm, as applicable.

A.1.4 Installation of grounding wire (messenger wires/copper wire) from strand wire to ground rod, which includes the utilization of various grounding hardware and accessories and burying of 8 ft. ground rod, applicable for new build. 5. 5. B. FIBER OPTIC CABLING TOTAL REQUIREMENT: ______The contract price of the following scope of work to be performed by the CONTRACTOR shall be P______per strand km:

B.3.2 Installation of grounding wire (messenger wires/copper wire) from strand wire to ground rod which includes the utilization of various grounding hardware and accessories and burying of 8 ft. ground rod, applicable for new build. 5. B.3.3 Installation of Cable Sticker 5. B.3.4 Identification of utilized cable position per pole in the as-built map.

5. B.4 Over lashing

5. B.4.1 Fiber Optic Cable

The contract price of the following scope of work to be performed by the CONTRACTOR shall be P______per strand km:

5. B.4.1.1. Laying-out of fiber optic cables and overlashing of such cables on existing strand wire including pole hardware installation as needed on the relocation requirement 5. B.4.1.2. Pullout of old/damaged fiber optic cables and strands.

5. C CABLE PLANT RECOVERY / TEARDOWN TOTAL REQUIREMENT: ______The contract price of the following scope of work to be performed by the CONTRACTOR shall be P______per strand km:

5. C.1.1 Trunk and Distribution Cables (Figure 8) and with Messenger Wires (steel strand) 5. C.1.2 Removal of amplifiers and power supplies. 5. C.1.3 Removal of coaxial cables including all passive (taps, splitters, couplers, etc.) devices, and pole hardware from the poles where these materials are attached. 5. C.1.4 Extraction of passive devices from the cables. 5. C.1.5 Removal of lashing wires wrapped around coaxial cables 5. C.1.6 Removal of messenger wires (steel strand) 5. C.1.7 Inventory of materials recovered. 5. C.1.8 Delivery of all the materials recovered to SKY CABLE warehouses.

Ground rod for all EOL, power supplies, splitters and couplers and amplifiers. 5. E.9 Forward /Reverse balancing and end of line testing 5. E.10 Installation Cable sticker 5.
