1. INTRODUCTION two major categories of standards targeted by the Health Information Standards Committee of Alberta (HISCA) for definition and adoption are:

- Health Information Standards; and - Health Technology Standards.

Under the Health Information Standards category, data standards are defined from two perspectives:

- Data Administration (Data Dictionary) Standards.

This level of standard deals with fundamental properties of data elements, and focuses on definition, information exchange format, and use of the data element.

- Data Architecture (Modelling) Standards.

This level of standard builds upon the basic definition and properties of the data elements as defined at the data administration level. It extends their use in the context of standard applications (modelling requirements) and standard data groupings/collections (data sets).

The standards described in this document are of the data administration type, dealing with the definition of data elements as they relate to the health business.

The purpose of this document is to define standards for stakeholder demographic data elements. The elements included in this standard are those required to identify, locate, or communicate with stakeholders (persons and organizations). These data element standards are focused more on person-specific information, with less emphasis for now on organization-specific information.

The definitions, formats and use of the data elements described in this document were gathered by reviewing provincial, national and international health information standards. These include HL7, BC Health Information Council Standards, Saskatchewan Health Information Standards Council, CIHI Health Information Framework, Government of Alberta Data Standards Sub-Committee (ADSS) data standards and the International Standards Organization (ISO). Key stakeholder organizations were then surveyed to obtain feedback on the standards, including the Alberta Health Authorities, health professional organizations, Alberta Wellnet, and business areas within Alberta Health and Wellness.

There are several other stakeholder demographic data elements that have not been included in this current list of elements, but are candidates for future standards definition efforts. Some of these elements are for eligibility purposes and others are for additional information relevant to the health system but not required for identifying, locating or communicating with stakeholder, for example Ethnicity.

This document is divided into the following sections:

1. Introduction This section provides a brief description of the purpose and contents of the document.

2. Justification This section discusses the results of the literature search, the business case for the proposed standard, the impact on privacy, confidentiality, and security, the relationship to existing standards and/or legislation, and the summary of stakeholder consultation.

3. Data Models Conceptual and logical data models are included to provide context for the data elements defined in this document.

4. List of Data Elements. The stakeholder demographic data elements for which standards are proposed are categorized and listed in this section.

5. Stakeholder Demographic Data Standards This section includes the data elements detailed in the standard template.

2. JUSTIFICATION Results of Literature Review Standards work from various jurisdictions were considered in the development of this standard. The Government of Alberta has a set of data standards that were reviewed for alignment. Both British Columbia and Saskatchewan, have initiated work to address a fully structured address that is Canada Post compliant. However neither standard has been implemented and therefore there was no complete solution to standardizing address attributes at the atomic data element level. The Pan-Canadian Client Registry Messaging Specification for the Canadian Institute for Health Information was considered for alignment wherever possible.

Business Case for the Health System Once approved, this standard will applied to the Alberta Health and Wellness central repository/directory for person information. All other applications will use the standard when exchanging demographic data with the repository. This standard may also be applied wherever stakeholder information is being collected, stored, or exchanged.

Impact on Privacy, Confidentiality, and Security At this time, a PIA is not required. This standard sets the stage for a common opportunity to share information between custodians and affiliates. A PIA would be required for the custodians and affiliates that do wish to exchange stakeholder information between applications.

The Health Information Act (HIA) is the primary legislation for prescribing the rules for the collection, use and disclosure of stakeholder registration information between custodians. Additional related legislation is contained in the Alberta Health Care Insurance Act (AHCIA), the Health Insurance Premiums Act and the Freedom of Information and Protection Act (FOIPP). All requests to collect, use and disclose identifiable stakeholder information through the Alberta Health and Wellness central repository/directory, must be reviewed and authorized by the Customer Services and Registration Branch Director.

Relationship to Existing Standards and/or Legislation This standard is meant to be to an enhanced version of the originally approved standards approved in December 1999. Standards from Saskatchewan and British Columbia were reviewed. As well, standards from the Health Authorities and the Canadian Institute for Health Information were consulted. Where possible, attempts were made to align with the Government of Alberta data standards and the International Organization for Standardization (ISO).

Summary of Stakeholder Consultation The CIO of Alberta Health and Wellness originally approved the Stakeholder Basic Demographic Data Standard in December of 1999. The standardization process for HISCA includes an annual review of approved standards. In November of 2000 feedback was consolidated into a draft document. In November of 2001 the Provincial Person Health Identifier (PPHI) Project was initiated. The review of the stakeholder standard was extended to allow for an extensive consultation with the Health Authorities and Boards, Alberta Wellnet, the Alberta Data Standards Subcommittee, key business areas within Alberta Health and Wellness, and national bodies such as the Canadian Institute for Health Information. The purpose of the review was not to re-design the standard but to update key attributes.

Requirement for Standard: A unique identifier, along with a unique combination of ID Type + ID Domain provides the ability to uniquely identify stakeholders within the health system. The assigning authority identifies the issuer of the identifier.

These data elements provide the standard information required for stakeholder identification. No matter what type of id is assigned, the standard is the same. ID Types apply to a person, organization or to a person or organization in a specific role. Business rules are used to identify the type of object to which an ID Type can be assigned.

The value of the assigning authority can be changed without affecting the other elements of the ID. This ID structure provides the ability for applications to assign/restrict security permissions at various levels. Examples: ID Type ID Domain ID Assigning Authority

Business Rules / Coding Guidelines: Business rules are used to identify the object (Person or Organization) to which an ID Type can be assigned. For example, ‘HAID’ is the ID Type representing Health Authority Numbers and would not be assigned to a person.

Each ID Type will have its own set of business rules. Example: ULI (N9) The ULI (Unique Lifetime Identifier) is a unique and permanent number assigned as an internal identifier by Alberta Health and Wellness to all stakeholders of interest to the health system in Alberta. There are two types of stakeholders recognized in the health system. These are: Stakeholder PERSON - a human being, alive or not, of interest to the health system, and Stakeholder GROUP - a collection of people, formal or informal, with distinct characteristics of interest in common. The collection may be comprised of zero or more people. Organization is a type of Group.

It has been included here because the use of this element when implementing the ID structure can improve the flexibility of the structure. For example, the organization responsible for assigning a specific domain of Ids might change, without affecting the ID Type or ID Domain.

Presentation/Display Format: The standard way to represent the ULI (Alberta) in print or display medium is as follows: 99999-9999 Valid Values: Value Sets for each comprise the identification strings assigned by the ID Assigning Authority for each.

Business Rules / Coding Guidelines: ULI N9 PHN (C15) The PHN (Provincial Health Number) is the identifier assigned by the provincial health jurisdictions for identification within their health systems. For Alberta, PHNs are the subset of ULIs that have been assigned to stakeholder persons upon registration and presumes eligibility for basic health services as defined in the Alberta Health Care Insurance Plan. Different provinces have different algorithms for assigning identifiers. Requirement for Standard: Accurate name information is a key requirement in any service-oriented business operation and for the most part can be used to uniquely identify a person. Where common descriptors are applicable table lookups assure uniform usage of descriptors. Development Notes: ‘Given’ is being proposed in place of ‘First’ to conform to HL7 standard nomenclature.

‘Last’ is being proposed as opposed to HL7’s nomenclature of ‘Family’ because the term ‘family’ conveys a meaning not intended to be conveyed by a person’s last name or surname.

The Canada Post addressing standard does not limit the length of the name.

Presentation/Display Format: AAAA Valid Values: A person may be known by more than one name at the same time or over time. The various person name types include:

ALIS Alias - A personal name which an individual is using or has used.

PREF Preferred Name - The name a person prefers to use in communicating with other health system stakeholders.

LEGL Legal Name – The person’s name as legally recognized by the province.

PROF Professional Name - The professional name that is used by a person.

MADN Maiden Name – The name that an individual was born with, and has given up for a married name.

Business Rules / Coding Guidelines: Particular business areas may not require the entire list of name types. However, when using any of the specified name types, the business rules for applying the values must be consistent across business areas.

A stakeholder can have more than one name. This code differentiates between the different names associated with a stakeholder.

Implementation Considerations: Either the preferred or professional name would be used depending on business context and business unit.

An alias is a personal name recorded for an individual that is not supported by legal documents. However, telephone exchanges are only capable of using a 15 digit packet. The county/area/local combinations cannot exceed 15. 15 is expected to be the maximum for 5+ years. - It is predicted that North America will run out of phone numbers within 5 years. A country dial code is used to reach the particular telephone system for each nation (or each special service such as INMARSAT). An area code (routing code) is used within many nations to route calls to a particular city, region or special service. The local number is the specific telephone number to be dialed.

The International Telecommunications Union (ITU) is the global organization responsible for telecommunications standards, including the assignment of telecommunications zone code. Their Operational Bulletin lists changes to national telephone numbering formats in the "Telephone Service" chapter. However, current editions of the Bulletin are only available on the paying service, and not available for general web access.
