INTRODUCTION The two major categories of standards targeted by the Health Information Standards Committee of Alberta (HISCA) for definition and adoption are: - Health Information Standards; and - Health Technology Standards. Under the Health Information Standards category, data standards are defined from two perspectives: - Data Administration (Data Dictionary) Standards. This level of standard deals with fundamental properties of data elements, and focuses on definition, information exchange format, and use of the data element. - Data Architecture (Modelling) Standards. This level of standard builds upon the basic definition and properties of the data elements as defined at the data administration level. It extends their use in the context of standard applications (modelling requirements) and standard data groupings/collections (data sets). The standards described in this document are of the data administration type, dealing with the definition of data elements as they relate to the health business. The purpose of this document is to define standards for stakeholder demographic data elements. The elements included in this standard are those required to identify, locate, or communicate with stakeholders (persons and organizations). These data element standards are focused more on person-specific information, with less emphasis for now on organization-specific information. The definitions, formats and use of the data elements described in this document were gathered by reviewing provincial, national and international health information standards. These include HL7, BC Health Information Council Standards, Saskatchewan Health Information Standards Council, CIHI Health Information Framework, Government of Alberta Data Standards Sub-Committee (ADSS) data standards and the International Standards Organization (ISO). Key stakeholder organizations were then surveyed to obtain feedback on the standards, including the Alberta Health Authorities, health professional organizations, Alberta Wellnet, and business areas within Alberta Health and Wellness. There are several other stakeholder demographic data elements that have not been included in this current list of elements, but are candidates for future standards definition efforts. Some of these elements are for eligibility purposes and others are for additional information relevant to the health system but not required for identifying, locating or communicating with a stakeholder, for example Ethnicity. Introduction This section provides a brief description of the purpose and contents of the document. 2. Justification This section discusses the results of the literature search, the business case for the proposed standard, the impact on privacy, confidentiality, and security, the relationship to existing standards and/or legislation, and the summary of stakeholder consultation. 3. Data Models Conceptual and logical data models are included to provide context for the data elements defined in this document. 4. List of Data Elements. The stakeholder demographic data elements for which standards are proposed are categorized and listed in this section. 5. Stakeholder Demographic Data Standards This section includes the data elements detailed in the standard template. JUSTIFICATION Results of Literature Review Standards work from various jurisdictions were considered in the development of this standard. The Government of Alberta has a set of data standards that were reviewed for alignment. Both British Columbia and Saskatchewan, have initiated work to address a fully structured address that is Canada Post compliant. However neither standard has been implemented and therefore there was no complete solution to standardizing address attributes at the atomic data element level. The Pan-Canadian Client Registry Messaging Specification for the Canadian Institute for Health Information was considered for alignment wherever possible. Business Case for the Health System Once approved, this standard will be applied to the Alberta Health and Wellness central repository/directory for person information. All other applications will use the standard when exchanging demographic data with the repository. This standard may also be applied wherever stakeholder information is being collected, stored, or exchanged. Impact on Privacy, Confidentiality, and Security At this time, a PIA is not required. This standard sets the stage for a common opportunity to share information between custodians and affiliates. A PIA would be required for the custodians and affiliates that do wish to exchange stakeholder information between applications. The Health Information Act (HIA) is the primary legislation for prescribing the rules for the collection, use and disclosure of stakeholder registration information between custodians. Additional related legislation is contained in the Alberta Health Care Insurance Act (AHCIA), the Health Insurance Premiums Act and the Freedom of Information and Protection Act (FOIPP). All requests to collect, use and disclose identifiable stakeholder information through the Alberta Health and Wellness central repository/directory, must be reviewed and authorized by the Customer Services and Registration Branch Director. Relationship to Existing Standards and/or Legislation This standard is meant to be to an enhanced version of the originally approved standards approved in December 1999. Standards from Saskatchewan and British Columbia were reviewed. As well, standards from the Health Authorities and the Canadian Institute for Health Information were consulted. Where possible, attempts were made to align with the Government of Alberta data standards and the International Organization for Standardization (ISO). Permission to reproduce columns 1-4 from ISO 3166-1 and columns 1& 4 from ISO 639/2 was granted by IHS Canada, the official representative for the Standards Council of Canada, the International Organization for Standardization member body in Canada. No further reproduction is allowed, without prior written confirmation from IHS Canada at 240Catherine Street, Ste 305, Ottawa Ontario, K2P 2G8. Summary of Stakeholder Consultation The CIO of Alberta Health and Wellness originally approved the Stakeholder Basic Demographic Data Standard in December of 1999. The standardization process for HISCA includes an annual review of approved standards. In November of 2000 feedback was consolidated into a draft document. In November of 2001 the Provincial Person Health Identifier (PPHI) Project was initiated. The review of the stakeholder standard was extended to allow for an extensive consultation with the Health Authorities and Boards, Alberta Wellnet, the Alberta Data Standards Subcommittee, key business areas within Alberta Health and Wellness, and national bodies such as the Canadian Institute for Health Information. The purpose of the review was not to re-design the standard but to update key attributes.
