Accident & Incident Reporting Policy

This policy is applicable CST Schools Trust (CST). This policy is designed to be used by offices, schools and academies. The content and instructions are a mandatory part of the Health & Safety Management System. Deviation from this Policy (usually in situations where Schools and Academies choose to outsource their health and safety management support) must be agreed with the Property Services team.

Introduction CST is committed to providing a safe workplace for staff, pupils, and all others who may be affected by its activities i.e. visitors, contractors, parents etc.

Every incident that carries the potential to cause harm, or has resulted in an injury, near miss or damage to property should be reported. This should include incidences of violence towards staff, threatening behaviour, verbal abuse etc.

Legislation

CST has a legal duty under the Health and Safety at Work etc Act 1974 and the Management of Health and Safety at Work Regulations 1999; to record and investigate work related accidents, near misses, ill health and injuries.

The Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) 2013 require CST to notify serious work related injuries, occupational diseases and dangerous occurrences to the Health and Safety Executive. All reports to the HSE under the RIDDOR Regulations must be submitted by the Health, Safety & Business Continuity Manager.

Policy

It is the policy of CST that any accident, incident or work related ill health should be recorded in order to investigate incidents at our schools and make changes to our risk assessments and policies where appropriate. Refer to the Incident Category Matrix for actions to undertake in the event of an accident/incident.

Reporting an accident / incident

Refer to Incident Reporting Flow Chart. • If the incident has caused injury or harm - summon a trained first aider or the emergency services. • Notify the Responsible Person of any hazards that led to the injury, to ensure actions can be taken to remove the hazard and the area is made safe. • First aider dealing with the incident or the Responsible Person shall complete the Incident Report Form at the time of the incident with as much factual detail as possible. If appropriate ask witnesses to remain on site and obtain statements. • Responsible Person to take photographs of the scene. • All serious near misses MUST be reported on the Incident Report Form and investigated (dependent on severity), as they are good indicators of potentially hazardous situations. • CST will keep records of all accidents/incidents in order to identify trends and investigate as necessary. Personal information will be anonymous as data will be subject to onward reporting to the CST Leadership Team.

Location of the Incident Report Form

A template of the Incident Report Form is available on the CST drive. It is a recommendation to have hard copies of the Incident Report Form in close proximity to the first aid kits in locations known to all first aiders and Responsible Persons.

Types of Accidents/ Incidents

CST has an Incident Category Matrix which should be referred to when reporting accidents and incidents at the school.

st 21 Century Learning Alliance Award 2) Email completed forms to CST’s Health & Safety Advisor who will investigate and report to HSE where necessary 3. Complete the Incident Report Form and file on site. Reports will be reviewed as part of the Annual Property Services Audit (APSA) process.

The Reporting of Injuries, Diseases & Dangerous Occurrences Regulations 2013 (RIDDOR), require any workplace to notify the Health & Safety Executive in the event of certain injuries, diseases or dangerous occurrences that happen at their premises. There is a stringent time frame within which CST must legally notify the HSE. This is as follows:

• Death/Specified Injuries – report immediately to the HSE (without delay) • Work related accident resulting in over 7 day absence needs to be reported within 15 days.

As a result it is vital that CST are notified to ensure the correct report can be filed with the authorities. The F2508 (Notification to the Health and Safety Executive) will be made through CST and the Incident Contact Centre serial number will be recorded and shared with CST.

Where appropriate, the following key areas will be considered as a guide during the incident investigation: • Management control • Activity procedures • Information/Instructions • Environment/ Premises • Human Factor

Remedial actions to prevent a similar incident should be developed, planned and implemented. These could be immediate and/or long-term actions. The actions should be implemented within a set time scale and should be monitored and reviewed for their effectiveness.

Responsibilities

All staff, contractors and consultants will:

1) All employees are responsible for logging injuries or near misses and for notifying their manager if they are taken to hospital in connection with an injury related to work. 2) Staff are responsible for notifying their line manager immediately if they are diagnosed with an infectious disease so that this information can be passed on for further investigation.

st 21 Century Learning Alliance Award 3) If staff go home from being injured at work and subsequently have 7 days or more away from work (this includes all days, not just weekdays) they must inform their Responsible Person at once. 4) Comply with the requirements outlined within CfBT’s Accident & Incident Reporting Policy; 5) Employees have a responsibility to ensure their own safety and the safety of others; 6) Report any concerns related to the way in which accident/incidents are reported.

Managers (including Governors & Head Teachers), Business Managers, Heads of Department and Supervisors will: 1) Ensure the adherence to the Accident & Incident Reporting Policy at the site(s); 2) Ensure there is a suitable Responsible Person for the site; 3) Ensure employees under their control have sufficient and suitable training with respect to accident and incident reporting and that this training is regularly refreshed.

Site Managers, Facilities Manager, Caretakers, Responsible Persons will: 1) The management is responsible for ensuring that all aspects of the Accident & Incident Reporting Policy are followed; 2) Consult the Incident Category Matrix and complete the Incident Report Form as necessary. 3) Where an injury from a work related activity results in a member of staff being absent from work for a period of 7 days or more (including weekends) this must be reported to Health & Safety Advisor immediately. Where a work related injury results in a member of staff not being able to fully complete their normal work activities for a period of 3 days or more (including weekends) this too must be reported to the Health & Safety Advisor immediately;

4) The site specific Responsible Person is tasked with ensuring the Incident Report Form is available in a location known to all staff.

Health & Safety Advisor and Real Estate & Compliance Manager will: 1) Ensuring this policy remains up to date and in-line with regulatory requirements; 2) Notify HSE of RIDDOR reportable incidents and conduct an an incident investigation. 3) Ensuring a suitably competent Responsible Person is nominated to oversee this management system; 4) Ensuring adequate training is delivered to Responsible Persons and other key stakeholders; 5) Provide advice and guidance to colleagues and stakeholders; 6) Act as the lead manager on all RIDDOR reportable incidents.
