AMBULATORY SURGICAL CENTERS (ASCs)

These are medical facilities specializing in elective same-day or outpatient surgical procedures. Patients treated in these facilities are not required to be admitted to the hospital and usually are well enough to leave after the procedure. AREA HEALTH EDUCATION CENTERS (AHECs)

These are non-profit organizations aiming to provide health service learning opportunities and improve the education and training of health professionals, primarily primary care providers, in rural or underserved communities. Trainees include medical, dental, physician assistants, nursing, pharmacy, and allied health. One of AHECs’ missions is to invest and increase diversity in future healthcare workforce and therefore AHECs routinely make outreach efforts available for K-12 students through health career camps, science enrichment programs, health careers programs, etc., specifically targeting minority /underrepresented groups.

ASSISTED LIVING FACILITIES (ALFs)

These facilities are a great housing alternative for older adults that need some assistance with daily personal care, such as dressing, bathing, eating, and toileting, but do not need the round-the-clock medical and nursing care of a nursing home. FEDERALLY QUALIFIED HEALTH CENTERS (FQHC)

A type of safety-net health care provider, FQHCs are federally funded, nonprofit health centers or clinics that served medically underserved areas and populations through a special Public Health Service funding mechanism from the Centers for Medicare and Medicaid Services. FQHCs provide primary care services regardless of the patient’s ability to pay. Services are also provided based on a sliding scale fee based on the patient’s ability to pay.

HEALTH HOMES

A Health Home is a Medicaid health care management service model for those living with multiple health chronic conditions, including mental health, substance use disorders, and HIV. REHABILITATIVE SERVICES These are services designed to facilitate the process of recovery from injury, illness, or disease and restore various physical functioning to as normal as possible. Rehabilitation services assist the patient to compensate for deficits that cannot be treated medically. Some of these services include physical therapy, occupational therapy, and speech therapy. SKILLED NURSING HOMES

These are living facilities for seniors with health services provided by a team of health professionals under the supervision and oversight of a Medical Director. The clinical team includes medical doctors, licensed practical, vocational, and skilled and registered nurses, as well as rehabilitative staff, to manage, provide, observe, and evaluate care.
