TBHC’s Universal Faculty Evaluation Form

Purpose: the goal of developing this Universal Faculty Evaluation Form is to provide faulty, Program Directors, and Chairmen with objective criteria to evaluate the faculty of our training programs. While we do not expect that all faculty will contribute to all of the items listed below, this scoring system will allow us to provide equivalency for their contributions to the training programs. Furthermore, by using a universal template, this permits the Hospital to provide meaningful measurements on significant outliers—and will permit us to acknowledge our master educators and roles models.

In section 1a, the faculty are evaluated by the residents in the 6 competencies. We recognize that it may be considered redundant to list Patient Rounds, as it is a component of patient care, but we feel that much of the practice of medicine is learned at the bedside, during clinical and teaching rounds, and we wish to stress the importance of this endeavor. Section 1b will permit each program to add specialty specific items, if needed. In section 2, the Program Director must review each faculty member’s scholarly, educational, and administrative contributions to the program—as required by your respective accreditation body (RRC or CODA). It is expected that the Program Director and Chairmen will work in conjunction to evaluate the faculty and oversight will be provided by the Chief Academic Officer.

Actions: it is our intent to provide this objective scoring system to provide each faculty member with clear guidance on the important matters for the residency training programs. Feedback via these evaluative forms will occur on an annual academic cycle, usually coinciding with the Annual Program Review. Faculty development and requirements will be made based upon this review.
