The aim of the directory is to provide information about organisations which are here to help and forms part of the Mid Sussex Wellbeing Service.

The Hub provides advice, guidance and support to make lifestyle changes such as losing weight, eating well, getting more active, reducing alcohol intake and stopping smoking.

Exercise Referral Exercise referral is aimed at individuals who are not used to regular exercise, or who are recovering from, or dealing with, a health issue. After referral by a health professional, a coordinator discusses the reasons for referral and develops a programme to match their aims and abilities.

Time to Talk is a friendly and approachable service offering talking therapies to people who are struggling with the following issues: Stress, worry and general anxiety, depression, panic attacks and agoraphobia, phobias, social anxiety, obsessive compulsive disorder (OCD), post- traumatic stress disorder, health anxiety, post natal depression, low self- esteem or low confidence, relationship difficulties or bereavement and reactions to loss. Referral to Time to Talk is through a GP, other health professional or self referral. They will complete a referral form and give you information on how to contact the service and book your appointment.

Specialist Day Services WSCC

Specialist day services are for people with longer term or complex social care needs, such as adults with learning disabilities or dementia.

The day services include support, activities and community opportunities that help adults to live as independently as they can and be a part of their own community.

CRi

CRi aims to support people affected by drug and alcohol problems. CRi does this by offering a wide range of services, including assessments, prescribing and onward referral to other specialist agencies.

The walks are designed to help you get fit and there are a variety of different levels of walks from Level 1 (beginner) to Level 5 (advanced) so there is something for everyone. The programme allows you to progress through the levels as your fitness improves.

Al-Anon Al-Anon Family Groups provide support to anyone whose life is, or has been, affected by someone else’s drinking, regardless of whether that person is still drinking or not. Al-Anon Family Groups hold regular meetings where members share their own experience of living with alcoholism. Al-Anon does not offer advice or counselling, but members give each other understanding, strength and hope.

Healthwatch West Sussex is independent, transparent and accountable. Healthwatch want to hear about your experiences - the good and the bad - so that they can provide feedback and hold organisations to account.

Stop Smoking Service Smokefree West Sussex provides free support with a trained advisor to help people quit smoking. The service, which is provided on a one-to-one basis or in group sessions, is available in a variety of locations across Mid Sussex, including GP surgeries, pharmacies and community venues. Engaging with the CCG There are many ways for you to get involved - from sharing your views of NHS services in a focus group; being part of a project group; or by filling out a survey. New opportunities will arise all the time, so please check the website often.

Mid Sussex Falls Prevention Services

Mid Sussex Falls and Fracture Prevention Service

The Mid Sussex Falls and Fracture Prevention Service will visit a person at home, who has fallen or is at risk of falling, for an assessment. The service will offer advice and an individual exercise programme, where appropriate, to improve strength, balance and confidence to reduce the risk of further falls. Referrals are accepted from GPs and other health professionals.

Princess Royal Hospital Balance and Bone Team (Haywards Heath)

The team runs an outpatient service, specialising in balance problems and falls. The team provides diagnosis, treatment and management of care through medical clinics, exercise groups or individual treatments. Referrals are accepted from GPs and other health professionals.

Queen Victoria Hospital Falls Prevention Service (East Grinstead)

The team assesses people in their own home, looking at risk factors, and refer people to appropriate treatment based in their own home, the Rehabilitation Unit or into the eight-week Balance and Bone Group. Referrals are accepted from GPs and other health professionals.

The Bladder and Bowel service offers individuals with bladder and bowel problems assessment, treatment, advice and support.

Individuals are seen in clinics, which are run at different venues, on a regular basis across West Sussex. Individuals with complex needs may be seen at home by a specialist nurse if they are unable to come into clinic.

The service provides advice and education to other healthcare professionals, carers, families and the general public.

St Peter & St James Hospice & Continuing Care Centre

St Peter & St James Hospice is a registered charity that provides specialist care, compassion and support for those living with progressive, life limiting illnesses from East and West Sussex. The Hospice’s aim is to encourage and enable all patients to live their lives to their full potential despite their illnesses.

St Catherine’s Hospice

St Catherine’s Hospice is a charity dedicated to providing specialised end of life care and support to local people, their families, friends and carers. Hospice care is based on the belief that each person is more than their illness, and that each of us has unique physical, emotional, social and spiritual needs. People are cared for in the hospice, in their homes and in nursing homes. The hospice also provides training, advice and support to other health professionals.

www.midsussexwellbeing.org.uk Tel: 01444 477191 11 Health Matters Wheelchair Service - West Sussex (Sussex Community NHS Trust) The Wheelchair Service will loan a wheelchair to anyone who has a long term mobility problem and is classified as unable to, or virtually unable to walk indoors or outdoors. The wheelchair must also be required for longer than three months.

You must be registered with a West Sussex GP and living in your own home. Referral forms will be accepted from GPs, medical consultants and other health care professionals, such as Occupational Therapists.

Carers Heath Team The Carers Health Team can visit you at home and check your general state of health and wellbeing. The team can offer advice on how to stay healthy and manage any health issues you might have. The Carers Health team is a free service provided by Sussex Community NHS Trust for carers living in West Sussex. Our aim is to improve both your own health and that of the person you are looking after.

The Northern Prevention Assessment Team (covering Crawley, Horsham and Mid Sussex) includes, health advisors (nurses), social care workers and support workers.The team aims to help people to stay healthy and remain independent. Normally they work with people who do not receive services from specialist health care teams or social care services. The team members can check your general state of health and wellbeing, help identify what may benefit you to retain your independence, provide assistance to access practical, emotional and social support or link you with agencies to ensure you are claiming the benefits you are entitled to.

NHS Choices & NHS 111 NHS Choices aims to provide people in with expert health advice, information and reassurance to care for themselves at home or to access appropriate healthcare, using the website.

NHS 111 You can use the NHS 111 service if you urgently need medical help or advice but it’s not a life-threatening situation. NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones.

Citizens Advice Bureau (CAB) The Citizens Advice Bureau provides free, confidential and independent advice. There are advice centres located in Haywards Heath, Burgess Hill and East Grinstead.

The CAB can offer advice to help people resolve their problems on a range of subjects, including debt, benefits, housing, legal, discrimination, employment, immigration and consumer issues.

The Pensions Advisory Service (PAS) is an independent, non-profit organisation providing free information, advice and guidance on state, company, personal, stakeholder and occupational pensions. It also helps any member of the public who has a problem, complaint or dispute with their occupational or private pension arrangement.

TaxAid TaxAid is a charity that helps people on low incomes with their tax affairs. TaxAid helps people on low incomes to understand the bits of the tax system that apply to them so that they pay only the right amount of tax.

TaxAid runs a national helpline and also gives advice via email. Their website is designed to make tax understandable to any taxpayer.

West Sussex Credit Union

A Credit Union is a savings and loans co-operative that is owned by its members. As a member you can borrow money at competitive interest rates and your savings are protected by the FSA Compensation Scheme. If you have a low income or no borrowing record, a Credit Union may be more willing to help than banks or building societies, so you can avoid using a high-interest lender.

Housing Benefit and Council Tax Support

Housing Benefit is a means-tested benefit that is designed to assist people to help pay their rent. If you are on a low income and you rent your home you may be able to get Housing Benefit. You cannot usually get Housing Benefit if you live in a close relative’s household and you can’t get Housing Benefit if you own your home or to help with mortgage payments.Council Tax Support is a mean’s tested benefit that helps people on low incomes pay their Council Tax.

Buy With Confidence Scheme and Consumer Direct Buy with Confidence is a scheme run by Trading Standards. All members of the scheme are audited by Trading Standards to ensure the trader is committed to operate in a fair and honest way. This includes a CRB check for members working in consumers’ homes.

Eligible disabled people may travel free at any time within West Sussex. As an alternative to the bus pass, West Sussex currently offers either a senior or disabled person’s railcard that offers a third off off-peak rail travel.

Mid Sussex District Council, Concessionary Travel

Mid Sussex District Council runs a scheme to help older people and disabled adult residents in Mid Sussex with the cost of travel.

The Blue Badge scheme is for people with severe mobility problems. It allows Blue Badge holders to park close to where they need to be. The scheme operates throughout the UK and is managed by the County Council who deals with applications and issues Blue Badges.

The service offers a wide range of menu choices, covering cultural as well as dietary needs, and including special requirements like soft foods or diabetic meals. Drivers deliver a daily chilled meal which they will cook for you in your microwave or leave for you to heat when you choose to eat it.

Independent Lives Independent Lives provides a range of support services for people with disabilities, care needs and caring responsibilities to enable them to lead full, active and independent lives. Independent Lives core work focuses on providing a direct payments support service to help people arrange and manage their own personalised care and support.

Elderly Accommodation Counsel (EAC) The EAC is a national charity that aims to help older people make informed choices about meeting their housing and care needs.

EAC has twin aims, firstly, to deliver an information and advice service directly to older people, their relatives and carers, and, secondly, to raise awareness amongst advisory agencies, professionals and policy makers of the importance people attach to information and advice which helps them to make their own decisions about how and where to live in older age.

Age UK West Sussex provides advice on a wide range of issues affecting people in later life, their families and carers. They work through a telephone-based personal alarm system providing rapid response at any time of the day and night. A unit is connected to your phone line and if you require help you simply press a button. Pendants are provided so you can summon help wherever you are. It may be that you want to move to a smaller property or be closer to relatives. The housing team can also advise you on the differences between sheltered housing and Extra Care housing, and how to access these, and about the range of specialist housing for older people in the district.

Shelter West Sussex Service Centre Shelter West Sussex offers specialist advice, information and advocacy on housing and community care for people living in the West Sussex area. Neighbourly Care in Hassocks, Burgess Hill and District can help with escorted shopping, local transport, light help in the home, minor household repairs, befriending/visiting or small gardening jobs for those who have no other way of getting help. 4SIGHT 4SIGHT works to support blind and partially sighted people in West Sussex and provides people with a wide range of practical support solutions to help them remain independent following sight loss. It does this in many different ways through services, clubs, visits, information, guidance, advocacy and support.

In addition, 4SIGHT offers low vision assessments, a listening and information service (based in local eye clinics), equipment loan and purchase, British Wire- less for the Blind, transport, transcription, an extensive audio library backed up by home visits, where requested, and a newsletter. RNIB RNIB provides help, support and information to people living with serious sight loss. It provides advice, support and information on eye health, including eye conditions, preventative measures and living with sight loss. It also offers practical help for blind and partially sighted people to support their everyday life, such as help to manage at home, travel, education, leisure activities, reading and writing, computers, TV, radio and film.

The service visits clients where they are living: at home, in hospital, or in residential and nursing homes. Counsellors are professionally qualified volunteers.

You can self refer or refer a relative or friend. Available as one-to-one, face-to- face counselling, or via the telephone or online.

There is no set charge, but a donation is asked for to cover costs.

CARE - East Grinstead CARE is available for residents of East Grinstead, Dormansland and Lingfield. The Cinnamon Trust The Cinnamon Trust is a national charity that helps elderly people and their pets. It does this by providing a home for pets when their owner dies or moves into residential accommodation that will not accept pets, a fostering service should the owner go into hospital and a dog walking service.
