Introducing the new East Community Health and Social Care Partnership

Introduction NHS and have joined forces to create a new Community Health and Social Care Partnership for East Glasgow. This leaflet is designed to provide information on this new Partnership, including details of the Public Partnership Forum which is being created to give you a greater say in the development of your local services. How many CHPs are being developed across Greater Glasgow? Nine CHPs are being developed across Greater Glasgow. Five of these are within the boundaries of Glasgow City Council, including one covering the East of the city. What area does the new East Glasgow Community Health and Social Care Partnership cover? The new East Glasgow Community Heath and Social Care Partnership covers the East end of the city including the following communities: , , , , Bridgeton, Broomhouse, , , , Craigend, Craigend Muir, , Crosshill, , , , East City Centre, , Fullarton, , , , , Greenfield, , , Mount Vernon, , , Riddirie, , , , , Swinton, Tollcross, , Trongate and West City Centre. The East Glasgow Community Health and Social Care Partnership will also provide local health services to the , , , , and Muirhead areas of NHS Greater Glasgow which lie within the boundaries of North Council. The present boundaries for the Partnership may be altered slightly to reflect future changes to Glasgow Community Planning Partnership boundaries and Glasgow City Council electoral ward boundaries.

What difference will this new Partnership make to patients? The East Glasgow Community Health and Social Care Partnership will develop closer partnerships with local council and hospital based health services to break down some of the barriers that have traditionally existed between community health, social care and specialist health services. This will create new opportunities to develop additional health services at a local level. It will also make it easier for you to access the local services you require regardless of whether these are provided by your CHSCP, local hospital or local council.

How will I access local health and social care services in the future? Local health and social care services will continue to be provided in local health centres, clinics and social work offices across East Glasgow.

How will the new Partnership be developed? A draft ‘Scheme of Establishment’, which sets out the broad principles for the development of the five new Glasgow CHSCPs, including details of the services they will manage and how these will be organised, has been submitted to the Scottish Executive for final approval. A local Development Group has also been formed to develop the new East Glasgow Community Health and Social Care Partnership over the next year, working closely with local staff, community groups and others with an interest in the development of local health and social care services.

How can I get involved in the development of local health and social care services? All CHPs are required to develop a Public Partnership Forum (PPF) by 1 April 2006. This will give service users, carers, local community groups and voluntary organisations an opportunity to get involved in the development of local services and provide feedback on a wide range of local issues. The PPF will build on existing local patient and public involvement arrangements to develop closer links with community groups and voluntary organisations across East Glasgow.
