Profession: Statistical Epidemiologist

Jessica is a Senior Statistical Epidemiologist working with MRC/UVRI Uganda Research unit on AIDS with over 15 years in research involving clinical trials, field epidemiology studies, community based trials, and Information, Education and Communication (IEC) activities; and 20 years of statistical and data management skills. She has been the lead Statistician in 17 scientific publications including HIV incidence in a general rural population cohort, and recently in highrisk fishing communities. She has 6 years experience in midlevel management, as head of the MRC statistics section which handles all the quantitative data for MRC/UVRI. During this period there were over 20 concurrent studies whose data management and analysis she supervised, which equipped her with skills of: supervising; training; and monitoring data entry, data cleaning, and data management; while maintaining high data quality in timely and efficient ways.

Training in statistics and analytical methods and skills has been part of her job for the last 15 years. From 2001, she became involved in various activities of university postgraduate education including lecturing MSc (Clinical Epidemiology) students at Makerere University, and tutoring MSc students at London School of Hygiene & Tropical Medicine (LSHTM). Since 2007, MRC introduced annual 2 weeks' courses in Epidemiology and Statistics for scientists from within Uganda and SubSaharan Africa. She has been involved in organising and conducting these courses, which has greatly improved her training skills especially to individuals with limited statistical knowledge and skills.

Specific Projects that Jessica has worked on 1. The Pneumococcal Vaccine Trial (PVT) This doubleblind placebocontrolled trial of the 23valent Pneumococcal polysaccharide vaccine in HIVinfected adults was conducted through the Entebbe TASO and Ministry of Health UVRI clinics. Recruitment of subjects started in 1995 and vaccination ended in June 1998. Followup of this cohort ended in June 2009 at the end of operational studies and prevention trials. Jessica setup the data entry screens, managed the data, and was the only Statistician on this Trial. Several presentations and publications came out of this trial including 2, 20, 22, 23, 26, 38, 40, 41, 46, 49 and 50 below.

2. The General Population Cohort (GPC) This GPC was set up in 1989 in a cluster of 15 neighbouring villages which were expanded to 25 villages in 1999. This cohort has been kept under epidemiological surveillance for HIV infection through annual census, facetoface interviews, and serosurveys. Jessica managed GPC data from August 1991 to July 1993, and has since 1993 been doing statistical analysis using this database. For most survey rounds, she was involved in questionnaire design, interviewer training, and survey planning. She used this cohort and data for her PhD research, and is currently supervising statistics section staff working on this project. Publications from this data include: 1, 10, 12, 17, 21, 24, 25, 31, 36, 37, 47, 48, 52, 53, 54 and 55 below.

3. TB and Worm studies These studies were designed to examine the human immune response to Mycobacterium , and interactions between TB, HIV infection and helminth infestation. They used the

3 04/03/13 former PVT cohort and Mulago Hospital. Jessica managed the data for these studies and did some of the statistical analyses. Publications include 28, 29, 33, 34, 35, 43 and 51 below.

4. Masaka Intervention Trial (MIT) This was a communityrandomised controlled trial, which aimed to determine the effectiveness of a behavioural change intervention, through Information, Education and Communication (IEC) alone or in combination with improved STD management on HIV transmission in an adult population of Masaka district and surrounding areas. This trial started in 1994 and was completed in 2000. Jessica mainly worked with the IEC section which involved behavioural interventions in the community and schools. She managed the data for IEC and did some analyses for both IEC (publications 39, 44, and 45 below) and for the main MIT survey work (publications 27, 30, 32 and 42 below).

5. Rogers Fellowship supervision Between April 2002 and December 2003, Jessica supervised a Rogers Fellow Ms. Sarah Kisa who was attached to the Statistics section. Her topic was “The Development of a Simulation model of the HIV epidemic in rural SouthWestern Uganda, and Investigation of the potential impact of various interventions.

6. The DART Trial The Development of AntiRetroviral Therapy in Africa (DART) was an openlabel randomised trial evaluating two strategic approaches for management of antiretroviral therapy (ART) in symptomatic HIV infected adults in Africa. The first strategy compared Clinical Monitoring Only (CMO) with Laboratory plus Clinical Monitoring (LCM). The second approach (terminated prematurely) compared Structured Treatment Interruptions (STIs: 12 weeks on, 12 weeks off ART) with continuous ART in patients who achieved CD4 cell counts >=200 cell/mm3 after 24 or 48 weeks on continuous ART. Enrolment at the 4 African sites (JCRC, Academic Alliance and MRC in Uganda, and University of Zimbabwe in Zimbabwe) started in January 2003 and ended in February 2004. Followup for this trial ended in 2008. Of the 3000 patients in the trial, 1000 were recruited at MRC, Entebbe. Jessica was in charge of the statistics and data management staff in MRC for this trial and coordinated data handling at the MRC site. She was involved in the trial design and protocol development and as such, was a member of the DART trial committee, and was coauthor on the publications: 4, 5, 7, 8, 9, 11, 14, 15, 16, 18, and 19.

7. Primary prevention of invasive cryptococcal disease using fluconazole prophylaxis This was a double blind randomized controlled trial of fluconazole as primary prophylaxis against invasive cryptococcal disease in HIV infected Ugandans living in Masaka. Its objective was to assess whether routine use of fluconazole as primary prophylaxis in HIVinfected African patients was effective in reducing the incidence of invasive cryptococcal disease and all cause mortality. From September 2004, 1540 nonpregnant adults with a CD4 count <200 were recruited to the study and randomised to fluconazole 200mg three times a week or identical placebo. Follow up ended in May 2008. Jessica was the trial statistician and member of the Data Monitoring Committee until September 2005.

8. The Fisherfolk communities studies This was a prospective cohort study set up in 2009 to determine HIV incidence, and risk factors for HIV infection, and to describe the molecular epidemiology and the social and behavioural characteristics in fishing populations of two lakeshore districts in Uganda in preparation for future HIV prevention research. Other substudies on the prevalence of worm infection in these communities; the relationship between schisto and HIV are also being conducted in these communities. Jessica is the Statistician on these studies and supervises statistics section staff working on these studies. Publication 3, and 6 is from this study.

9. The impact of orphanhood and education outcomes This was the research was done for Jessica’s PhD studies. It was carried out within the GPC (study 2 described above). During the study period (19892007): parental links for children aged <18 years were established; 18year time trends of orphan prevalence, orphan incidence, and education outcomes by orphan type (maternal, paternal, and double), calendar period, and agegroup were also 4 04/03/13 estimated. The aim of this study was to measure and better understand the causal link between child orphanhood and education outcomes, and the relative importance of parental characteristics, children’s attributes, and fostering arrangements in this relationship. A crosssectional study nested within the GPC collected more data on education experiences, parental characteristics, and literacy to enable a relatively comprehensive analysis of the effects of orphanhood on child education. A schools’ education survey on education quality in selected primary and secondary schools was also done. A PhD dissertation (13) on this study is available.
