PREFACE

The Student Manual is the official statement of the College policies and regulations and expected standards on the provisions of the student services that are due to all students. The Office of the Vice President for Academic Affairs and the Office of Student Affairs and Services have initiated the revision of this Student Manual to serve as guide for all students of the Ilocos Sur Polytechnic State College.

This manual includes the general information of the College and its accredited programs. Important features were discussed and updated in every chapter which includes: General Academic Policies and Procedure, Student Services Sector, the Office of Student Affairs and Services which had concentrated on giving details on matters related to Student Welfare Services, Student Development Services and Institutional Student Programs and Services. Important inclusions in relation to Student Programs and services were the new provisions pertaining to Scholarships (RA 10931-Universal Access to Quality Tertiary Education Act), and the Policies and Guidelines for Local Off-Campus Activities (CMO No. 63, s. 2017). Other approved guidelines, policies and procedures were also included like those of the incentives to Board Examination Placers and Cash Incentives to Student Achievers.

The 2019 Edition of the ISPSC Student Manual was approved by the Board of Trustees through Board Resolution No. 85-1128, s. 2019 dated September 18, 2019 and will be implemented starting October 2019, S.Y. 2019-2020.

Hopefully, this Manual shall serve as a common guide for students during their stay in the College.

Ilocos Sur Polytechnic State College is a vibrant and nurturing academic community that is very responsive to the needs of the people. It is highly committed to provide quality education and services at the highest standards to produce graduates who are globally competitive and great contributors to nation’s economy. To fully nourish, protect and care its manifold clients is a primary concern. And through its dutiful performance of its fourtold mandates, it genuinely serves its valued stakeholders.

To realize this vision, the College has institutionalized policies and regulations that are well crafted and designed to instill discipline and order in the community and to suit the needs of the students. Allowing students to pursue academic growth in a community that provides desirable experiences make them productive and engaged to the operations of the College.

The Student Manual is the official statement of the College policies and regulations, and expected standards of conduct that are applicable to all students. Policies and regulations are often not more specific than necessary and often are general enough to allow the College to responds to situations in their unique complexities and take into account the variations in values and goals of different academic units and administrative offices. As such, they provide a broad framework that is designed to create a level of consistency.

The primary purpose of policies and regulations, and the articulation of the expected standards of student conduct, is to further the vision and mission of the College and to protect the well-being of the community. These formulate policies and regulations enable all the members of the College to function as a community and respond to situations that threaten or violate order and discipline.

Making amendments and modifications of the policies and procedures is necessary to address new issues and questions, and to reflect the evolution of our community and the larger society within which our College exists.

It is therefore a challenging task for everyone in ISPSC community to understand policies and regulations in a larger context. This will allow us to fully render our services in accordance to our mandates. With high hopes and expectations, our commitment shall be profoundly rooted to the vision we try to achieve which is to transform lives and communities.

A. GENERAL INFORMATION

A.1.Brief History The Ilocos Sur Polytechnic State College was created by virtue of RA 8547 authored by the then congressman of the 2nd District of Ilocos Sur, Hon. Eric D. Singson. The charter was signed into law by President Fidel V. Ramos on February 24, 1998. ISPSC is a comprehensive multi-campus institution of higher learning with its main campus situated in Sta. Maria, Ilocos Sur. The other campuses are strategically located in five municipalities in the second district of the province. Its eight (8) original campuses was reduced to six (6) with Salcedo Campus and Suyo Campus reverted back to the Department of Education. The main campus was the former Ilocos Sur Agricultural College (ISAC) which had it early beginnings as a farm school way back in 1913. It evolved into an agricultural college in 1963 by virtue of RA 3529 authored by Cong. Pablo C. Sanidad. In 1995, RA 7960 was passed into law which called for Cong. Eric D. Singson’s vision for establishing a multi-campus polytechnic college in the second district of Ilocos Sur. Presently, the main campus located along the national highway at Pob. Norte, Sta. Maria, Ilocos Sur has its own Provincial Institute of Agriculture as evaluated by NAFES. The Campus is formerly the Narvacan School of Fisheries (NASOF) which was established in 1964 by virtue of RA 3476 authored by Congressman Pablo C. Sanidad. It is situated in the coastal barangay of Sulvec, Narvacan, Ilocos Sur and has an area of more than 10 hectares which was donated by three philanthropic families of the said barangay. Known as the former College of Fisheries and Marine Sciences, Narvacan Campus was also evaluated by NAFES as Provincial Institute of Fisheries. The Santiago Campus, formerly known as the Ilocos Sur Experimental Station and Pilot School of Cottage Industries (ISESPCI) was established in 1974 by virtue of RA 4430. It is situated on a 3.5 hectare area along the national highway in the municipality of Santiago, Ilocos Sur. It had been offering post-secondary courses since 1989 before its integration. At present, Santiago Campus house the College of Technology. The Tagudin Campus, formerly known as Tagudin General Comprehensive High School (TGCHS) is the oldest and the biggest in terms of student population. It started in 1916 as a municipal high school. It became a national high school in 1968 by virtue of RA 1477 courtesy of Cong. Pablo C. Sanidad. Tagudin Campus was designated as the College of Arts and Sciences from 1998 to year 2009. The Cervantes National High School of Arts and Trades (CNSAT) started its operation in 1972. It was converted into Cervantes National Agro-industrial School (CNAIS) by virtue of RA 6541 in 1983. Cervantes Campus is found in upland, interior municipality where its terrain is hilly and sloping, appropriate for livestock production, orchard, and other agro-forest crops. Cervantes Campus was then the College of Agro-Industrial Technology. The Campus, formerly known as the Southern Ilocos Sur School of Fisheries (SISSOF) metamorphosed from a fishery demonstration farm then known as the Ilocos Sur Marine Demonstration Farm (ISMDF) by virtue of PD 1050. It is located in the coastal barangay of Darapidap, Candon City, Ilocos Sur, occupying an area of more than 11 hectares which was donated by the local government of Candon during the incumbency of then Mayor Eric D. Singson. With its integration into the polytechnic college, it became the former College of Commercial and Social Services and now offers the program of College of Business Management. Superintendent Apolonio P. Labuanan, who worked hard and who lobbied generously for ISPSC’s conversion, was the first Administrator who served as Officer-in-Charge, of the office of the President from February 24, 1998 to July 15, 1998. Dr. Alejandro V. Directo, assumed as the first President on July 16, 1998 and has served for two terms with an extension that ended on December 31, 2007. As president, he laid the ground work for the state college. With his “SMILE” Principle, he advocated service with Sincerity, Morality, Integrity,

Loyalty and Efficiency & Effectiveness. There were noticeable and exemplary accomplishments along the four-fold functions of the College which included the accreditation of curricular programs. The second President, Dr. Rafael B. Querubin, re-engineered the college for a total make-over. He championed the “CHARMED” paradigm, a 7-Point Development Agenda which calls for: Community Capability Building and Responsive Networking; Human Resources Development and Career Building; Aggressive Academic Achievement; Resource Generation and Wise Allocation of Resources; Maximized Used of Resources and Effective Maintenance Program; E-Governance Management and; Developing Physical Facilities and Building Structures. He instituted a new set-up and drafted the Five- Year Development Plan of the College, 2011-2015 with three Vice Presidents: Academic Affairs, Administration and Resource Generation and Planning, , Extension and Training. The new set-up clustered the three (3) northern campuses of Narvacan, Sta. Maria and Santiago as the North Cluster. The other three (3) southern campuses of Candon, Tagudin and Cervantes converged to form the South Cluster. In adherence to CHED’s function of Quality and Excellence, the College has two (2) accredited Graduate Programs: Master of Science in Education and Master of Science in Agriculture in 2010. Different undergraduate programs of the college were also accredited by the Association of Accredited Chartered Colleges and Universities of the (AACCUP) which includes Bachelor of Elementary Education, Bachelor of Secondary Education, BS in Home Technology Education, BS in Hotel and Restaurant Management, BS in Agriculture, BS in Information Technology and BS in Industrial Technology. Since the chartering of ISPSC in 1998, much had been accomplished in terms of academic, technical, facilities and plant development. Linkages had been strengthened, enrolment had increased, programs levelled-up and the employees upgraded themselves, Collective Negotiation Agreement (CNA) benefits were granted and student achievers had tremendously increased. In January 2016, the new SUC President Dr. Francisco D. Lopez, continued the excellence in the delivery of service and in achieving greater heights of success for the College. The ISPSC had change remarkably in most of its area of service and achievement of such were being guided by his Strategic Development Goals for the College with an acronym “BRAVE D’ STORM” which stands for: Bridging networks and building communities; Resource Generation and Employees empowerment; Accreditation and Development of Academic programs; Virtual technology systems and website development; Empowering student services and enhancing Pedagogical system; Development of Quality, relevant and accessible Undergraduate and Graduate School system; Scholarship and Financial Assistance; Training of Employees for Global competitiveness; Organizational immersion and Revolutionary extension activities; Revolutionized Research and Gender Development programs; Maintenance, modernization and mitigation measures. New buildings and relevant infrastructures were constructed during the administration of Dr. Francisco D. Lopez which includes, the New Agriculture Building, Engineering Building, Graduate School Building, Graduate School Building Annex, Training Center, Refurbishment of the College Gymnasium, College Clinic, Women’s Dormitory, New Hostel Building, beautification of the perimeter fences, The New Academic Building, widening of Access Roads and the College Grandstand and the procurement of school buses. Aside from its infrastructure projects the college has strengthen existing international, national and regional linkages, promote student inter-country mobility, increased percentage of accredited programs by the AACCUP of which CTE Programs of Sta. Maria and Tagudin Campus acquired Level III Phase II accreditation. In terms of scholarship and financial assistance, in the school year 2016 and 2017, the college was able to process the application of 8,817 qualified scholars and in terms of extension activities, the college has intensified Community Engagement through Technical Advisory and Training Programs. Research outputs has also been intensified as manifested on the increased number of conducted researcher and utility models which had been approved by the patent office of the country.

Guided also by his vision as “A vibrant and nurturing Polytechnic Service College”, it continues to be the beacon of hope to the people in the different service areas together thread the path towards greater heights.

B. GENERAL ACADEMIC POLICIES AND PROCEDURES B.1. General Admission Policies 1. Eligibility for admission of a student shall be graduates from Senior High School level recognized by the Department of Education.

2. High school graduates of the old basic education curriculum may enrol as first year college students under the new higher education curriculum subject to the required bridging programs for the general education components.

3. First year, second year, third year, fourth year college students under the old higher education curricula who stopped schooling and intend to re-enrol may be admitted subject to admission requirements of the college. For professional or major subjects taken by the students other than general education, the college shall implement their own policy for crediting.

4. Transferees are required to take the admission examination and the Competency Assessment Evaluation. Admission requirements in terms of general weighted average shall be strictly observed.

For Board Programs: 1. A qualifying interview to all applicants shall be administered by the College. Credentials submitted shall be evaluated and verified by the evaluation committee. Qualified to be admitted are senior high school students, returnees and transferees with general weighted average of 83% and above.

2. The result of the ISPSC College Admission Test (ICAT) shall be the basis in the acceptance to the College. For Board Programs, the passing percentage is 75% and above.

3. Students who did not meet the prescribed requirement for the board programs are re- directed to enrol in the non-board programs.

For Non- Board Programs: 1. A qualifying interview to all applicants shall be administered by the College. Credentials submitted shall be evaluated and verified by the evaluation committee. Qualified to be admitted are senior high school students, returnees and transferees with general weighted average of 75% and above. 2. The result of the ISPSC College Admission Test (ICAT) shall be the basis in accepting student to enrol in the College.

Additional Admission Policies for Specific Programs A. Bachelor of Science in Hospitality Management (BSHM) 1. Graduates of the Alternative Learning System (ALS) and have passed the Philippine Educational Placement Test (PEPT) to qualify for college.

2. Must pass the interview conducted by the institute dean or assigned faculty. 3. Non-ABM graduates will be asked to enrol the prescribed subjects by CHED (CMO # 62, s. 2017 – Sec. 9, 9.3.2) as follows:    Fundamentals of Accounting/Business and Management – 3 units    Organization and Management – 3 units    Business Marketing – 3 units    Business Finance – 3 units    Applied Economics – 3 units 4. Must be physically and mentally fit. A medical certificate should be submitted; indicating the following tests: Hepatitis A and B, Urinalysis, Fecalysis, chest X-ray, ECG and Drug Test. The applicant should not have any illness that could restrict him/her from performing rigorous jobs and activities.

B. Bachelor of Science in Tourism Management (BSTM)

B.2. General Retention Policies 1. Students are classified according to academic status depending on the scholastic performance: a. CLEAR. Any student who does not have any failing, incomplete and/or conditional grade on the academic units enrolled during the semester. b. GOOD. Any student who obtained a final grade below “3.0” in less than 25% of the total number of academic units enrolled during the semester.

c. WARNING. Any student who obtained a final grade below “3.0” in 25% of the total number of units enrolled during the semester. d. PROBATION. Any student who obtained a final grade below “3.0” in 50% of the total number of units enrolled during the semester. The load for the succeeding semester shall be reduced to 25% as prescribed in the curriculum. Probation could be removed with passing grade of 3.0 or better in more than 50% of the total number of units enrolled in the succeeding semester. e. DISMISSAL. Any student who obtained a final grade below “3.0” in 75% but less than 100% of the total number of units enrolled during the semester may be dismissed from the program. Any student on probation who again failed in 50% or more of the total number of units enrolled during the semester shall be dropped from the roll of the college. f. PERMANENT DISQUALIFICATION. Any student who obtained a final grade below “3.0” in 100% of the total number of units enrolled during the semester will no longer be allowed to enrol in the program. g. RE-ADMISSION OF DOUBLE PROBATION, DISMISSAL OR DISQUALIFIED STUDENTS:  They must first qualify themselves for re-admission before they  are allowed to register during the succeeding semester and the load be reduced to 50%.  Re-admission applications are processed at the Office of Student affairs  and Services.  Re-admission of dismissed or disqualified students shall be evaluated by the Dean/Registrar and with recommendations from the Director of Student Affairs and Services and the Campus Administrator, in case of conflict, it may be elevated to the Vice  President for Academic Affairs and the decision shall be final. 2. For strict compliance of the retention policy of the college, a passing grade/maintaining grade is required in the general education, professional education and specialized/major subjects.

3. A student must undertake and pass the Competency Assessment Evaluation/qualifying exam as an entry requirement to his/her specialization. 4. The maximum number of years a student is allowed to pursue an academic program in the college beyond the program length of his/her chosen degree is 1.5 times the prescribed number of years of the program.

In the event that the student fails to finish the course within the prescribed maximum residency period, he/she will no longer be allowed to re-enrol in the same program. Provided that there are valid reasons and humanitarian concerns for the continuance of residency. However, if a student opts to shift to another program, he is only given one chance and should finish the course within the prescribed time frame.

B.3 Academic Calendar and Classes a. The academic calendar shall follow the school calendar approved by the Commission on Higher Education but with certain modifications to suit the needs of the College. b. Each semester shall consist of at least 18 weeks. Class work in the summer sessions shall be equivalent to class work in one semester.

c. All class hours loss due to fortuitous events such as typhoons and other natural calamities shall be made up for. d. Schedule of classes and examinations shall be prepared by the College Deans in accordance with the academic calendar and after thorough consultation with the Campus Administrators, the Deans and College Registrar, and the Vice President for Academic Affairs. B.4. Admission and Registration a. Entrance Requirements for incoming freshman undergraduate students are:    Report Card (Form 138) or High School Transcript of Record    Certification of Good Moral Character    Two identical ID pictures (2” x 2”)    Birth Certificate issued by the PSA; and if necessary  A satisfactory result of a College Admission Test administered by the college b.  Entrance requirements for transferee undergraduates students are:    Transfer credentials which include certified true copy of grades    Certificate of Good Moral Character    Two Identical ID pictures (2” x 2”)   Birth Certificate issued by PSA Note: No student shall be denied admission by reason of nationality, race, age, sexual/gender orientation, religious belief, socio economic status, political affiliation, membership in student organizations, physical handicap, nor shall pregnant students, certified reformed drug abuses, and those suffering from acquired immune deficiency syndrome be discriminated against. Provided, however, that all admission requirements are satisfactorily complied with.

B. 5. Student Transfer a. Subject Validation   The college allows for the accreditation of subjects taken by student transferees from the College where they came from.  The College/Institute from where the student-transferee is presently enrolled in  undertakes the  evaluation of the subjects taken from the previous university/college   The Office of the Registrar is furnished a copy of the evaluation for permanent recording in the student’s academic file. b. Transferees from other Schools/Universities Students from other schools, colleges and universities wishing to transfer to ISPSC may do so provided that they undergo the same screening procedures that an entering new student undergoes.

B.6. Registration Registration of students shall only be done on the regular registration period as indicated in the school calendar. No Student Shall be allowed to register in any subject after the registration period has been closed. Rules and procedures on cross enrolment, adding, changing and dropping of subjects as well as pre-requisites of subjects shall be observed. A student is not allowed to enrol simultaneously in more than one course. Request for overload, cross-enrolment, waiver of pre-requisites and pre-requisites during enrolment shall be done officially, signed and approved by the Registrar and furnished a copy of the Office of the College Dean.

B.7. Late Registration Fee Late registration fee is collected from students who do not come to enrol during their scheduled date of enrolment. However, the following are exempted from paying late registration fee: a. a student who is delayed due to natural calamities or military operation; b. a student is delayed due to hospitalization caused by illness or accident. A medical certificate shall be attached to the application for exemption. c. a student who is delayed to an emergency at home, like the death of an immediate member of the family. A promissory note shall be submitted to the College Dean before he/she is allowed to enrol. Death certificate shall be attached to the application for exemption. Note: The provision under Late Registration fee is only applicable to those who waived on the Free Higher Education Act (i.e. free tuition) and to those who did not qualify for free tuition

B.8 Cross Registration Cross-registration/enrolment to other institution is allowed only if the subject/s needed is/are not offered at a given term in any of the ISPSC Campuses. Cross-registration/enrolment permit will be issued by the Office of the Registrar only upon submission of the approved request issued by the College Dean. Furthermore, subjects enrolled by a student in any college without permit will not be credited. The maximum number of units for which a student may cross register in two or more colleges and/or universities shall not exceed 24 units for non-graduating students and 27 units for graduating students. No student shall be allowed to cross-register/enrol in any other institution for the purpose of simply increasing his academic load or obtaining a higher grade. However, a student may be allowed to cross register in other school if he is graduating within the school year and the subject to be enrolled is not a major subject and is not simultaneous with practice teaching for education students. The Office of the Registrar shall prepare the cross-registration form to be presented by the student to his/her chosen school.

B.9. Simultaneous Enrolment of Pre-requisite and Advance Subject Simultaneous enrolment of pre-requisite and advance subject maybe allowed under the following conditions:    the pre-requisite is a repeated subject    the advance subject was invalidated for failure to enrol the pre-requisite subject  these are the only subjects left to complete the academic requirements Request for simultaneous enrolment shall be prepared by the student, to be recommended by the Dean and to be approved by the Vice President for Academic Affairs.

B.10. Subject Substitute Subject substitute is allowed if the subject involved no longer exist due to the revision of the curriculum provided that the subject substitute is allied to the required subject and has the same number of units. The request must be recommended by the College Dean and the College Registrar and approved by the Vice President for Academic Affairs.

B.11. Classification of Students An undergraduate student enrolled in a regular semester carries the full semester load as prescribed in the curriculum he/she is enrolled in. Student shall be classified as either regular or irregular based on the following curricular requirements:

a. A regular or full-time students carries the full load in a given semester by the curriculum in which he/she is registered. b. An irregular student carries less than the full load called for in a given semester by the curriculum in which he/she is registered. As to curriculum year, students are classified as follows: a. Freshman. A student who is enrolled in the first curriculum year of a given course. b. Sophomore. A student who has finished the first curriculum year or 25% but not more than 50% of the total number of units required in his/her course. c. Junior. A student who has completed the first two years of his/her curriculum or 50% or more but not more that 75% of the total number of units required in his/her course. d. Senior. A student who had completed the first three years of his/her curriculum or 75% or more but not more than 85% of the total number of units required in his course.

B.12. Maximum Residence Rule A student must finish the requirement of a course of any college within a period of actual residence equivalent to 1 ½ times the normal length prescribed for the course; otherwise he/she shall not be allowed to register further in the college.

B.13. Academic Load An undergraduate student enrolled in a regular semester is limited to take the prescribed number of academic units in the curriculum he/she is enrolled in. The student checklist shows the regular number of units that a student has to carry within a semester. On regular semester, the normal academic load shall not exceed the specified number of academic units in the curriculum he/she is enrolled in. However, as a matter of policy, exemplary student shall be allowed to enrol more than the maximum provided a permit is approved by the College Registrar in consultation with the College Dean. On summer term, the normal academic load shall be six (6) units for subjects with laboratory and nine (9) unit for non-laboratory subjects. Students who are employed whether on full-time or part-time shall not be allowed to enrol more than 15 units. However, as a matter of policy, exemplary students shall be allowed to enrol more than 15 units provided a permit is approved by the College Registrar, in consultation with the College Dean. A graduating student is allowed to carry not more than 28 units in a regular semester. For non-laboratory courses, one unit of credit shall be at least eighteen (18) hours of instruction per semester in the form of lectures, demonstrations, seminars, tutorials or their combinations. For laboratory courses, one unit of credit shall be at least thirty (30) hours of instruction per semester in the form of lectures, demonstrations, seminars, tutorials, field/ laboratory work, or their combinations. Provided that three-fifths (3/5) of the time shall be devoted to laboratory and two-fifths (2/5) devoted to classes. Teacher Education Students who are enrolled in Practice Teaching are not allowed to enrol any other subjects, unless given special permission by the College Registrar, upon recommendation of the College Dean.

B.14. Advanced Credits/Accreditation/Validation of Unit Earned A transferee from a recognize college shall be given equivalent credits if the subjects he has earned there apply to any of the college curricula. Provided that he meets the following regulations on accreditation: a. Subject taken in a state universities or colleges and other private institutions are accredited, provided that the course descriptions and number of units are the same. b. If the number of units in a given subject earned by the student is less than the subject being offered by the college, the student has to enrol the subject.

c. In case of slight differences in the description of the subject, the College Registrar, in concurrence with the College Dean will issue a certification to the effect that such subject can be accredited towards the subject being offered by the college.

B.15. Dropping/Adding Changing/ Substitution of Subjects B.15.1. Dropping of Subjects. a. Voluntary dropping of subjects shall be allowed only within the period from the opening of classes to the time the midterm examination is administered. b. Dropping forms are available at the Office of the Registrar and must be fully accomplished and submitted to the office of the Registrar before a student is officially considered as “dropped” from the roll. Officially dropped subjects will be deleted from the Reports of Grades and Official Transcript of Records. Official dropping of subjects are allowed only on justifiable reasons; such as health, unavailability of the subject needed in the curriculum to complete the full load, and employment. c. Changing and Adding of Subjects. Changing/adding of subjects shall be made only with the consent of the concerned instructors or professors and upon approval of the Registrar. Provided, that it shall be done within the second week after the opening of classes. d. Substitution of Subjects. Substitution of subjects may be allowed provided it does not involve subjects under the same department. Provided further, that the student needs the required subject not offered in the major department but said subject has already been taken in another department. Substitution of subjects shall be approved by the Registrar, in consultation with the College Dean.

B. 16. Petition of Unscheduled Subjects Subjects not regularly scheduled for a given term can be offered upon written request of at least ten (10) non-graduating students or five (5) graduating students, duly endorsed by the Registrar and College Dean and approved by the Vice President for Academic Affairs.

B.17. Withdrawal from the College a. A student may withdraw from the College during the enrolment period and before the start of classes through a formal request in writing is entitled for a one hundred percent (100%) refund of his/her payments made except the fees intended for entrance, registration and admission test. b. After the start of classes, only tuition fees are subject for refund in accordance with the following schedule:    Within one week after the opening of classes – 90%    Within two weeks after the opening of classes – 80%    Within the third week after the opening of classes – 30%   Within the fourth week after the opening of classes – NO REFUND. In claiming for the refund, the student shall present and surrender his/her receipt/s. Note: The provision under Section b of B.15 is only applicable to those who waived on the Free Higher Education Act (i.e. free tuition) and to those who did not qualify for free tuition c. Before approval of the student’s withdrawal from the College, he/she is required to report to the Office of Guidance and Counseling and submit himself to the exit interview.

B.18. Suspension of Classes and Change of Schedule a. Classes in a college or unit shall be dismissed or suspended by the Campus Administrator, provided, it shall be in accordance with the schedule as prescribed in a memorandum relative to it. However, in cases of fortuitous events for which a report shall be submitted to the College President

through the Vice President for Academic Affairs, the former, or in his absence the latter shall have the sole authority to dismiss or suspend classes at any time in a given campus. b. Class schedule approved by the College Dean shall not be changed nor moved in another room, time, and date without prior approval of the concerned professor or instructor and the Campus Administrator.

B.19. Class Attendance The standards of attendance shall be strictly adhered to by instructors and professors to prevent the awarding of academic credits to students who do not meet the minimum requirements. Student absences shall not exceed twenty (20) percent of the required number of school days for classroom and laboratory instruction. Otherwise, the student shall be dropped from the subject except for reasons of illness which is duly certified by a physician or of other justified reasons wherein absences shall be made up depending on the instructor or professor. Provided that dropping the student from the subject shall be done a day before the midterm examination, otherwise, the student shall have a failing grade. Absences incurred outside of the allowed number of hours will automatically mean a grade of 5.00 or Failed for the student in the concerned subjects Student absences must be reported by the professor concerned to the Guidance Office. The Guidance Officer will then inform the parents, in writing, the number of absences the student has incurred. Any student who absents himself/herself from class must obtain a re-admission slip from the Guidance Office which must be presented to the subject professor before entering his/her classes.

B.20 Absences and Make-up Work/ Special Quizzes/Examination Absences during classes for whatever reason are taken against the student, whether or not they are excused. However, a student may make up for any work missed during an excused absence. He/She is given a special quiz/class work and must be taken within one week after the student has returned to class. If a student fails to take the special quiz during the prescribed period, he/she shall not be given a score or a failing mark in the missed quiz or class work.

Students who are physically absent from their classes due to participation in student activities such as athletic meets, conferences, trainings and workshops, among others where they represent the College shall not be considered absent. Missed activities/topics covered by the class during the students’ attendance to such activities shall not be included in their quizzes or examinations, otherwise, their previous grades in the last grading period or the midterm examination should prevail. Time lost by late enrolment shall be considered as time lost by absence. Students are marked “late” if they arrive within the first fifteen minutes of the class period. If the student enters the class after fifteen minutes, he/she will be marked absent even if he/she is allowed to join the class for the remainder of the period. Students who leave the class after the roll call and not return or only return towards the end of the period is considered absent. Although members of the faculty are expected to begin their classes promptly, various contingencies which are sometimes unavoidable may cause some of them to be late for class. In such case, students should not leave the class until after fifteen minutes. Call Slip. No one is allowed to disrupt the class by calling on a student during class hours without an official call slip issued by the Guidance Counselor.

B.21. Leave of Absence a. A student who wishes to defer for a particular semester or school year may do so provided that he/she applies for a leave of absence from the College. b. The request will have to be formally submitted to the Office of the Registrar and endorsed by the College Dean.

c. A student who leaves the College for reason of suspension, dropping or expulsion due to disciplinary action shall not be entitled to honorable dismissal. Should he/she be permitted to receive his transcript of record or the certification of his academic status in the college, it shall contain a statement of the disciplinary action rendered against him/her. d. Before approval of the leave of absence, the student is also required to report to the Office of the Guidance and Counseling to undergo the necessary interview regarding the application for a leave of absence. e. A student who goes on leave on a particular semester is advised to return to the College during the semester that he/she goes on leave in order to maintain his/her status as a regular student. f. A student is allowed to go on leave only for one (1) year with an extension of one (1) year. Approval for requests of extension of the leave of absence is made on a case to case basis. g. A student who has not returned after five (5) years of absence from the College will be accepted for enrolment provided that he/she will enrol as freshman. h. Students who withdraw from the College without any formal leave of absence may have their registration privileges curtailed or entirely withdrawn and be disqualified from graduation with honors.

B.22. Scholastic Delinquency The faculty of the college shall formulate and approve suitable and effective provisions governing undergraduate delinquent students, within the context of general college policy. Students who shall incur nine academic failures in nine (9) subjects shall be permanently barred from re-admission to the course, provided, that these shall not include failures incurred in their last curriculum year; provided further, that the failures were not due to unofficial dropping of subjects due to illness or other valid reasons. Any student wishing to officially withdraw from the College and transfer to another school, college or university may do so provided that he/she has already cleared himself/herself of all liabilities and responsibilities (administrative, academic and financial) in the College.

B.23.School Fees and other Charges School fees and other charges shall be based on the approved schedule by the Board of Trustees upon the recommendation of the College President and in accordance with CHED Order No. 13, s. 1998 and CHED Regional Memorandum No. 59, s. 1999. Information on tuition and other fees are being issued by the Accounting Office from time to time and are posted in strategic places within the campus.

B.24. Examination and Grades Examinations are inherent and essential parts of instruction and shall be administered by the instructors or proctors in conformity with existing institutional policies or rules for the purpose of formative and or summative evaluation of student’s performance. The schedule of examination shall be prepared by the College Dean to be posted one (1) week in advance. Students are required to secure their examination permits and present the same to the proctor before they are allowed to take the examination. In accordance with the prescribed grading system, the academic performance of the students shall be evaluated and graded by the faculty concerned at the end of each term. The faculty concerned has the sole authority to determine and give grades to his/her students. All faculty members shall submit their report of grades to the College Dean seven (7) calendar days after each term. In justifiable cases, deviation from the rule may be authorized by the College Dean. No faculty member shall be allowed to change any grades after he/she has submitted the grade sheet to the Office of the College Registrar. In case of error, the concerned faculty member shall make a request to the proper authority through channels, informing the Registrar to rectify the erroneous grade

of any student. If request is granted, a copy of the approved request authorizing the faculty to change the grade shall be forwarded to the Office of the College Registrar for the correction of the record. No grades shall be changed after one year from the initial filing and the change to be done should not be to the prejudice of the student. In the undergraduate courses, the minimum grade for major and minor subjects shall be 3.0. A grade of 4.00 means conditional failure. It may be made up or removed by passing the removal examination scheduled within one (1) week after the final examination. If the student passes the removal examination, a grade of 3.00 is given; if he/she fails, the grade is 5.00 and he/she has to repeat the subject. A grade mark of “Inc” is given when the student passed his/her class standing throughout the semester but failed to take the final examination due to illness or other valid reasons, or when the student passed his/her class standing throughout the semester but failed to submit on time any of the class requirements. Provided, however, that completion of the deficiency shall be done within one (1) year by passing the final examination or by meeting all the requirements of the course. Otherwise the “Inc” grade shall automatically become “5.0”. At the start of the semester or summer term, the students have the right to be informed of the grading system and how their grades will be computed. In case the faculty member fails to submit the grading sheets on time, the salary for the last week of the term will be withheld until a clearance from the Office of the Registrar is secured. The grades of the students who attend or participate in school-related activities such as sports competition, conference, seminars, workshops and others shall be maintained or may even be increased depending on their performance in the said activity, but in no case shall be lowered nor decreased. Provided, students concerned should report to his/her classes after the said activity/ies.

B.25. Grading System At the start of the semester or summer term, the students have the right to be informed of the grading system and how their grades will be computed. The grades of students are computed in accordance with the prescribed norms and standards established by the College and with regulations prescribed by the Commission on Higher Education (CHED) and the Department of Education (DepEd). There shall be two (2) grading terms or periods: midterms and finals. Computation of midterm and final grades: a. Equivalent grades for examinations and quizzes: 50 = +50

B.26. Graduation Requirements No student shall be recommended for graduation unless he/she has satisfactorily completed all the requirements prescribed for graduation. No student shall be allowed to graduate from the College unless he/she has completed at least one year of residence immediately prior to graduation. All prospective candidates for graduation shall file an application for graduation to the Registrar upon enrollment. The application shall serve as a basis for the Registrar to examine the academic records of each candidate and determine whether he/she is eligible for graduation. The Academic Council has the sole authority to determine whether a student shall graduate or not. The Academic Council, upon recommendation of the College Registrar, shall also determine the meritorious student awardees during the commencement exercises wherein only graduating students are qualified to receive such awards. However, in cases where there is no eligible graduating students, the college is allowed to determine qualified awardee from the non-graduating students. All candidates for graduation shall have cleared their deficiencies and their records completed not later than six (6) weeks before the school year ends except in those subjects both academic and non-academic in which the student is enrolled during the current semester.

B.27. Thesis/Special Projects/Special Problems Students enrolled in the College are encouraged to produce thesis/special projects/special problems that are scholarly written and are of high quality standards. Students enrolled in the 2-Year technical/ladderized courses are required to submit a special project for the completion of the course. Students enrolled in the degree courses have to present a thesis and must be defended and passed before a panel. If for any reason the special project or thesis cannot be completed within the semester, the student should re-enroll for the coming semester. He/she shall be given a grade mark of “IP” while the work is in progress. A grade mark of “IP” is permitted only once, otherwise it will affect the academic standing of the student which disqualifies him/her for honor awards. A numerical grade will only be given when the student is ready to submit the required number of copies of the approved manuscript.

B.28. Dean’s List The College recognizes the performance and excellence of students in the academics by coming up with a semestral list of top achievers. Dean’s Lister are given tuition fee discounts (if not covered with the Free Higher Education Act) for the coming semester and/or awards of recognition during the recognition program. The Dean’s List is posted within strategic places in the College and students included in the list should report to the Office of the Scholarship Coordinator to determine how much the tuition fee discount is: GPA Tuition Discount 1.00 - 1.49 100% 1.50 - 1.59 50%

1.60 - 1.75 None Qualification Standards of the Dean’s Lister a. He/She has obtained a GPA of 1.75 and above b. He/She has no grade of 2.5 in any of his/her subjects during the semester c. He/She has no marks of “Dropped”, “INC” or “Failed” in any of his/her subjects including NSTP d. He/She is enrolled as a regular student who carries the prescribed regular load.

B.29. Graduates with Honors Awards for academic excellence shall be conferred to students who exemplary completed their respective degrees with flying colors. For this purpose, the following range of weighted average shall be used: Summa cum Laude: 1.0 – 1.25 and with no grades lower than 2.0 Magna cum Laude: 1.26 – 1.5 and with no grades lower than 2.25 Cum Laude: 1.51 – 1.75 and with no grades lower than 2.5

All awardees for academic excellence shall have completed in the College at least seventy five(75%) of the total number of academic units required for graduation and shall have been in residence in the College for at least two(2) years immediately prior to graduation. Awardees for academic excellence who are disqualified due to the lowest grade obtained shall be awarded where he/she qualifies. Provided, that the awardee has not incurred an incomplete, dropped or failing grade; provided further, that those who qualify as to the weighted average but disqualified by the preceding provision, shall be honored with a “With Distinction” award. All awardees for academic excellence shall have taken during each semester a not less than fifteen (15) units of credits unless due to justifiable reasons or except when the under load occurred during the last semester of schooling and the units enrolled are the only units left to complete the course. Justifiable reasons includes health, unavailability of subjects needed in the curriculum to complete the full load, and employment. For transferees, they will qualify for Latin honors (i.e. Cum laude, Magna cum laude and Summa cum laude) provided, they comply with the preceding provisions and that resident units and all credited units from the previous school shall be included in the computation of the weighted average; with no incomplete, dropped or failed subjects from the College and from the school where she/ he previously attended.

NSTP units shall not be included in the computation of general weighted average but to qualify as awardee for academic excellence, the grades should not be lower than 2.50.

B.30. Other College Awards (Student Awards) The College holds the Annual Recognition Program for students before the end of the school year. A committee for this purpose is created by the President to set guidelines, screen and deliberate on the possible awardees. Members of the committee include the College Dean, Head for Admission/College Registrar, and Director for Student Affairs and the Board of Advisers of the different school organizations. The following awards in the form of certificates, medals or ribbons shall be presented to deserving students during the recognition program: a. With Distinction for those who are disqualified for academic excellence honors but have excellent academic performance and those whose general weighted average is 1.76 to 2.00 with no incomplete nor failing grades.

b. Leadership Award for those who have outstanding achievement in co-curricular activities for at least two (2) consecutive school years. Candidate must be a graduating student who has served as an officer of any student organization. The candidate will be subjected to interview and peer and subordinate evaluations. Proofs of leadership are required for deliberation purposes. c. Loyalty Award for those who have studied in the College from first year high school up to their graduation in the tertiary level. Provided, that the awardee shall not have entered nor enrolled in any other school except in any of the ISPSC campuses; d. Meritorious Award for those whose exemplary performance in various fields give credit or distinction to the organization where they belong or for the benefit of the College. Provided, that said exemplary performance were done or exhibited during the time they are pursuing their respective courses. e. Special Awards – given to students, whether graduating or not, who won in local, regional, national or international competitions in appreciation of their efforts in bringing the College at par with the other schools, colleges and universities in the country.

f. Non-graduating students who have obtained the highest grade point average and without incomplete, dropped or failing grades in their respective Colleges will also be receiving scholastic awards in recognition of their academic excellence.

B.31. Commencement and Baccalaureate Exercises The Head for Admission/College Registrar and the Director for Student Affairs and Services and Head for Alumni Affairs shall be the chair and co-chairs, respectively, of the committee in charge in the conduct of the commencement and baccalaureate exercises with the College Deans, Registrars/Acting Registrars and advisers of graduating classes as members. Candidates for graduation with degrees or titles which require no less than four (4) years of college instruction shall be required to wear academic gowns during the baccalaureate service and commencement exercises

E.32 Application for Clearance Graduating students should submit a duly accomplished clearance form before the release of their souvenir program. Students who wish to transfer to another school must also submit such clearance form before he is allowed to apply for any of the following: transcript of records, certificate of good moral character, transfer credentials and others. Clearance forms are available at the Office of the Registrar. No student will be issued his credentials unless he has been cleared of all accountabilities.

C. THE STUDENT SERVICES SECTOR

C.1. Vision, Mission and Goals C.1.1. Vision The Student Services Sector envisions to providing with: a. Creative, critical, highly functional and globally oriented students to graduate from the College b. A supportive yet challenging environment that encourages the students’ development. c. Balanced co-curricular and extra-curricular activities of the different student groups that provide opportunities d. An atmosphere of healthy social interaction between and among students, faculty and administrative staff. e. An environment complimentary to the academic training of students with different growth activities focusing on improving leadership, self-management and development skills;

f. the development and enhancement of career goals and plans leading toward the achievement of a rewarding career, lifestyle and job; and g. The creation of a working environment with technologically advanced equipment, an updated information system and complete testing facilities. C.1.2 Mission The Student Service Sector serves the students in the maintenance of a healthy social relationship through varied services to facilitate the optimum development of their potentials

C.1.3. Goals Development Goals The overall development goal of student services is to develop disciplined, responsible and highly trained students. Enabling Goals In pursuit of the development goals, the Student Services Sector aims to: a. Enhance staff commitment and professional capability b. Provide and approach/system of efficient delivery of service. c. Create an atmosphere of democratic social interaction; and d. Maintain a strong feedback mechanism foe evaluation.

C.1.4 Objectives In conformity with CMO No. 09, s. 2013, the College specifically the office of Student Affairs and Services sets the following objectives: 1. Ensure proper balance between rights of the institution and student rights. 2. Improve the quality of Student Affairs and Services among Higher Education Institutions, 3. Promote access to quality, relevant, efficient, and effective student services to the students. 4. Support student development and welfare; and 5. Ensure that the College provide holistic approach for Student affairs and services and comply with the minimum requirements of the Commission on Higher Education.

C.2. Student Affairs and Services Student Affairs and Services are the services and programs of the College that are concerned with academic support experiences of students to attain student holistic development. Academic support services are those that relate to student welfare, student development and those that relate to institutional programs and services (CMO No. 09, s. 2013)

C.3 Organizational Structure The organizational structure shows the chain of command in the student services sector. It aims to provide the students a clearer understanding of the flow of leadership, responsibility and communication concerning student services in the college. The structure was patterned according to the provisions of CHED Memorandum Order No. 09, 2013. (See Annex A)

C.4 Officials a. The Director for Student Affairs and Services is one who plans, organizes and execute, monitors a sound student services programs of the college. He/She is considered the most understanding faculty member within the school system from whom the students, faculty and staff, personnel, often seek advice. He/She is under the immediate supervision of the Vice President for Academic Affairs.

b. The Director for Student Affairs and Services has a direct supervision among the three major areas of student service which are the Student Welfare Services, Student Development Services and the Institutional Student Program Services.

c. The Director of Student Affairs and Services works in close coordination with the Campus Administrator, deans and program heads, and other college heads like, dormitory services, commercial center and rentals, security services and CDRRMC and the head for general support services. d. The units under the office of the Student Affairs and Services are: Admission; Library Services; Sports Development/PE; Culture, Arts and Heritage; NSTP; TVET and Scholarship; Health Services; Guidance Services; Guidance Services; Placement Services; and Alumni Affairs. e. Each campus of the College has Campus Coordinator for Student Affairs and Services

D. OFFICE OF STUDENT AFFAIRS AND SERVICES

D.1. STUDENT WELFARE SERVICE The Student Welfare Service office of the college offers basic services that are necessary to ensure and promote the well-being of students.

D.1.1. Guidance and Counselling Service Unit D.1.1.1. Mission, Goals and Objectives D.1.1.1.1 Mission The ISPSC guidance and Counseling Office support the Philosophy of the college to develop man in his totality and to become productive members of the society. D.1.1.1.2 Goal The Guidance Program of ISPSC aims to provide development, preventive and remedial programs to all the clients through responsive and outcomes based guidance and counseling services. D.1.1.1.3 Objectives a. To organized data and information which are necessary for the understanding of the student’s growth and development and functional use; b. To provide information to student important to their growth and development which are not commonly provided in formal instruction; c. To provide necessary counseling assistance to students in which a discussion and resolution of problems will be pursued in a non –authoritative manner; d. To provide appropriate assistance to students and graduates in their adjustment to various levels of educational personal programs and their job search, respectively; e. To conduct assessment of the Guidance Program in order to cater to students’ needs and problems.

D.1.1.2. Personnel a. The Head for Guidance and Counselling Service heads the office who shall plan, supervise, monitor and evaluate all matters relative to guidance and counselling programs and activities of the College. b. Each campus of the College is assigned with a Guidance Counselor.

D.1.1.3. Services Pursuant to the provisions of the CMO No. 09, s. 2013, the College and the office of the Guidance and Counselling Service adheres in providing the following services: a. Individual Inventory Service – serves as the student’s profile which includes records of psychological test and others, counselling visits and other pertinent information about the student. b. Information Service- provides an updated array of academic- educational, vocational-occupational and personal-social materials which can be useful information for better adjustment and personal growth of the student. c. Appraisal – provides information about a student through the use of psychological tests and non-psychometric devices. d. Counselling Service –provides an individual or group intervention to facilitate positive change in behavior, attitudes and feelings of a student. e. Referral Service- it is a helping process which provides a mechanism for coordination with multi-disciplinary team of specialists to ensure that special needs of students which the college cannot provide are met. f. Follow-up – provides a systematic monitoring to determine the effectiveness of guidance activities in general, and placement in particular.

D.1.2. Career and Job Placement Service Unit The Career and Job Placement Service leading task is to address the student internship, pre-employment and employment needs of the graduating students and alumnae. One of its major functions is the networking with industry practitioners for the needed feedback that will assist College administration in curricular improvement and students’ skills development. It assists the professional and personal aspirations of students by providing information and activities that augment existing ones in order to enhance personal and career competencies that are needed as they enter and advance in their chosen fields of works. The Career & Job Placement Office, in coordination with the Student and Alumni Affairs Office, provides relevant services to all graduating students & graduates, including placement to industry partners and providing assistance for their career advancement and development.

D.1.2.1 Vision, Mission, Goals and Objectives D.1.2.1.1. Vision Engaging students in the process of discovering, emerging and hunting a high-performance career pathway, leading to positive impacts on the community and the global workforce.

D.1.2.1.2. Mission Dedicated to assisting students with making connections between their academic experience and career paths and providing career-related counseling, internship, trainings, seminars, resources to help students elucidate academic and career goals, launch career plans, develop job-search skills, and make successful career transitions that helps in ensuring competencies in their future pursuits.

D.1.2.1.4. Objectives a. Provide related careers information and services to undergraduate and postgraduate students in the most appropriate and effective way. b. Entice and qualify organization to promote opportunities successfully and make contact with the most suitable students. c. Work with alumni, providing access to careers advice and engaging them to mentor students and offer internships. d. Implement leading systems and streamlined procedures within a responsive, skilled and motivated organization to achieve these tasks, including monitoring our performance.

D.1.2.1.5. Personnel a. The Head for Placement Services reports directly to the Office of the Director for Student Affairs and Services and coordinates with the different College Deans and Program Heads. b. Each campus has designated Coordinator for Placement Services.

D.1.2.1.6. Services The office provides the following services that help students and alumni boost their knowledge, skills and attitudes that helps them in ensuring their competencies in their future pursuits.

D.1.2.1.6.1 Career Education Supports the students in becoming successful, educated citizens by providing them with opportunities to explore a variety of careers and options for their future. This service helps students to discover a bridge between classroom learning and workplace and post –secondary realities, and is intended to make their learning meaningful and relevant.

D.1.2.1.6.2 Job Placement The concerned officer of the college typically meet students prior to internship deployment and graduation to discuss both the deployment and employment strategies. The officer will help students and alumni develop an appropriate job-seeking approach, depending on their education, skill levels and personal circumstances. This usually includes writing a resume, practicing interview techniques and going out for internship deployment and on job leads the placement officer has already vetted. Program Internship coordinators may also help students and graduates secure internships, work-study opportunities and part-time employment.

D.2. STUDENT DEVELOPMENT SERVICE These are programs and activities designed for the enhancement and deepening of leadership skills and social responsibility, which includes student organizations and activities, professional organizations and societies, special interests, leadership training programs, student council government, student discipline, student publication and media. (CMO No. 09, s. 2013)

D.2.1. Student Organization and Activities This office looks into the recognition/accreditation, supervision and monitoring of student groups and organization, including the evaluation of their activities. D.2.1.1. Goals and Objectives. a. To enhance the growth of students as individuals and members of the academic community by encouraging the formation of different clubs and organization within the College; b. To complement the academic and curricular growth and development of the students through various student projects and activities such as sports fest, quiz bees and the like.

D.2.1.2. Personnel The office is headed by the Coordinator of Student Affairs and Services who handles all the different mandated and accredited student clubs and organizations and the Supreme Student Council.

D.2.1.3. Services a. Accredits student clubs and organizations. b. Monitors the projects and activities of the different accredited student clubs and organizations, the different college councils and the Supreme Student Council c. Implements the Rules and Regulations Governing Organized Student Activities (RRGOSA) d. Facilitates meetings and other joint activities of student groups. e. Endorses student request for funding of projects, training and seminars and the use of facilities of the college. f. Approves the posting of promotional materials and advertisements within the Campus. g. Recommends college-wide activities geared towards the wholesome development of students. h. Provides administrative and technical support to the official College Publication

D.2.1.4. Student Clubs and Organizations Different clubs and organizations are organized and recognize to enhance the growth of students as individuals and members of the academic community; and also, to complement the academic and curricular growth and development of the students through various student activities such as sports quiz, quiz bee and the like. The following RULES AND REGULATIONS shall apply to all student organizations, whether accredited or not, and all their school-related activities by bona fide students of the Ilocos Sur Polytechnic State College: a. Cultural – the organization promotes performing and visual arts such as painting, theater, music, dance and other talents;

b. Socio-civic – the organization engages most of the time in outreach programs such as medical and dental missions, visit to social welfare institutions and other related socio-civic activities; c. Academic – the organization concentrates in the fields of instruction and research or enhances the course specialization of its members; d. Advocacy group – the organization promotes awareness on specific social issues and concerns; e. Religious – the organization centers on religious or faith-related activities as well as spiritual and moral concerns; f. Sports – the organization emphasizes physical education and development; g. Political – the organization operates with a definite political orientation and perspective; h. Fraternity, Sorority and Confraternity – the organization is a nonpolitical college-wide student group whose members belong to different colleges and whose primary aim is brotherhood and sisterhood with specific objectives and goals.

D.2.1.4.1. Requirements for Accreditation of Student Clubs/ Organizations It is the procedural aspect in operationalizing any organized student groups to establish their identity by applying for the grant of authority to operate as an organized group. The following are the guidelines governing accreditation of student organizations:

a. Before the end of the current school year, the OSA will announce the start of the accreditation procedure for the coming school year. A Certificate of Accreditation, which is good for one (1) school year, awarded to the student organization will serve as its passport in the recruitment of its members;

b. Any group of fifty (50) bona fide students may apply for pre-accreditation; provided that the following requirements are submitted to the OSA in duplicate two (2) weeks before the formal closing of classes in a school year

 Letter request for Accreditation duly signed by the elected President  of the Organization and the faculty adviser address to the Director of Student Affairs and Services.    Constitution and By-laws of the Organization    Action plan or Proposed Calendar of activities for the School Year.  Accomplishment and Financial reports for the previous school year  (applicable only for clubs or organizations applying for re- accreditation)   Accomplished OSAAS forms

D.2.1.4.2. Guidelines on the Operation of Students Clubs and Organizations a. A Certificate of Accreditation for the School year is issued to a student club/organization upon approval of its request for accreditation. The certificate should be signed by the Director of Student Affairs and Services, the VP for Academic Affairs and the College President. Every accredited student organization shall have a faculty adviser as designated by the Campus Administrator. Before such adviser is to be appointed and has assumed office, the student may not hold any meeting or

undertake any activity for any purpose, except to draft its constitution and by- laws.

D.2.1.4.3. The Board of Adviser for Student Organizations a. The Board of Adviser of Student Organization (BASO) handles the accreditation process of student clubs and organizations. This is composed of representative from the students, faculty, administration, the Coordinator of Student Affairs and Services and is headed by the Director of Student Affairs and Services.

D.2.1.4.4. The Student Government a. The student government of the College shall be officially called the Supreme Student Council (SSC). The SSC serve as the link between the College administration and the student population. b. The Supreme Student Council shall be governed by its Constitution and By-Laws. c. Election of the officers of the SSC is, on the other hand, governed by the Student Election Code which is implemented and carried out by an independent student body, the Commission on Student Elections. d. The Commission on Student Elections is tasked to oversee the orderly conduct of student elections and to religiously carry out the rules, regulations and guidelines specified in the Election Code.

D.2.1.4.5. Federation of Student Councils All elected Supreme Student Council presidents of the different campuses of the College shall confederate and form the federation of the Student Council. The elected president of the federation shall sit as the Student Trustee in the Board of Trustees of the College. This federation shall exist to serve the purpose of unifying all efforts of the college system student councils in addressing the needs of the student population of the College through a representation in the College Board of Trustees

D.2.1.4.6. Organized Student Activities The establishment and operation of any organized student activity in the College shall provide a forum for academic and personal interaction among students, faculty and administrative staff, as well as provide a medium for more productive endeavors towards the attainment of the goals of the College and that of society; provided, however, that the activities shall be along the psycho-physical and socio-cultural development of the studentry; and provided further, that such activities shall be approved by the College President or his/her duly authorized representative.

D.2.1.4.7. Fraternities, Sororities and Confraternities

Due to issues involving fraternities, sororities and confraternities, the following guidelines are specifically imposed:

a. Each fraternity, sorority or confraternity shall have at least two (2) advisers who must be regular employees of the College, either teaching or nonteaching staff; b. The said advisers shall signify their intention to serve the organization by way of submitting an Oath of Commitment to the Office of Student Affairs;

c. All fraternities, sororities and confraternities shall be members of a confederation. It is through this confederation assembly that accreditation is possibly granted. Only those that are endorsed by the confederation assembly merit official recognition by the College administration. Otherwise, they are illegitimate interest groups which have no juridical personality and hence barred from doing business in the campus.

d. The confederation assembly, being a Central Committee composed of representatives from member fraternities, sororities and confraternities, operates with advisers who are officially designated by the College President through a Special Order;

e. The confederation assembly may promulgate other conditions and internal rules governing fraternities, sororities and confraternities as deemed necessary.

D.2.1.4.8. Recruitment Week

The Office of Student Affairs and Services will, at the start of the school year, announce the first official week of classes as the recruitment week for organizations that have successfully met the requirements set forth in the pre-accreditation process.

D.2.1.4.9. Constitution and By-Laws

Student organizations shall have a constitution and by-laws ratified by the members and approved by the College President upon the recommendation of the Director for Student Affairs and Services. No such organization shall be allowed to function in the College without prior approval as required.

D.2.1.4.10. Faculty Adviser

The student organization has the privilege of choosing its own faculty advisers who are employed in the College on a full-time basis. The said advisers shall be under the supervision of the OSAS Director.

D.2.1.4.11 Pledge of Commitment

The faculty advisers and the duly elected president of the student organization together with the OSA Director will sign a Pledge of Commitment with the College President. This is to solicit the cooperation of the student organizations to carry out the mission of the College. This is required from the student organization before its Certificate of Accreditation is awarded.

D.2.1.4.12. Recommendation for Probationary Status

The OSAS may recommend the probationary status of operation of a certain student organization who has applied for accreditation based on any of the following grounds:

D.2.1.4.13. Disciplinary Actions

All disciplinary actions relative to this Rules and Regulations shall be in accordance with the provisions of this Manual.

D.2.2. Student Discipline

D.2.2.2. Personnel This is spearheaded by the Coordinator of Student Affairs and Services in each campus with direct link to the office of the Director of Student Affairs and Services. He/she judiciously implements the institutional rules and regulations governing student behavior and conduct.

D.2.2.3. General Provisions Students shall, at all times, observe the laws of the land and the policies, rules and regulations of the College as embodied in the student code. No disciplinary action shall be imposed without the due process of law. Complaints by various parties against students for debts shall not be taken cognizance by the College. However, if the case involves the moral character of the student, the Director of Student Affairs and Services or the Guidance Counselor recommends appropriate disciplinary action to higher authorities. In case of unpaid accounts of the students to the college, the imposition upon the students concerned of existing rules and regulations as withholding of grades or barring from examination shall be without prejudice for such other actions as may be warranted under the circumstances. A student shall be subjected to disciplinary action for committing any of the acts categorized as follows: a. College Offenses Any student who has observed a violation of the Student Code of Conduct and Discipline classified as college offenses can report such violation to the Security Officer/Security Guard or directly to the Office of Student Affairs and Services, who coordinates with the College Dean in discussing the case of the student with the College Discipline Board

b. Academic offenses Academic offenses are to be handled by the faculty concerned, the College Dean, Program Heads, and the Coordinator of Student Affairs and Services. Based on the investigation conducted, the student shall be given the corresponding penalty for the violation committed.

c. Personal Offenses Varied approaches depending on the situation and circumstances are to be used in the discussion of the offenses committed by the student against his/her co-student.

On all occasions, the College officials shall exercise fairness in applying disciplinary measures to the erring students. This means that the student has previous knowledge of the standards of student behavior in the academic community, that he be informed of the nature of the charges against him/her, that he be given a fair opportunity to disprove them and that there is a provision for appeal of a decision. When the misconduct of a student necessitate serious penalties and when the student questions the fairness of disciplinary action taken against him/her, he/she should be granted, upon request , the right to be heard before a Hearing Committee duly constituted by the SUC President.

c.1. The Hearing Committee should include the Campus Administrator, College Dean/Program Dean, Director for Student Affairs and Services, Campus Coordinator of Student affairs and Services, the SSC President and the student and an adviser of his/her choice. No other person/s who may be interested in the particular case may sit in the judgment during the proceeding.

c.2 The student shall be informed in writing of the proposed disciplinary action taken against him/her, with specificity and sufficient time to give the student a chance to prepare for the hearing.

c.3. The Student The student shall have the right to be assisted in his/her defense, by an adviser of his/her choice.

c.4. The burden of proof shall rest upon the College officials bringing the charge. For purposes of clarifying who is responsible for determining disciplinary action to be meted on the erring student, the following will be observed:

c.4.1 . The Faculty The faculty member has the sole responsibility to discipline any student judged guilty of the offenses classified as academic (behavior whose locus of occurrence is inside the classroom or school corridors). Should the offensive behavior merit a penalty, a recommendation for disciplinary action is made by the faculty concerned in coordination with the Coordinator/Director of Student Affairs and Services and the College Dean.

c.4.2. The Coordinator/Director of Student Affairs and Services The Coordinator/Director of Student Affairs and Services conducts his/her investigation of the case and recommends re-trial, as the case may be, to the College Dean.

c.4.3. The College Dean The College Dean is the next higher body who decides on the gravity or seriousness of the offense as reported by the faculty and as determined through due process of investigating the case by the Coordinator of Student Service. Offenses within the jurisdiction of the College Dean are those which fall under the category of Academic Offenses. In cases wherein the student has committed a college

offense, the College Dean, together with the Coordinator for Student Service, determines the gravity or seriousness of the offense.

c.4.4. Campus Coordinator of Student Service The Campus Coordinator of Student Affairs and Services has the sole responsibility to determine the violation committed by any organized group of students in connection with activities conducted outside the classroom but within the campus and outside of it. Likewise, college offenses are also handled by the Campus Coordinator of Student Affairs and Services in coordination with the Campus Administrator and the College Dean where the student belong.

c.4.5. The College Discipline Board This is composed of the President (as Honorary Chairman), Vice President of Academic affairs, the Deans, the Head of the Legal Office, and the Director of Student Affairs and Services (as Secretary General) who shall constitute the highest governing body for the discipline of behavior problems of students and is therefore empowered to pass the ultimate decision on the case. The student however has the freedom to appeal the sanction to the Board of Trustees of the College. The College Discipline Board shall have the responsibility of passing ultimate decisions of the case presented by either the Dean (for academic offenses) and or the Director for Student Affairs and Services (for College offenses). The College Discipline and Grievance Committee, composed of the Campus Administrator, College/Program Dean, Program Head, the Coordinator for Student Discipline and the concerned faculty advisers, conduct preliminary investigation of the case involved and present its findings to the College Discipline Board for appropriate action. It shall include in its report the initial findings of the Civil Security Officer and the committee’s endorsement of disciplinary measures on the case. The Civil Security Office shall be composed of a duly designated Head of Security Service and all Security Personnel of the College. The Civil Security Office shall have the primary responsibility to apprehend any person inside the college premises caught violating the Student Code of Conduct and Discipline of the College. The CSO shall cause the preliminary investigation of the case as reported and submits its finding to College/Program Deans ( for Academic offenses) or the Director, Student Affairs and Services (for College offenses). The report of the CSO shall form part of the documents necessary in the litigation of the case.

D.2.2.4 Major and Minor Offenses D.2.2.4.1. Minor Academic Offenses a. Loitering during Class hours, making unnecessary noise such as singing or boisterous conversation causing annoyance. b. Coming to school not in proper uniform during uniform days or wearing an attire not befitting a college student.

C.2.3.4. Services Provided 1. School Organ The Ricochet is the official system-wide student organ of the institution. Each campus has its own campus-wide student publication. To wit: The Farmers’ Bugle (Sta. Maria Campus), The Tower (Narvacan Campus), The Hands (Santiago Campus), The Sand Castle (Candon Campus), The Ilocos Collegian (Tagudin Campus) and The Advancer (Cervantes Campus). Republic Act 7079 also known as the Campus Journalism Act serves as the basic guidelines of the student organ. The Adviser of the Ricochet is designated by the President while the Advisers at the campus level are designated by the Campus Administrator respectively. The compositions of the editorial board of the campus-wide publications are selected through a competitive examination. An intensive information drive using face to face communication (classroom campaigns), traditional media (leaflets, posters, radio announcements, bulletin board announcements), and emerging platforms (social media, school’s website) to disseminate the scheduled screening dates are conducted. A three-day examination schedule usually held in the third week of the month of March is held. The designated Adviser together with two English teachers serve as judges for the selection. On the other hand, the selected Editors –in- Chief from the respective campuses convene in the Main Campus usually in the month of August to elect the members of the Ricochet Editorial Board. Anybody from them can run as EIC of Ricochet. The EIC who garners the highest vote will seat as the EIC of the Ricochet.

2. Yearbook The official system-wide yearbook of the institution is Diaspora. To secure a copy of the yearbook is a choice, meaning it is not a requirement for graduation. However, every graduate is encouraged to have a copy of the yearbook.

The Yearbook publication is directly under the supervision of the Director for Information Services through the Office of the Head for Public Relations, Publication and Information. An editorial board elected from the graduating class assists in the production of the said publication.

D.3. INSTITUTIONAL STUDENT PROGRAMS AND SERVICES

The Institutional Student Programs and Services provide programs and activities which caters student’s essential services that include Admission, Scholarship and Financial Assistance, Food, Health, Security and Safety, Housing and Residential Services, Multi-Faith, Foreign and International Students, Services for Student with Special Needs and other programs such as culture and arts, sports, social and community development.

D.3.1. Admission Services

The Office of Admission Services is tasked to take care of the processing of students’ entrance and requirements. Requirements and procedures for admission are in place with consideration/reasonable accommodation for those applicants and/or persons with disabilities as stipulated in RA 7277.

D.3.1.1. Goals and Objectives a. To screen prospective students both in the graduate and undergraduate levels for possible placement in the different courses offered by the College. b. To assist incoming students in their choice of course within the College and at the same time assess the student’s potential for higher education based on the specialization of the College.

D.3.1.2 Personnel The office is headed by the Head of Admission Services under the Supervision of the Director of Student Affairs and Services in close Coordination with the College Registrar, College Deans and, Guidance and Counselling Office.

D.3.1.3. Services Provided The office offers the following: a. Career orientation programs. This service which is given to secondary schools upon request, aims to promote the specialization of the college to prospective incoming freshmen students. On a broader level, this aims to assist graduating senior high school students in the choice of their course and college. b. Screens and interviews freshmen applicants. c. Administers scores and releases results of the ISPSC College Admission Test (ICAT)

D.3.1.4 Application Procedure Prospective first year applicants are required to apply in person to the Office of Admission Services to undergo the preliminary interview. a. The list of qualified applicants will be released either through posting in strategic places within the College or through mail notification.

D.3.2. The College Registrar

This office is headed by a College Registrar. The administrative staff of this office includes several registrars assigned to each college, data encoders and clerical staff.

D.3.2.1 Goals and Objectives a. To assist top administration in the formulation and enforcement of policies on admissions, enrolment, accreditation, graduation and other related academic matters. b. To maintain an updated registry of student records. c. to ensure a systematic and orderly management of student records. d. To prepare a periodic report on the statistical data as to enrolment and academic performance of the students.

D.3.2.2. Personnel a. The College Registrar heads the office b. The Administrative staff of this office includes several registrars assigned each to a college, statisticians, data encoders and clerical staff.

D.3.2.3. Services This office prepares and issues the following: a. Official Transcript of Records b. SF 10 (Formerly Form 137) c. D.3.3.2. Personnel a. The College Head of TVET and Scholarship reports directly to the Director of Student Affairs and Services. b. The Scholarship Committee is composed of the Director of Student Affairs, Head of TVET and Scholarship, Head of Admission, College Registrar, and the campus coordinators of scholarship.

D.3.3.3. Services a. Takes charge of the different scholarship and educational grants offered to students either by the college, the national government or by any interested individual, group of persons, or private company. b. Disseminates information on possible scholarship slots. c. Prepares and implements the Memorandum of Agreement between the grantor and the college. d. Conducts the screening of interested students wishing to avail of scholarship grants. e. Undertake the processing of scholarship papers of the students

D.3.3.4. Duties and Responsibilities of the Office a. Acquaint new students about the different scholarship programs that the College can offer; b. Assist the deserving /honor students and give scholarship; c. Help the students especially to the less fortunate who had financial problems on tuition fees and other related needs in school.(Applicable to Third & Fourth Year/Graduating students in the priority courses of the college); d. Coordinate at the Accounting office in preparing the Billing Statement and submit to the concerned agencies; e. Prepare payroll for various scholarship programs f. Collect Certification of Grades from all scholars and submit to the concern

agencies; g. Make financial reports of the different scholarship programs h. Collect the amount borrowed by the students in the STUFAP Loan program of the

college; and i. Prepare and submit the year-end report of the STUFAP Loan Program at the CHED Regional Office (every December during the annual meeting of all Scholarship

Coordinators) j. Coordinate with the Technical Education and Skills Development Authority (TESDA) regarding the different qualifications in Training for Work Scholarship Program; k.

Prepare and submit pertinent documents to TESDA for the training; and l. Collect the assessment and training cost from TESDA which serves as income of the college.

a.1. Free Higher Education (FHE) a.1.1.Starting A.Y 2018-2019, all Filipino undergraduates enrolled in the College will enjoy free tuition, miscellaneous, and other school fees, provided they:    Pass/meet the admission and retention policies of the College;    Have no previous undergraduate degree; and   Are not overstaying at the college level (e.g. maximum residency rule of the College plus one-year grace period as provided by law) a.1.2. Students who are not eligible will be charged tuition and other school fees at the rate of tuition and other school fees. a.1.3. Students should maintain good academic standing and comply with the retention requirements set by the College. a.1.4. For shiftees, any semester that the student has availed of the benefits for Free Higher Education will be subtracted from the expected duration of the current program in which the student is enrolled. a.1.5. For Transferees, any semester wherein the student has availed of any form of government-funded student financial assistance programs (StuFAPs) shall be subtracted from the expected duration of the current program in which the student is enrolled. a.1.6. For returning students and other students who have gone on a leave of absence (LOA), the determination and computation of the free higher education benefit shall be without prejudice to the most expedient way of completing their studies. a.1.7. Only the first copies/printouts of school ID, Library ID, and student handbook shall be free, any subsequent request by the student will be charged the usual fees. a.1.8. Student publication/newsletter are considered as part of “developmental fees” which are fees used to fund students’ activities, projects and programs. a.1.9. All high school graduates are eligible to apply for admission regardless of years completed (i.e. 4th year or Grade 12) or Senior High School Track. The cost of bridging programs that are approved by the Board of Trustees of ISPSC shall be billed to the UniFAST and reimbursed to the institution. The conditions to this are: (1) that the student is already admitted as a bona fide student; and (2) that the bridging program be embedded in the curriculum of the college (CHED Memorandum Order No. 10, s. 2017) a.1.10. All summer classes that are part of the curriculum will be covered by the Free Higher Education provisions of the law. a.1.11. If petition classes are provided, these should all be integral parts of the curriculum of the relevant programs. In addition, only the standard fees for offering programs shall be covered by the law. Special arrangements wherein institutions charge petitioning students higher tuition fees or ask petitioning students to cover the fees for an entire class, are not reimbursable to UniFAST. a.1.12. Tutorial classes are not covered under Free Higher Education

a.2. Free Technical-Vocational Education and Training (FTVET) a.2.1. All Filipino learners enrolled in their first National Certificate (NC) I,II shall be able to avail of free TVET. a.2.2. The following students are not eligible to avail of the free TVET.  Learners who have obtained a bachelor’s degree as of  the date of application of free TVET.  Learners who are already holders of a certificate or diploma for a technical-vocational program equivalent to NC III,IV, V, unless the trainee is enrolled in  bundled programs leading to level IV or diploma  Learners enrolled in TVET programs not registered  under TESDA  Learners who voluntarily opt out of the Free TVET  provisions  Learners who are not eligible to avail themselves of the Free TVET shall be charged the training cost and other training-related cost.

TES grantees can only enrol in the following:  Undergraduate course/program that has Board  Approval  Undergraduate courses with Certificate of Program  Compliance (COPC)  TVET Program with Certificate of Program Registration (COPR) and officially listed in the TESDA Compendium of Registered Program a.3.3. Students are not eligible to avail the TES benefits  Students enrolled in programs not included in the  Registry of CHED recognized programs  Students who have exceeded the Maximum Residency Rule of their program.   Learners enrolled in TVET programs not in the registry.  b. Children of ISPSC employees with permanent appointment are entitled to a full free tuition and laboratory fees, subject to the provisions as embodied in Section 10, Rule II of the Implementing Rules and Regulations of the Collective Negotiations Agreement:

b.1. The employee shall be a bona fide member of either the faculty or non- teaching personnel union for at least six (6) months and occupying a permanent plantilla position. b.2. d. Student Financial Loan Program (Graduating Students) Students may apply for a student loan through the Student Financial Assistance Program. The loan may be used for payment of school fees, projects, conduct of thesis and other emergencies. The loan is interest free and payable either in full or by instalment but it should be fully paid before the student graduates. A student needs a guarantor from the faculty or administrative staff and his parents before the loan can be granted. It is only available on a first-come-first served basis with preference to students from low income families, as there is only a limited number of students who can be serve for this purpose.

Health Services Health Services refers to services that provide primary medical and dental care and treatment, health education and wellness program, and is offered by the Health Services Unit to serve the needs of the students and employees of Ilocos Sur Polytechnic State College. It has a full time physician, dentist and nurses and is open from 8AM to 5PM with no noon break, Monday to Friday.

Provide preventive measures for simple and common ailments. c. Participate in the dissemination of health information to personnel and students. d. Create awareness among school populace on the importance of health and safety. e. Monitor the Implementation of the standards of school health program relative to school plan. f. Establish/ strengthen linkages with government and non-government organization for school community health work. g. Encourage the provision of needed health facilities, medicines and supplies. h. Promote healthy lifestyle such as but not limited to healthy diet, physical activities, and no smoking and drinking of alcoholic beverages and substance abuse, and provide healthy environment. i. D.3.5.5. Personnel The Health Services unit of the College is headed by the College Physician or Medical Officer. With him/her is the College Dental Officer and all the nurses of the College who equally distributed in all of the College campuses. The office has a direct coordination to the Office of the Coordinator of Institutional Student Program and Services, Office of the Director of Student Service and to the Office of the Vice President for Academic Affairs.

D.3.6.1. Personnel The Safety and Security Services is under the Office of the Student Affairs and Services with coordination to the Office of the Head of Security Services and CDRRMC under the Director for Administrative Services.

D.3.6.2. Objectives a. To establish a safe, accessible (for persons with disabilities) and secure environment for every students and personnel of the college. b. To establish mechanisms to look into the safety and welfare of the students and to the College in general. c. Conduct activities and programs addressing issues on disaster risk reduction and management concerns to include persons with disabilities’ needs in compliance with RA 10121 ( Philippine Disaster Risk Reduction and Management Act of 2010) d. D.3.7.2. Personnel The Head of Dormitory Services reports directly to the Director for Auxiliary and Resource Generation. It coordinates as well to the Office of Student Affairs and Services especially on issues pertaining to student concerns.

D.3.7.3. Services a. In the College 1. Operates student dormitory within the vicinity of the college 2. Gives priority to freshmen and students from more distant places, coming from low income families and those who apply earlier. b. Off-Campus Dormitories 1. Provide a list of accredited boarding houses and dormitories within the vicinity of the college. The list is made available to parents and students during the opening of classes. 2. Sets out guidelines to boarding houses and dormitories willing to be included to the roster of accredited residence quarters. Accreditation is made on a yearly basis and requirements are submitted to the office for evaluation. 3. Conducts an ocular inspection of the applying dormitory/boarding houses for verification purposes.

D.3.9.1. Goals and Objectives a. To ensure that academic accommodation is made available to persons with disabilities (PWD) and learners with special needs b. To provide avenues for proper consultation and conference with students with disabilities, indigenous people, solo parents etc. themselves together with their teachers, parents/guardians, personal assistants and other concerned professionals whenever necessary. c. To established programs and activities designed to provide equal opportunities to persons with disabilities (PWD), indigenous people, solo parents, pregnant women etc.

D.3.9.2. Personnel The Coordinator of the Student affairs and Services of the College going to the Director of Student Affairs and Services looks after this particular service. He shall likewise coordinates the activities of the different student clubs and organizations of students with special needs and persons with disabilities of the college. And lastly, coordinates with the Head for Health Services for profiling.

D.3.9.3 Services a. Provisions/programs for life skills training intended for Students with special needs, persons with disabilities etc. b. To create better atmosphere for the members of ISPSC Family in becoming Citizen Sportsmen, building good rapport, cooperation and teamwork in achieving a common goal the “The Fitness for All”.

b. To encourage greater participation in sports, recreation and wellness activities in the Campuses of the College.

c. To develop the habit of joining the three areas of concern (Sports, Recreation, and Wellness Program) as a way of life in order to attain the ultimate goal of the program “The Good Life”.

D.3.11.4. Objectives a. To encourage more participation of all students, faculty and personnel in the sports, recreation and wellness activities. b. To conduct regular activities on sports, recreation and wellness program. c. To arouse interest in, and to support the sports development through participation within the campuses of the college. d. To attain its goals of becoming a citizen sportsmen, physically fit and enjoying good life. e. To conduct regular amateur sports competition to develop and select athletes who are worthy representatives of the college in higher level of competitions. f. To properly train selected athletes so that they will bring honor and glory to the college. g. To produce citizens who are properly developed physically, emotionally, culturally, and mentally healthy, and: h. To develop high level of knowledge and skills, understanding and appreciation, and positive attitudes and values designed to optimize the full potential of athletes and students.

D.3.11.5. Personnel

The office is being headed by the Head of Sports Development/PE under the office of the Director of Student Affairs and Services and to the office of the Vice President for Academic Affairs.

Further, each campus of the College have individual sports development coordinator who is tasked to supervise related activities undertaken in each campuses.

D.3.11.6. Services and Program of Activities

a. Wellness Program – Physical Fitness Activity at least twice a week for all. The campus Sports Coordinator in cooperation with the Medical Health Unit and all PE faculty will prepare the Activities for this purpose. b. Recreational Activities – in order to encourage more participation, each campus will provide a recreation Hall and Fitness area/ gym / room were all recreational games and fitness equipment and paraphernalia will be housed and it will be open for everybody’s use. c. Sports Development Program - as being mandated by the law, all educational institution shall undertake regular sports activities. Every member of the

Selected players who won in the Intramurals/ Intercampus/ Interschool/ Intercollege. d. Must be a bona fide college students and officially enrolled of the current Academic year. e. Must carry a minimum academic load of twelve (12) units in the first and second semester or term of the current school year must have passed the required percentage as follows: (PASUC / SCUAA GUIDELINES 2017)

D.3.12. Library Services

D.3.12.1. Mission, Goals and Objectives

Mission The College Library exists to support the philosophy, vision and mission of the Ilocos Sur Polytechnic State College and the goals and objectives of the different Colleges, providing excellent library services on behalf of instruction, research, extension, production, and other scholarly activity. To this end the Library works closely with the faculty, staff and students of the college to achieve its goal and objectives, specified below.

Goals Provide the best possible access to relevant information within a suitable environment. In pursuit of this goal, development efforts revolve on two major thrusts:

1) development of the Library collection focusing on subject areas which support new academic program and new areas or research, and 2) computerization of Library resources and services for faster access to the Library’s holdings.

Objectives a. Develop and maintain appropriate collections of print, non-print, and electronic resources in support of ongoing College functions; b. Provide instruction in library and information literacy skills aimed at facilitating research and developing an individual’s ability to select, evaluate and use information resources; c. Use technology to improve the delivery of programs, services and resources;

d. Promote library and information services and resources to faculty, students and staff; e. Develop and provide facilities that promote effective study and research; ensure appropriate storage, maintenance and preservation of information resources; enhance staff productivity and collaboration; and serve as a community gathering place for the exchange of ideas; f. Cooperate with other libraries, museums and historical societies in the province, region and nation in increasing the availability of resource materials; g. Encourage the habit of reading and the use of libraries in order to develop students’ potential for self-education and intellectual development for lifelong education; h. Administer ongoing analysis, planning and assessment to ensure effective stewardship of library resources and enhance the Library’s ability to meet its mission and goals.

D.3.12.2. Personnel The head of the Library Services is a certified librarian with regular faculty status who has additional specialized training and certification endorsement in library media centre programs and activities. The Librarian is responsible for the operation of the library within the college and for related media service and implement policies and guidelines as formulated by the Library Committee.

D.3.12.3. Library System and Services Each of the six campuses of the College maintains a library that provides materials and services to meet the instructional, research and extension needs of the various constituents of the College. Each library serves the students, faculty, researchers, officials and employees of ISPSC and the Graduate School. The different college libraries also share their collections among themselves.

Researchers from other institutions and government offices or agencies, teachers and students from other schools, alumni, government officials/workers and private individuals are allowed to avail themselves of the resources within the library premises, subject to the rules and regulations of the College.

An open-shelf system is used in all the campus libraries for greater access to the various academic materials. The classification scheme used is the Dewey Decimal Classification System.

Every campus library is open from 7:30 AM to 5:00 PM every day, no noon break (except holidays). Students can make use of the library from 11:30 AM to 1:00 PM for their reading and research works. For campuses offering graduate studies, the library is also open from 7:30 AM to 5:00 PM during Saturdays.

Continuous instruction in the use of the library and its resources is provided and lending policies is fair and extended to all legitimate users. The library also offers bibliography making, indexing and abstracting services.

D.3.12.4. Circulation Policy

b. School Identification Card School Identification Card is the permit/passport of a student to use the library. It must be presented at the Control Desk upon entrance whenever you check out library materials. Student ID card must be presented each time a Circulation book is borrowed or returned. Visitors should present Visitor’s ID given by the guard on duty.

c. Circulation Policies The length of borrowing and quantity of materials borrowed by students and faculty/staff should be based on the size of the collection, the needs of the school, and the availability of reliable shelves.

D.3.12.5. Computer and Internet Policy and Guidelines The College Library Committee is committed to ensuring access to information through technology. Access to digital information is fundamental for supporting continuous lifelong learning. Recognizing its role to provide access to electronic information, the College offers public access to the Internet.

The Internet is an immense global network of networks that is constantly growing and evolving, allowing an online user to communicate and connect to information around the world.

The Library upholds and affirms the right of every individual to have access to materials constitutionally protected on the Internet. The content of the Internet is not managed or governed by any entity, therefore users may encounter materials they consider offensive. The library supports both open and guided access to information. The Library also supports the right to privacy and confidentiality of Library users in accordance with the Intellectual Property Rights Law. Parents and guardians are responsible for monitoring Internet access by children. The Library Committee and its employees assume no responsibility for damages of any type arising from the use of the Centre’s internet workstations.

Illegal use of the Internet is prohibited. Library users may not use the Library’s Internet access to view, print, distribute, display, send or receive images, text or graphics of obscene material or material that violated laws relating to child pornography. Library

users may not disseminate, exhibit or display to minors materials that are harmful. Library users must abide by the Internet Use Guidelines as approved by the Library Committee.

D.3.13. Ancillary Services The Coordinator for Ancillary Services provides assistance in other student- related concerns such as the identification cards, insurance and uniforms.

D.3.13.2. Student Group insurance The College offers a student group insurance Package to all students and is paid upon enrolment. College Deans are required to prepare a list of enrolees to be submitted to the Vice President for Administration for consolidation. Documents relative to the processing of claims should be complete. School Uniform The college prescribes and may sell uniforms to students at reasonable price. Every student should wear the prescribed uniform on the days specified by the College. They are likewise required to wear the shop/laboratory uniforms during their laboratory period; the prescribed uniform during the PE classes and NSTP uniform during their NSTP classes. Learning Resource Center Apart from the College Library, the different colleges have established their own Learning Resource Centers through the effort of the college faculty members and student councils. These complement the educational needs of the students by making available some books, periodicals, magazines and other information tools.

E. Implementing Guidelines on Incentives to Board Examination Placers (Board Resolution No. 815, s. 2016) 1. Cash incentives and a testimonial dinner will be given to placers of PRC Board Examination directly aligned to the Bachelors’ degree. Cash incentives for the placers of the board exams are as follows: Php. 50,000.00 - 1st to 3rd Placer Php. 30,000.00 - 4th to 6th Placer Php. 15,000.00 - 7th to 10th Placer a. The list of PRC Board Examination with the corresponding bachelor’s degree are covered but not limited to the following depending on the offered course of the College:

Cash incentives and a testimonial dinner will also be given to placers of allied PRC Board Examination indirectly aligned to the Bachelors’ degree, accredited national certification examination by professional organizations, other government initiated examination. The Board Examinations are covered but not limited to the following: a. PRC Board Allied Exam b. Professional Organizations Examinations such as IT and ICS c. Other Government Initiated Examination (TESDA)

Testimonial dinner will be held once a school year that includes all the Board Exam placers and passers, winners and achievers in all levels of competitions within the covered year charged to the Student Activity Fund. 4. For those who will not land in the Top 10 National Board Passers, a cash incentive of Php. 10,000.00shall also be given to the highest individual board passer per discipline based from the performance rating among all examinees system-wide with a rating not lower than 80.00 per cent.

Sources of Fund (i.e cash incentives) shall be coming from the collection of fees like Student Activity Fund (SAF), Sports, Cultural and Publication Funds complementing each other depending on the availability of the particulars of events and competitions.

4. Municipal, institutional and collegiate level of achievements will be awarded during the scheduled collegiate recognition day.

5. Only student achieves who made it in highly competitive events duly endorsed by the Office of the President are entitled to receive cash incentives. To recommend to the President, a committee shall be formed to be led by the Vice President of Academic Affairs as Chairman and Director for Student Services, Head for Culture, Arts and Heritage and Head of Sports Development as members to screen whether the competition is highly competitive or not.
