NORFOLK PUBLIC LIBRARY TEEN TECH WEEK TOURNAMENT ENTRY FORM/PERMISSION SLIP The game is Super Smash Bros for Wii U. Rated E for Everyone. This permission slip guarantees that parents/guardians are aware of the video game and that their child’s photo may be used by the Norfolk Public Library.

RULES: Gamers must behave in a manner respectful of the other participants, their personal property, and library property. All library rules of conduct apply. Gamers who do not follow the rules or who behave inappropriately will be asked to leave. 1. Gamers play at their own risk. Norfolk Public Library cannot be responsible for participants health or fitness. 2. ALL gamers must be between 10–18 years old. No exceptions. 3. All gamers must handle themselves in a courteous manner. No one likes a poor sport. 4. Abuse of any equipment (consoles, controllers, projectors, etc.) will result in the gamers disqualification from the tournament and reimbursement of any damaged equipment. 5. No cheating. If the suspicion of cheating arises the gamer will forfeit the tournament. 6. Arguing with the library staff about a decision may result in disqualification from the tournament. 7. Permission slip must be signed by a parent/guardian and presented to library staff at registration. 8. Photos from this program may be used in presentations about video games, online and identified by the library and used in future promotions. The signature below serves as permission for the gamer’s photo to be used in any of the above ways. HOW THE TOURNAMENT WORKS: Two gamers compete in a single elimination tournament. A limited number of gamers can participate (max. of 30 per branch). Sign-up begins 30 minutes prior to the start time of the branch tournament and ends ten minutes after. Practice prior to the tournament for best results. Library staff will complete a paper bracket with the registered gamer’s name and character of choice. The game will be set up the same for each match: 3-stock, no specials (NO SMASH BALL), and on the Battlefield stage. The top three winners from each branch tournament will advance to the Championship. A prize will be awarded to each winner, as well as a ticket to enter the Championship. The Grand Prize for the Championship winner is a 3Doodler Create 3D Printing Pen.
