Chen and Guo Journal of Medical Case Reports (2018) 12:188 https://doi.org/10.1186/s13256-018-1732-x CASE REPORT Open Access Unusual case of primary spontaneous hemopneumothorax in a young man with atypical tension pneumothorax: a case report Youwen Chen* and Zhijian Guo Abstract Background: Spontaneous life-threatening hemopneumothorax is an atypical but treatable entity of unexpected circulatory collapse in young patients, affecting 0.5–11.6% of patients with primary spontaneous pneumothorax. Spontaneous pneumothorax is a well-documented disorder with a classic clinical presentation of acute onset chest pain and shortness of breath. This disorder might be complicated by the development of hemopneumothorax or tension pneumothorax. Case presentation: A 23-year-old Asian man was referred to the emergency room of Xiamen Chang Gung Memorial Hospital with a 1-day history of right-sided chest pain that had been aggravated for 1 hour. A physical examination revealed a young man who was awake and alert but in mild to moderate painful distress. His vital parameters were relatively stable at first. The examining physician noted slight tenderness along the right posterolateral chest wall along the eighth and tenth ribs. Primary spontaneous pneumothorax was considered, and a standing chest X-ray confirmed the diagnosis. A right thoracostomy tube was immediately placed under sterile conditions, and he was referred to the respiratory service. While in the respiratory department, approximately 420 mL of blood was drained from the thoracostomy tube over 15 minutes. Our patient developed obvious hemodynamic instability with hypovolemic shock and was subsequently admitted to the cardiothoracic surgical ward after fluid resuscitation. -

Ten minutes after arrival in the emergency department the patient starts to have shortness of breath with stridorous sound. He is now diaphoretic and pale. B/P- 80/40, HR- 140, RR- 36 labored. Background. The management and outcomes of occult hemopneumothorax in blunt trauma patients who required mechanical ventilation are not well studied. We aimed to study patients with occult hemopneumothorax on mechanical ventilation who could be carefully managed without tube thoracostomy. Methods. Chest trauma patients with occult hemopneumothorax who were on mechanical ventilation were prospectively evaluated. The presence of hemopneumothorax was confirmed by CT scanning. Hospital length of stay, complications, and outcome were recorded. Results.Atotalof56chesttraumapatientswithoccult hemopneumothorax who were on ventilatory support were included with a mean age of 36 ± 13 years. -

The management of hemodynamically stable patients is still under dispute. The aim of this study is to review and improve our management of penetrating abdominal injuries. Methods We retrospectively reviewed a 10-year period from the Trauma Registry of our level I trauma center. The data of all patients regarding demographics, clinical and outcome parameters were examined. Further, charts were reviewed for FAST and CT results and correlated with intraoperative findings. Results A total of 115 patients with penetrating abdominal trauma (87.8% men) were analyzed. In 69 patients, the injuries were caused by interpersonal violence and included 88 stab and 4 firearm wounds. 8 patients (6.9%) were in a state of shock at presentation. 52 patients (44.8%) suffered additional extraabdominal injuries. 38 patients were managed non-operatively, while almost two-thirds of all patients underwent surgical treatment. Hereof, 20 laparoscopies and 3 laparotomies were non- therapeutic. There were two missed injuries, but no patient experienced morbidity or mortality related to delay in treatment. -

Assessment of the airway patency of the victim. Technique of ensuring airway patency (head tilt; jaw thrust; insertion of the nasopharyngeal and oropharyngeal airways). Assessment of the upper airway of the victim usually requires positioning the individual supine on a flat, firm surface with the arms along the sides of the body. Unless trauma can be definitely excluded, any movement of the victim should consider the possibility of a spine injury. As the patient is placed supine, stabilize the cervical spine by maintaining the head, neck, and trunk in a straight line. If the neck is not already straight, then it should be moved as little as possible to establish the airway. If the patient cannot be placed supine, the jaw thrust maneuver can be applied with the rescuer at the victim’s side. Common causes of airway obstruction in an unconscious patient are occlusion of the oropharynx by the tongue and laxity of the epiglottis. With loss of muscle tone, the tongue or the epiglottis can be forced back into the oropharynx upon inspiration.
