1 Executive Summary
Annual Implementation Report No. 2, covers the performance period for year 2 of the IQA program starting from April 01, 2016 to March 31, 2017. This report is submitted pursuant to Task Order Deliverables under Clause F.5.1.3, at the end of the second performance year. This AIR comprises an introduction to the project, results achieved during the second year of the project implementation, problems encountered and actions suggested to enhance qualitative and quantitative values of the works. The report also shows a preview of public events like collecting feedbacks of local community directly benefiting from the USAID stabilization program.
The main objective of the annual report is to give a preview of overall status of different ongoing and upcoming sub-projects in buildings, roads, power and water sectors during this period. USAID enhanced the projects list by adding new projects under FIP & KPRP. These sub-projects include rehabilitation and reconstruction of roads, grid stations, education buildings, rehabilitation of damaged electrical infrastructure and construction of solar based tube wells for irrigation and water supply.
The report also indicates issues arising throughout the year in each sector and suggestions by HPK to tackle these issues. Later in the report, recommendations from HPK to crop better results in the upcoming years are suggested.
HPK categorized all the subprojects assigned to it in three zones i.e. North, South and Centre. The North Zone comprises districts in Malakand Division and Bajaur Agency, while the South Zone comprises North/South Waziristan Agencies, and Bannu, Tank & DI Khan districts. Central zone comprises districts of Nowshera and Charsadda.
One of the core components of the annual report is to demonstrate the accomplishments made within the year. Focusing on key deliverables during second year of the project, HPK successfully;
Reviewed and verified the structural, architectural, electrical and WATSAN designs for signature projects like Tehsil Level-Government Complex, Kabal and GHSS Akbar Pura, Nowshera. Checked and verified detailed Engineer’s cost estimates for construction projects including schools, governmental buildings, basic health units, transmission lines, grid stations, roads, and bridges. Provided construction quality assurance for all the ongoing construction projects. During this tenure 12 militancy damaged schools, 07 BHUs, 04 water supply schemes and 02 headworks in the North Zone attained final completion. In South Zone allied works on DI Khan-Tank Road and BMG Road, 01 bridge on WSM Road and 05 bridges on BMG Road attained final completion. In addition, numerous subprojects are near completion.

Certified US$ 32.026 Million worth of payment milestones in accordance with the relevant Project Implementation Letters (PILs) in all four sectors of the project.
USAID added reconstruction of 57 flood damaged schools to HPK’s scope in November 2015 and physical construction on all the schools is underway and assessment surveys for further schools were conducted with PaRRSA. Work on 23 new schools is about to commence soon.
In January 2017 through a modification no. 04 in the Task Order, USAID assigned the task to HPK to inspect the ongoing building works funded by USAID on their behalf. Under this amendment, HPK visited various buildings in KP and Gilgit Baltistan.
During the course of construction numerous types of issues are generally encountered which are regularly communicated to USAID on weekly / monthly basis. A preview of some of the encountered issues is described in the relevant section.
This report likewise covers the prompt actions taken by HPK to address the issues. HPK suggested corrective measures to USAID and other stakeholders throughout the year and communicated the status of all the subprojects through its reporting system which, as per provisions of the TO contains field visit reports, weekly email updates, monthly progress reports etc.
The security situation in the North, South & Central Zones remained peaceful due to effective involvement of LEAs. During this tenure site visits by HPK staff were regularly conducted adhering to the security protocols set forth in the security assessment plan.
Task Order deliverables which serve as means of communication with USAID are also discussed in detail in this report. TO deliverables include but are not limited to QA Plan, Pre-Construction Visit Reports, Security Plan, Monthly Progress Reports, Weekly Email Updates, Minutes of Coordination Meetings and Success Stories etc. During the second year HPK successfully submitted all deliverables within the stipulated time frame.
The potential adverse environmental impacts of the construction activities on local environment, were monitored by HPK throughout the year as per guidelines and recommendations of the Environmental Documentation Form (EDF) and Environmental Management and Monitoring Plan (EMMP).

2 Project Introduction
The United States Agency for International Development (USAID) has appointed Halcrow Pakistan (Pvt.) Ltd. (HPK) in March 2015 to provide Architect-Engineer services for Independent Quality Assurance (IQA) of USAID’s Stabilization Program Activities in the Federally Administered Tribal Areas (FATA) and Khyber Pakhtunkhwa (KP) province.
Further, in January 2017 via Modification 4 to the Task Order, the USAID expanded the geographical and programmatic scope of HPK. The objective was to conduct third party visits to various projects funded by the USAID across Pakistan on their behalf.
HPK is required to provide services on activities being undertaken in the following two USAID stabilization programs: 2.1 FATA Infrastructure Program
In FATA Infrastructure Program, USAID is in partnership with FATA Secretariat. The main objective of the FATA Infrastructure Project is to improve conditions for the long-term development of the Federally Administered Tribal Areas by constructing or rehabilitating public service infrastructure in Buildings, water, energy, irrigation systems and roads.
FATA Infrastructure Program is being implemented in FATA as well as in KP Province. The project, financed by USAID under Fixed Amount Reimbursement (FAR) procedure, was started in 2010. The project aimed to restore and rehabilitate the infrastructure in the four major sectors including Buildings, roads, power and water. Halcrow Pakistan (HPK) had been entrusted by USAID to provide Architect- Engineer services for Independent Quality Assurance on this project in FATA & KP.
In March 2017 USAID enhanced the projects list by adding new projects under FIP. These sub-projects include rehabilitation and reconstruction of roads, grid stations, education buildings, rehabilitation of damaged electrical infrastructure and construction of solar based tube wells for irrigation and water supply. FATA Secretariat has hired the services of M/s Cameos Architects & Consultants to provide the engineering services such as identification, site assessment and detailed engineering designs for the upcoming “Rehabilitation and Reconstruction of Damaged Infrastructure in the Conflict Affected Areas of FATA”. 2.2 KP Reconstruction Program
In KP Reconstruction Program, USAID is in partnership with Provincial Reconstruction Rehabilitation and Settlement Authority (PaRRSA). The main objective of the KP Reconstruction Program is to enhance the stabilization and development of Khyber

Pakhtunkhwa by rebuilding public infrastructure for education, health, water, roads, sanitation and increasing the capacity of the provincial government.
The people of KP, especially of Swat, Dir (L), Dir (U) and Buner districts of Malakand Division, suffered heavy losses in the war against terrorism and militancy. During more than one year of violence in the valley of Swat, Dir (L), Dir (U) and Buner districts, the suspected militants mainly used the destruction of schools as an effective tool to bring the government to its knees, while reversing the process of education in the restive mountainous districts. According to an official data, the insurgents destroyed 177 (primary, middle and high) schools, when the tension gripped the districts. The violence came to a halt following the Pakistan Army offensive against the Taliban in 2009.
For post conflict, Malakand Reconstruction and Recovery Program (MRRP) (Now KP-Reconstruction Program), an Assistance Agreement (No 391-011) was signed in September 2009 between the GoP and the USG. This was followed by an Implementation Letter (IL) No. MLK-01, signed in December 2009 for initial obligation of funds of US$ 36M for the rehabilitation and/or reconstruction of damaged facilities of Education, Health, Watsan and enhancing government capacity in the four districts of the Malakand Division: Dir. (L), Dir. (U), Swat and Buner. The project started in June 2010. Later, an additional sum of about US$ 16 M was sub-obligated for flood affected infrastructures in KP/ FATA and for capacity building of government in KP. The Independent Quality Assurance for this Project was awarded to HPK in April 2015; the IQA services are expected to continue for Four (4) years.
After addition of 57 flood damaged schools to the scope in August 2016, joint identification of new schools is also underway and approximately 50 more schools are likely to be added to the scope. Thus the expected figure will be 100 plus.
Construction activities on the signature projects like GGHS Akbar Pura located in District Nowshera and Tehsil Level Judicial Complex, Kabal, District Swat are likely to commence as the design review task is completed by HPK. 2.3 Types of Projects
The sectors and the types of engineering projects in this Task Order (TO) include:

Geographically planned / upcoming projects under FATA Infrastructure Program are located in South Waziristan, Bajaur, Orakzai, Kurram, Mohmand and Khyber agencies and Frontier Regions of Peshawar, Kohat and Tank.
The Projects in KP are primarily in areas that are part of the Malakand Division and other districts of KP like Charsadda, Nowshera and DI Khan.

2.5 Scope of HPK Services
HPK is required to provide the following services under this task order:
Review of designs, drawings, technical specifications, Bills of Quantities (BOQ), engineer’s estimates prepared by the Government of Pakistan (GoP) agencies/departments; and Independent Quality Assurance (IQA) for construction activities in accordance with the technical specifications, construction drawings and EDF/EMMP. Verifying the payment milestones as identified in Project Implementation Letters (PILs).
Under amendment 04 to the Task Order of HPK is expanded the geographical and programmatic scope for providing Architect- Engineer services. Modification is made to replace services provided in "KP and FATA" with "throughout Pakistan". HPK would now be providing the same services outlined in this award throughout Pakistan and not only in Khyber Pakhtunkhwa and Federally Administered Tribal Area.
This document aims to provide a plan for undertaking quality assurance of construction activities and milestone certification.
Although QA roles may be over-lapping between the supervision consultants and the IQA contractor i.e. HPK, this document has been prepared with the objective to elaborate how HPK undertakes its role as independent quality assurance contractor.

3 Results Achieved
During the second year of the project, following targets were set and successfully achieved by HPK as per TO requirements;
Element-1: Provided general management and administrative support to USAID, including reports & deliverables; Element-2: Provided design quality assurance and reviewed bill of quantities (BOQs) and cost estimates; Element-3: Provided construction quality assurance support to USAID, including reports & deliverables.
Accomplishments against TO deliverables are described in detail in the following sections and summarized in Annexure-A. 3.1 Element 1: General Management and Administrative Support
Under this element the activities executed for the second year are covered but not limited to general advice and support, overall management and administrative support, security, performance monitoring, branding and marking and communications with USAID through reports as per TO by deploying the proposed staff in the project / design offices
Annexure-B presents an organization chart of all HPK personnel who rendered services during the second year of the project.
3.1.1 General Advice
HPK provided general advice to USAID FATA/KP office and its G2G partners that related mostly to engineer’s estimates, preparation of milestones, revision of PILs and other issues relating to power, building and road sector projects.
3.1.2 Overall Management & Administrative Support
HPK provided management and administrative services by using proper and efficient management and administration practices, information technology, reporting system, human resource management, asset control, security and cost control systems.
HPK further ensured an effective and optimized use of staff, office space and other facilities.
The deployed human resources and their utilization in the first year have been outlined in Annexure-C.
3.1.3 Security
A Security Assessment Plan has been prepared and duly approved by USAID was framed to mitigate prevailing threats and risks to enable the staff to discharge their duties in a safe and secured

environment. Standard Operating Procedures (SOPs) for each activity have been formulated to be followed in different eventualities during all project related activities by the project staff.
The security situation in KP is calm however, incidents of target killing and bomb blasts occur occasionally. But the security situation in FATA areas has been volatile despite military operation “Zarb-e- Azb”.
While planning field visits of any HPK IQA staff member a Journey Management Plan (JMP) is prepared by the project security manager with the approval of Chief of Party. The Visiting team follows the travel protocols as per duly approved Security Assessment Plan.
While planning field visits in FATA areas permission for entering is sought through FATA Secretariat and concerned political agents. After the consent from the political agent is received the HPK IQA staff will visit the construction sites with the security cover managed by the security department of HPK.
Considering the upcoming projects in Orakzai, Kurram, Mohmand, Bajaur, Khyber and South Waziristan Agencies, HPK security team is planning to carry out threat and risk assessment of all the project areas as per HPK’s Security Assessment Plan and the dynamics of the FATA regions, the security measures will be taken accordingly.
In perspective of overall security situation in the country the staff is warned time and again about the various risks and threats through training sessions and security drills etc.
3.1.4 Reporting System
HPK timely submitted all reports, deliverables or outputs required under the Task Order i.e. Quality Assurance Plan, Pre-Construction & Field Visit Reports, Security Plan, Cost Control Report, Branding and Marking Plan, Monthly Progress Reports, Weekly Email Updates, Minutes of Coordination Meetings, Success Stories, Annual Implementation Report and PAKINFO Reporting.
3.1.5 Project Communications
Chief of Party (CoP) as well as HPK’s key staff coordinated with USAID TOCOR for all the communications set forth in the TO. The CoP frequently provided weekly email updates and held numerous coordination meetings with USAID and other stakeholders. 3.2 Element 2: Design Quality Assurance and Cost Review
HPK provided design quality assurance under the task order from time to time. The activities executed during the second year of the project under this element also covered review of bill of quantities

(BOQs) and engineer’s cost estimates. The activities executed for the second year under this element are detailed in this section.
3.2.1 QA for Design Review Services
HPK reviewed structural designs and drawings to ensure its conformity to the desired codes and standards. HPK reviewed the structural, architectural, electrical and WATSAN designs and drawings of the following sub-projects during the reporting year;
Tehsil Level-Government Complex in Kabal, District Swat. Government Higher Secondary School Akbar Pura, District Nowshera.
After conducting the pre-construction visit to Jamrud Bypass located in Khyber Agency during the reporting year, design review was started on directions of USAID.
3.2.2 Third Party Inspection
In January 2017 through a modification no. 04 in the Task order, USAID assigned the task to HPK to inspect the construction process of substantially completed building structures all over Pakistan on their behalf. Under this amendment, HPK visited the below listed ongoing projects and submitted comprehensive reports comprising site observations and conclusions.
i. Centre for Advanced Studies in Energy, UET Peshawar ii. Faculty of Education - Karakoram International University, Gilgit iii. Pre-fabricated Smart Container School - GGPS Daulat Tajazai, Lakki Marwat iv. Burns & Trauma Centre, HMC Peshawar
In addition to the above stated projects, further such visits are anticipated in near future.
3.2.3 Design Codes & Technical Specifications
Testing was carried out as per ASTM, AASHTO, BS EN, however mix-designs of asphalt/cement concrete was made according to relevant sections of American Asphalt/Cement Concrete Institute Manuals. Similarly hot and cold weather concreting was done according to ACI Manual sections 305-1 and 306-1. Concrete test results were evaluated as per ACI-214.
Standard specifications of National Transmission & Dispatch Company (NTDC) WAPDA were followed for inspection and testing of electrical material/equipment.

3.2.4 Review of Bill of Quantities (BOQs) and Engineer’s Cost Estimates
HPK periodically reviewed numerous BOQs and engineer’s cost estimates of sub-projects under buildings, roads, power and water sectors. These cost estimates served as the basis for establishing costs in the PILs in concurrence with existing site conditions as mentioned below:
i. Review of revised engineer’s estimates of all partially and fully damaged BHUs in Malakand Division. ii. Review of pending milestones of Munda and Amandara Head Works for PIL revision. iii. Preparation of revised milestones of Khwaza Khela Bridge in District Swat. iv. Revision of milestones and review of engineer’s estimate of militancy damaged schools (PIL-09). v. Revision of milestones and review of engineer’s estimate of partially damaged BHUs (PIL-21). vi. Preparation of milestones and review of engineer’s estimates of flood damaged schools (PIL-28).
In addition to above stated reviews of engineer estimates and revision of milestones, below reviews are underway:
i. Review of engineer’s estimate of GHSS Akbar Pura. ii. Revision of milestones and review of revised engineer’s estimate of flood damaged schools (PIL-30). iii. Review of engineer’s estimate of new 23 flood damaged schools. iv. Review of engineer’s estimate of Jamrud Bypass. v. Review of engineer’s estimate of additional works on BMG Road. vi. Review of engineer’s estimate of 132kV Transmission Line from Timergara to Khar.

3.2.5 Element 3: Construction Quality Assurance
HPK delivered Independent Quality Assurance services for the second year of the project as per the Task Order. Activities described under this element are, establishment of field offices, construction quality assurance, inspections, environmental compliance and record keeping. The activities executed under this element are detailed below:
3.2.6 Establishment of Field Offices
As per original task order HPK has already established project office in Peshawar and field offices in DI Khan and Swat and to conduct IQA activities for the ongoing 57 flood damaged schools, 132kV Grid Station and Transmission Line Khar, HPK established an additional field office in Timergara and additional laboratories in Timergara and Peshawar. It is pertinent to mention here that almost 40 flood damaged schools are likely to be added to the scope.
However, to cater the upcoming projects in Orakzai, Kurram, Khyber, Mohmand, Bajaur and South Waziristan Agencies and in Frontier Regions of Peshawar, Kohat and Tank, HPK plans to shift its DI Khan office to Hangu or Kohat to meet the upcoming sub-projects requirement.
HPK plans to hire additional staff for upcoming works in FATA regions after USAID’s approval as the pace / quantum of work expedites.
3.2.7 Construction Quality Assurance, Inspections & Testing
The project teams deployed in North, South and Central Zones continued and followed procedures as laid down in the approved QA plan to ensure that all construction activities were performed in accordance with the approved designs, drawings and technical specifications.
During site inspections HPK field staff performed the following main activities;
Conducted independent inspections of the ongoing works as per frequency outlined in the QA Plan. Verified the quantities and quality of construction/installation work and ensured conformity to design drawings, technical specifications and requirements by conducting requisite tests as per codes and standards during the course of construction of each sub-project. Certified invoices of the implementing agencies for payments based on the acceptance of works after independent quality testing.

Monitored environmental compliance. Reported safety conditions at construction sites and identified violations of safety regulations. Recorded security incidents, extreme weather conditions, and any other events that could potentially affect the progress of work. Regularly communicated the above works to USAID along with pictorial records.
The comparison of progress of all the subprojects between April- 2015, April-2016 and March 2017 has been outlined in Annexure-D.
3.2.8 Environmental Compliance
Environmental monitoring compliance of the project against approved Environmental Documentation Form (EDF) and Environmental Mitigation and Monitoring Plan (EMMP) is also the main component of the Task Order.
A two tiered environmental compliance monitoring protocol was followed which was inspection of environmental compliance against the approved EMMP by the HPK field staff visiting sites on regular basis and by an HPK Environmental Compliance Specialist (ECS).
HPK conducted a detailed assessment of local environmental and social conditions of the project area. It is pertinent to note that the assessment was done only under the scope of EMMP and comprehends the issues and concerns identified in the document only.
During the course of time, environmental screening was carried out for upcoming sub-projects in Frontier Regions of KPK. Details of environmental aspects of sub-projects were reported to USAID in form of pre-construction reports.
All environmental non-compliances noted were reported to USAID with recommendations for corrective actions in the form of field visit reports and monthly progress reports. Field supervision teams followed up with the construction contractors for implementation of corrective actions. Major issue observed at site was the unawareness of supervisory consultant and contractor about the environmental component of the project. All non-compliances were discussed in detail at site with contractors and supervisory consultants for rectification and advised for implementation of EDF/EMMP.
During the reporting year as per Task Order clause no. F.5.1.10, a success story of DI Khan – Tank Road was prepared and submitted to USAID. The submitted success story is attached as Annexure – E.

3.3 Milestone Certification for Construction Projects
HPK certified the achievement of payment milestone of all the ongoing construction projects during this period. Payment milestone certification was done in accordance with relevant PIL based on established testing protocols as per QA Plan.

4 Significant Implementation Problems
Sector wise significant implementation problems encountered during the first year of IQA Project are described below: 4.1 Building Sector
i. Work on Water Supply Schemes is very slow especially in Buner and Swat. Some contractors have abandoned the work due to nonpayment of the executed tasks. In Buner, only 05 schemes and in Swat only 01 scheme are completed but work on the remaining schemes is either stopped or going on a slow pace. PaRRSA needs to ease the cash flow of the executing agencies (TMA Mingora in Swat and PHED in Buner) to clear the liabilities of the construction contractors. ii. Approval of the revision in PILs of flood damaged schools is pending which is required to regularize payment of the completed milestones. USAID to expedite the process of approval. iii. Commencement of work on Tehsil Level-Government Complex Kabal is significantly delayed due to delay in redesigning of the buildings by the consultant PePAC. It is already awarded to the construction contractors. iv. Work on the signature project GHSS Akbar Pura could not be started due to delay in design by the design consultant SHAZ. v. For flood damaged schools, information like construction drawings are not being shared timely by the executing agency and design consultant. 4.2 Road Sector
4.2.1 BMG Road
i. Work on joint/groove has been completed in the constructed rigid pavement except the last 165 m length where joint sealant is yet to be provided. ii. FWO/NESPAK are yet to provide the breakup of cost estimate for proposed works against savings on BMG Road.

4.3 Power Sector
4.3.1 132kV Grid Station Khar, Bajaur Agency:
i. The limited capacity of the supervision consultant M/s Allied Engineering Consultant (AEC) necessitated the enhanced frequency of visits of HPK’s IQA team. The IQA team had conducted 46 visits to the grid station till the reporting year. ii. Balance set of Factory Lab tests results as well as inspection certificates of already inspected local / imported material and equipment duly vetted by AEC are yet to be submitted by TESCO to HPK for review. iii. TESCO is yet to provide a complete set of corrected approved foundation drawings (AutoCAD) duly vetted by AEC for 132kV Transmission line, Khar. iv. Vetted documents of 132kV Grid Station /Transmission Line Khar i.e. design specifications and electrical drawings are yet to be submitted by AEC. 4.4 Water Sector
4.4.1 Munda Head-works:
Submission of an updated “Maintenance Plan” as a milestone is awaited from PaRRSA.
4.4.2 Amandara Headworks:
Submission of an updated “Maintenance Plan” as a milestone is awaited from PaRRSA.
4.4.3 Dhana Weir:
Removal of identified snags is pending since long.

5 Actions taken by HPK
HPK always suggested immediate and corrective actions to USAID and other stakeholders throughout the second year of IQA services in face of the problems encountered from time to time. A glimpse of actions taken is as below:
i. HPK timely communicated all issues raised on sites to USAID through weekly email updates, field visit reports and monthly progress reports. ii. HPK field staff enhanced their inputs where supervision consultant was not deployed. iii. HPK furnished investigation reports and suggested corrective measures as and when advised by USAID. iv. HPK timely communicated non-compliances at site to USAID which the executing agencies could not get rectified by the construction contractors. v. HPK assisted the implementing agencies in procurement of electrical equipment.

6 Observations/Recommendations
Following are some of the observations/recommendations for smooth execution of the project activities:
i. It has been noted that capacity of the design and supervision consultants M/s SHAZ and PePAC deployed on 57 flood damaged schools and Tehsil Level-Government Complex Kabal respectively, is not enough to timely respond to the construction related issues. This necessitated enhanced involvement of HPK staff in design review and discharge of IQA activities. ii. Cash flow problems of the construction contractors need to be addressed by the executing / implementing agencies for expedition of the ongoing works especially water supply schemes. iii. It is noted that the stipulated completion dates of majority of the sub-projects is not properly pursued by the executing agencies which causes unlimited delay in completion. It is recommended that the works schedules of each sub-project be strictly monitored and appropriate actions / measures be adopted. iv. For the upcoming sub-projects under KPRP and FIP the capacity of design and supervision consultant be properly scrutinized by the executing agencies before award of work. v. HPK recommends timely revision of PILs by the USAID.

USAID always wishes to uplift the socio-economic conditions of dwellers by improving connectivity and providing approach to local markets and basic
facilities with expansion and improvement of roads. Moreover, it plays a vital role in the enhancement of security of the area. One can say that DI Khan-Tank Road is a beacon of success for the people of Tank.
