We are equally committed to caring for families with the same love and sensitivity we provide to patients. We want to ensure that through the last moments in life for these patients, they are cared for and treated in a manner that their families would like them to be. After many years of being affiliated with Calvary, we are often asked one question: “What’s the secret to your reputation for patient care?” The answer begins and ends with our staff. They’re the ones who: • Work hand-in-hand with the families to ensure that we give their loved ones the care that they deserve; • Rush over to the homes of our home hospice patients in the middle of the night; Thomas J. Fahey, Jr., MD (left) and Frank A. 1 Jack Scalice Senior Lieutenant for the Security Department “Sometimes people do come to the front desk just to have somebody hear them out. They don’t have anybody to vent to about what they’re going through. Or what their loved one is going through. We listen and give feedback as to what we may have gone through with our own loved one who was very ill. There is a compassion component that you need to have even though you are in a security role. We are often the first person that somebody encounters coming into Calvary Hospital. It’s important for us to show them compassion as they are coming in. A lot of times they are very upset, they’re almost kind of lost from the shock of what’s going on with their loved one.”

Our Moral Compass Calvary’s core values are compassion, non-abandonment, dignity and love. Our adherence to these principles has helped make Calvary an international model of palliative and end-of-life care. We envelop our patients and their families in a “community of care.” We care for the whole person. We are much more than just a place that provides medical care. Whether we hug our patients and families frequently, stay by a patient’s side all night, or update family members regularly, Calvary employees understand the importance of what they do each and every day. We understand that this is the first and only time that our patients will be making this journey. Our staff makes this journey with patients and their families. However long we care for these patients, we know that it is our responsibility to make each day as comfortable as possible for them. We don’t abandon them when they need us most. Our commitment extends to providing trained pastoral care – from all faith traditions – tailored to each patient’s needs and to the needs of family members.

2 The Inpatient Experience Calvary cares for patients at our 200-bed facility in the Bronx, our 25-bed Brooklyn Satellite at NYU Lutheran, The Dawn Greene Hospice at Mary Manning Walsh Home in Manhattan, and Ozanam Hall in Queens. As soon as our patients are admitted, our goal is to get them settled and comfortable. Our patients are housed in private rooms with shared bathrooms. However there is one double room in the Brooklyn Campus. Telephone and TV service are provided at no charge. Within the first few hours, our patients meet their entire team of caregivers who will be with them during their stay. This includes their doctor, nurse, Calvary Care Technician, family care practitioner (social worker), spiritual care and dietitian. Our nursing department, comprised of registered nurses, nurse practitioners and Calvary Care Technicians, is an important reason why Calvary is world-renowned for its patient care. CCTs have in-depth education to maximize the patient care experience. Whether they have spent their entire career at Calvary or have come to us after working at other , our staff members want to be here caring for our patients. We encourage family members to decorate patients’ rooms with personal mementos that are meaningful and soothing to their loved ones. When we find out things that can make our patients feel more at home such as a favorite dessert, music, or religious customs, we often take the initiative to make them a part of the patients’ stay with us.

Kalmalee Golden, RN Registered Nurse “I have learned so much from my patients. I encourage patients and family members to tell me what they need. Nothing is too small to tell me. Being here for people during their end-of-life journey and helping them transition is very important to me.”

3 Hugh McLaren Senior Cook “Putting all these meals together for the patients. It’s a joy. I feel good knowing that what I’m doing for them will make them feel much better. I’m working here for 28 years and I have never called in sick. Rain, snow, shine, blizzard – I have to be there. There’s nothing that can stop me from being here for these patients.”

Jane Madio, RN Director of Clinical Operations for Calvary@Home “We are on call to speak with family members at all times and assist them through any crisis. One of the most important things we do with the Calvary@Home program is strive to keep our patients comfortable in the place they want to be. It often involves working with the families closely. We support our families and teach them how to care for their loved ones. It is their love and support that gets the patients where they are. We sit and listen to them and really hear what they have to say. It’s our mission to be able to help them with interventions and services that they need so they can provide the best care for their loved ones.”

Stefan Benkowski Calvary@Home Music Therapist “Music therapy can help the patients where they need it most. Music is so emotionally connective, just engaging with music with someone can bring out a lot of emotions in different ways – in both directions. And that’s part of what’s beautiful about music therapy.”

4 Patients select their meals from a daily menu. If requested, the Nutritional Services department is happy to accommodate special requests, at no extra charge. We realize how special certain foods are to our patients, since a dish may evoke happy and comforting memories. Unlike other hospitals, we encourage visitors at any time – including pets. People who wish to stay overnight may do so in the patient’s room or in several rooms on the premises – at no charge. We understand that when a loved one is near the end of life, making it easy for family and friends to visit and be present is a simple yet important thing to offer.

Our Brooklyn Campus Since opening our 25-bed Brooklyn Satellite in 2001, this Campus has cared for more than 5,000 inpatients and their families. In this vibrant borough, Calvary has also brought our unique care to thousands of home care and hospice patients; provided bereavement support to hundreds of adults, children and teenagers; and educated many people on how to obtain quality end-of-life care for themselves and their loved ones.

CalvaryCare SM in Your Home Calvary@Home is comprised of our comprehensive home care and hospice services to patients at home who are suffering from advanced cancer or other chronic and acute terminal illnesses, who live throughout the metropolitan area. • Home care is available in all five boroughs as well as Westchester, Rockland, Nassau and Putnam counties; hospice services in the Bronx, Brooklyn, Queens, and Manhattan, and Nassau, Westchester and Rockland counties. • Calvary partners with more than 25 nursing homes in Brooklyn, Manhattan, the Bronx and Westchester, Rockland, and Nassau counties, to provide hospice to their residents.

• At The Dawn Greene Hospice (TDGH), under the supervision of Calvary@Home, the Hospital provides short-term inpatient care for Manhattan residents. Since opening in 2013, TDGH has cared for more than 500 patients at this 10-bed facility. • Our newly established partnership with Ozanam Hall in Queens allows us to now offer an inpatient level of hospice care there. Under the skillful leadership of the Carmelite Sisters, Ozanam Hall provides Short-Term Rehabilitation and Skilled Geriatric Care for 432 residents. Calvary provides care in 6 beds at this time. Wherever we care for a Calvary@Home patient, the quality of care is the same top standard for which we are known across the country and around the globe.

Our Holistic Approach to Patient Care Social workers play an integral role in the lives of our patients and their families. To reflect this, Calvary renamed this department Family Care, and the Department’s caregivers are Family Care Practitioners. The Family Care Practitioner helps to identify what support the family needs and to ensure that they get it. This is the person family members are encouraged to contact whenever any issue arises. Every member of this department has a Master’s degree in

5 Social Work and advanced end-of-life training. They choose to be here, working with people in one of the most intimate and difficult times in the life of any family. For more than a decade, Calvary’s Family Care Center (FCC) at the Bronx Campus has served thousands of adults and children. It offers people a respite from the emotional turmoil that can exist on the patient floors, the opportunity to check email, read a newspaper, get a manicure or a cup of coffee, or simply gather their thoughts. No other hospital puts as much time and effort into caring for the family as Calvary does.

Spiritual Care Most hospitals employ just a handful of chaplains for their patients. Spiritual care is so valued at this Hospital, we employ 30 full-time chaplains. They are available seven days a week to care for the spiritual needs of patients and families. Chaplains lead worship services regularly at our interfaith chapel in the Bronx, visit the Brooklyn Campus and provide individual counseling. They also perform sacraments and other religious rituals. A Spanish-speaking ministry is available upon request. Spiritual care providers are also dedicated to caring for the spiritual needs of Calvary@Home patients, wherever they are receiving our care. For chaplains interested in professional education, Calvary offers an accredited 20-week, 400-hour Clinical Pastoral Education (CPE) Program, the country’s only one that provides students with hands-on experience with terminally ill patients.

Stephanie Mastropaolo, LMSW Family Care Center Coordinator “The social workers here provide emotional support for the family up on the unit. Down in the Family Care Center, I’m here as additional support for the families. I provide crisis intervention as well as emotional support. I think what’s beautiful about the work that we do here is that we do build relationships with families. They come back to visit us. While we see a lot of death and pain and sadness, we also see a lot of love. And the commitment of families to each other which is so beautiful. The staff here embody the mission of Calvary – to provide comfort, compassion, to make sure that people have dignity at the end of their life.”

6 T.A. John Thoonumkaparambil Lead Chaplain “You have any number of patients who need your help and your presence – unconditional, compassionate presence which is non-judgmental. They need you there with them as they struggle to cope with their lives and their pains and their struggles as well as their joys. Working with patients as well as the families has helped me to be a better person by being aware of what’s going on within me as I meet with them. It has helped me to be more , more giving, more generous with my time.”

The Jewish Community • Jewish patients and families can look forward to amenities such as: kosher food, a daily visit from one of three Rabbis, a Kosher Hospitality Room for families seeking a Kosher meal, dedicated Shabbos Lounge, Kabbalat Shabbat service, and celebrations of all major holidays. • Since 2012, Calvary has partnered with Yeshiva University/RIETS, National Association of Chevra Kadisha (NASCK) and other organizations to educate people about how to access excellent end-of-life care in accordance with Jewish law (halakha). • In 2016, the Hospital completed restoring a historic 135-year-old Torah scroll. It can now be used in religious services. This project appealed to people of all faiths and across the diverse Jewish community.

Bereavement Support Calvary first began offering bereavement support groups more than 25 years ago. Today, two dozen groups for adults, children and teens meet regularly in the Bronx, downtown Brooklyn and Manhattan. A Calvary connection is not necessary to participate in any of our free bereavement support groups. Calvary’s adult groups are organized by one’s relationship to the deceased. Thus, there are groups for bereaved spouses, adult children, parents, siblings, etc. There is even a group specifically for men and one conducted entirely in Spanish.

7 Our children’s bereavement program includes: • Precious Moments groups – for school-age children ages 6-10; • Tween group for children ages 10-12 and teen groups – for ages 13-17; and • Calvary’s Camp Compass®, a week-long summer day camp for grieving children and teens. Our partner, Kingsborough Community College, has been the home of Calvary’s Camp Compass® for the past several years. The Hospital shares its expertise with an annual 10-week bereavement course for funeral directors and mental health clinicians featuring various Calvary experts, and a four-week “Mentors through Mourning” program for school teachers and school guidance counselors. These courses take place at the Hospital’s Bronx Campus and at a convenient location in downtown Brooklyn.

Therapeutic Recreation Every day, there is a hospital activity planned especially for patients such as ceramics, flower arranging, bingo, or music. We also coordinate monthly visits by Juilliard students, a monthly patient party, and holiday barbecues on the Hospital terrace. For many patients, these activities may help them regain a sense of normalcy of what their lives used to be like before their illness progressed. We know that whatever activities they choose to attend, our patients get a great deal of joy and comfort from therapeutic recreation. At Calvary, we consider it our duty to help our patients enjoy what time they have left and to understand why we say that Calvary is the place “where life continues.”

Palliative Care Institute In 1985, the Hospital established the Palliative Care Institute to help educate health care professionals about how to provide the best end-of-life care to patients. Since then, it has welcomed thousands of physicians, nurses, social workers and other health care professionals from dozens of countries for visits ranging from a day to a few months. In 2005, the NIH’s National Cancer Institute designated Calvary “an international leader in palliative care” and invited us to participate in the Middle East Cancer Consortium (MECC). Comprised of the national ministries of health from Egypt, Turkey, Israel, Cyprus, Jordan, and the Palestinian Authority, MECC members have visited Calvary regularly over the past decade. Annual visitors to the PCI include health care professionals from coast to coast as well as 800 students, residents and fellows from throughout the greater New York area. The students include EMT members training to become paramedics for the Fire Department of the New York.

8 Raman Mohabir Bereavement Counselor “My biggest challenge is to normalize the feelings that grieving adults and children have – When will my life get back to normal? Why did this happen to me? Can I laugh and be happy? We have compassion for every person who comes to us, whether they’re patients, family members or someone without a direct connection to Calvary.”

Eddie Gorman Coordinator of Therapeutic Recreation “You’re challenged every day. Patients come in with a wide variety of things they want to say or do or feel. I want patients to know we are going through this journey with them. I want to hold their hand and let them know that at the end of it, we’re all here together. The mission here at Calvary is self-respect, dignity, non-abandonment, love. I think we are able to provide that through therapeutic recreation.”

Carmela Aiese, RN Hospice Nurse “You become a nurse to help people get better. I don’t think it’s the type of job you can go to day-to-day and not feel for what you’re doing. In Hospice we’re helping people to die gracefully, comfortably, peacefully. I guess the most satisfying thing about working at Calvary is getting to meet so many families, we really have the opportunity to touch a family, to make somebody’s life better, and make somebody’s death better. I think that the employees, the medical staff, all the employees at Calvary have a very high standard of commitment and compassion.”

9 Jodian Savizon Calvary Care Technician “O ur core principles of compassion, dignity, love, non-abandonment are human rights. My goal is to make every one of my patients feel like they’re the most important person.”

Nathalie Auguiste Housekeeping “I am very proud of my work here. Family members and staff are very grateful for all that I do. Family members often thank me for taking the time to listen and for caring.”

Volunteers Calvary’s reputation for the best end-of-life care would not be possible without the hundreds of dedicated people who volunteer regularly for us. Volunteers at the Bronx or Brooklyn Campuses often transport patients, assist at mealtime, sit with anxious patients, and help with bereavement calls to family and friends. Volunteers with Calvary@Home can often be found providing companionship to our patients and offering a respite to their caregivers. Many volunteers are fluent in many languages or have knowledge of different cultural practices. These skills are a huge plus for a place like Calvary that accepts patients of every religious and ethnic background.

10 Message from the Executive Medical Director It’s hard to believe that I have been with Calvary since 1963. In that time, a lot has changed in healthcare. I am relieved that one thing that has not changed, is the caliber of employee that we continue to attract. This year’s annual report spotlights employee all-stars across departments. I would like to share some of my favorite memories: James E. Cimino, MD When Jack first joined the Calvary staff in 1962, we were known as the “House of Calvary.” A year later, he asked me to come “just” to help out for a weekend. After Jack became Medical Director in 1968, we worked together over the next few decades to help make Calvary an international model of palliative care while revolutionizing the American hospice movement. Jack created our Palliative Care Institute in 1985. He taught the Calvary model of “high-touch” patient care to thousands of local medical students and post-doctoral fellows as well as health professionals from around Michael J. Brescia, MD the world. Until his death in 2010, Jack was a beloved figure to the entire staff, inspiring them all with his work ethic and caring. 12 Message from the Calvary Fund Chairman Dear Friends, It is a pleasure to thank you for your generous support of Calvary’s mission of compassionate, comprehensive care for those near the end of life, and care for the loved ones of our patients. As Chairman of the Calvary Fund, I am deeply moved, time and again, by the outpouring of support from donors who have been giving to Calvary for years on end, and also from those who gave for the first time in 2016. So many family members describe our caregivers as “angels.” Every one of our angels has a guardian watching over him or her, and you are that guardian. Whether you made an outright gift or included Calvary in your estate planning, your philanthropy has made a real difference for those who turn to us at a critical time in their lives. In 2016, the Calvary Fund Board of Directors began an effort that will be ongoing, to grow Calvary Hospital’s culture of philanthropy. The objective of this effort is to increase Calvary’s capacity to serve all who seek our care, and also to place Calvary at the forefront of education and resources relating to palliative Carlos M. Hernandez and end-of-life care throughout the metropolitan area, the nation, and the world. Through the Palliative Chairman, Calvary Fund, Inc. Care Institute, Calvary has gained recognition by the National Institutes of Health as an international leader in palliative care education, and we are proud to have clinicians from across the globe visit Calvary, for periods ranging from a few days to several months, to observe our care. As this effort continues, our friends can expect to hear more about CalvaryCareSM, and about the development of strategic plans that will increase our leadership in the fields of palliative and end-of-life care. We look forward to sharing additional information about how you can assist us in strengthening Calvary’s culture of philanthropy. As you read through this Annual Report, please know that your imprint is on every page, just as your impact is felt across Calvary’s entire care continuum. We could not provide our special care without you. You inspire Calvary’s leaders and caregivers, and you enable them to sustain Calvary as a place of hope – a place “where life continues.” Thank you.

A Calvary Fund Director and his wife made a challenge grant of $100,000, and our friends stepped up to the plate to match it. We raised a total of $247,261, bringing the value of the challenge to $347,261. Challenge grants can be tremendously effective in raising significant funds in a set period of time, and we are grateful to our Director for partnering with us in a way that more than doubled his and his wife’s initial gift. In addition to his generosity, Joe has advocated for our care in the halls of the New York State Assembly, which passed a landmark End-of- Life bill in 2000, which has assured access to our care for countless patients and families. In 2016, the Pat Cairo Family Foundation awarded a grant of $100,000 for Calvary’s general operations, and an additional grant of $50,000 to renovate lounges for our nursing staff. The RTS Family Foundation has also been a steward of Calvary’s wound care. In 2016, RTS renewed its support, in the amount of $190,000, for inpatient wound care. This grant includes support for nursing education, which will enable Calvary to transmit our expertise in wound care to new generations of Calvary nurses.
