Would you like to apply for a health resort, convalescent care or rehabilitation stay or apply for a grant for a health resort stay?

Obtain the application form (HV-KUR 1) from your regional or branch office, at the healthcare centres or download it from our website www.bvaeb.at and give it to your doctor, who will fill in the medical section. Please provide your personal details and send the form to your regional/provincial office or return it in person to one of the healthcare centres.

Please note: Every claim for extended medical treatment requires prior approval. Please ensure you wait for such approval.

Cost contribution: We are legally obligated to collect a contribution from patients to cover the costs of rehabilitation, health resort and convalescent care stays, which is payable before or at the start of the stay.

For rehabilitation stays, you pay the cost contribution for a maximum of 28 days per calendar year.

The amount of the cost contribution to be collected depends on the gross monthly income of the main insured person at the time of the application (also applies to co-insured relatives).

You do not pay a cost contribution if you are under the age of 18 or if you are exempt from prescription fees (exception: people who are exempt from prescription fees because they have reached the upper limit). 8

The Rehabilitationszentrum Engelsbad specialises in the following areas:

Rehabilitation in the event of musculoskeletal disorders • Stays after operations on joints and the spine • Chronic musculoskeletal complaints

The Rehabilitationszentrum Engelsbad has 124 single-occupancy rooms equipped with a shower, toilet, television set, wireless internet and telephone, and some with a balcony or access to a terrace. 19 of these rooms are suitable for wheelchair patients (electronic height-adjustable desk and the toilet can be electronically adjusted to the height of the patient etc.).

All of the facilities at the Rehabilitationszentrum Engelsbad are accessible for disabled patients. There is a cosy café in the foyer to pass the time. All of the dishes in the restaurant are freshly prepared and available as a buffet.

Medical facilities at the rehabilitation centre include functional areas for various non-invasive diagnostic and therapeutic procedures, x-rays, ultrasound and outpatient care, as well as treatment, exercise and training rooms in line with the required treatment for the indication. The Therapiezentrum Rosalienhof provides a comfortable and safe environment with its 126 single-occupancy rooms. The single rooms are equipped with a shower, toilet, radio, television set, wireless internet, telephone and balcony.

A breakfast buffet is provided every today, with a choice of three set menus, offered at midday and in the evening, which are prepared using fresh and often organic produce following the instructions of dieticians. The Therapiezentrum Justuspark offers bright, friendly and comfortable individual rooms for 100 patients set in a first-class hotel environment. Each room has its own shower, toilet, television set, wireless internet connection, telephone and balcony. Parking spaces are available in the underground car park.

The modern treatment programme, based on the latest scientific findings and which integrates an innovative exercise and nutrition concept, is the cornerstone of a successful stay at the Therapiezentrum Justuspark, encouraging both convalescence and a long-term change in lifestyle.

The Therapiezentrum Buchenberg has 145 accessible rooms. The eleven spacious, dual-occupancy rooms on the neurorehabilitation ward meet the technical standards of a hospital. Family members may accompany the patient to care for them and train them. The single rooms are equipped with a shower, toilet, television set, wireless internet connection, telephone and balcony. Dual-occupancy rooms are also available for patients with a companion or for couples. Our treatment objectives include, among others, capacity to work, independence, mobility, ability to interact socially and to pursue recreational activities, as well as long-term prevention. Mainly regionally sourced organic products are used here in keeping with the Grüne Haube concept.

You have access to baths that feature radon, a locally available resource, if this course of treatment is prescribed by a physician. The Bad Schallerbach healthcare facility offers active treatment and exercise training, passive applications and preventive measures. We also offer support in matters such as quitting smoking, nutritional advice and psychological care.

The Bad Schallerbach healthcare facility has 114 single-occupancy rooms with a balcony and comfortable beds, as well as five double-occupancy rooms for couples and patients with a companion. All rooms are equipped with a shower, toilet, television set, wireless internet and a room safe.

The restaurant offers a breakfast buffet with an organic food section, as well as three set lunch and dinner menu options. Its Grüne Haube certification for high-quality vegetarian meals means that regional products are used and prepared in a way that reflects their organic provenance.

To provide its medical care, the healthcare facility has a laboratory, ergome- try, echocardiography, duplex sonography of the neck vessels, resting ECG, 24-hour blood pressure measurement and 24-hour ECG as required by the indication.

All areas at the healthcare facility are accessible for disabled patients. All meals in the restaurant, certified under the Grüne Haube programme, are freshly prepared and offered in a buffet format.

Patients are invited to relax in numerous lounges, a library and two cafés. The leisure facilities on offer include an indoor swimming pool, saunas, infrared cabins, AlphaSphere loungers, cycling ergometer room, climbing room, bowling alley, disc golf facility, orienteering jogging trail and two beach volleyball courts. The Gesundheitszentrum Resilienzpark Sitzenberg focuses on optimising lifestyles on a sustainable basis, concentrating on the topics of health promotion and prevention. Inpatient health promotion stays are available to all actively employed persons. The stay is divided into a two-week basic stay and a follow-up week after three months.

The Gesundheitszentrum Resilienzpark Sitzenberg will open in autumn 2021!

Section two – Inpatient rehabilitation

Rehabilitation helps to restore the health of people who have been physi- cally or mentally affected by an accident or illness.

Rehabilitation stays are approved for periods of 22 or 29 days depending on the indication.

Your cost contribution In accordance with statutory provisions, you are required to pay a cost contribution for a maximum of 28 days per calendar year towards your rehabilitation stay. The amount of the contribution is calculated at the time the application is submitted. You always pay the cost contribution rate valid for the current year of your stay (see page 7).

Desired facilities You can indicate your desired location on the application form. We will be happy to comply with this request if possible. However, preference is given to owned facilities. We only approve contract facilities if such BVAEB-owned are at full capacity.

Transportation to the facility We will pay for an ambulance or taxi to the nearest rehabilitation facility if there is a medical reason to do so. Transportation for the journey to a rehabilitation facility must be approved by us before the start of the stay.

Interruption or early departure You may only interrupt or prematurely terminate your stay for important reasons (e.g. illness, unforeseeable events, appointments with the authorities) and only with the consent of a physician.

Travel costs If you are exempt from paying prescription fees during your stay, you can apply for travel expenses to the nearest rehabilitation facility to be reimbursed. However, you will not receive any travel expenses if you are exempt from paying prescription fees as a result of reaching the upper limit. Inpatient rehabilitation 19

Section four – Convalescent care stays

Stays for convalescence are available for patients to speed up recovery following severe illness or surgery.

Such stays are granted for a period of 22 days.

Your cost contribution You pay an additional amount for your convalescent care stay in accordance with the statutory provisions. The cost contribution amount is calculated at the time the application is submitted. You always pay the cost contribution rate valid for the current year for your stay (see page 7).

Transportation to the facility We will pay for an ambulance or taxi to the nearest facility if there is a medical reason to do so. Transportation for the journey to rehabilitation must be approved by us before the start of the stay.

Travel costs If you are exempt from paying prescription fees during your stay, you can apply for travel expenses to the nearest rehabilitation facility. However, you will not receive any travel expenses if you are exempt from paying prescription fees as the upper limit has been exceeded. Convalescent care stays 37

Section five – Health resort stays

A health resort stay requires a medical indication and a specific amount of time to have passed since the last stay.

Health resort stays are approved for a duration of 22 days.

Time limits Two health resort stays will be approved within five calendar years, whereby rehabilitation stays (except for follow-up treatment), Phase III outpatient rehabilitation programmes, treatment stay contributions and corresponding services of other social insurance companies are to be counted. You may only apply for a repeat health resort stay 18 months after the end of the last stay at the earliest.

Your cost contribution You pay an additional amount for your stay at the health resort in accordance with the statutory provisions. The amount of the contribution is calculated at the time the application is submitted. You always pay the cost contribution rate valid for the current year of your stay (see page 7).

Desired facility You can indicate your desired location on the application form. Preference will be given, however, to our own facilities. Only if these are full will we approve contract facilities.

Scope of coverage We only pay for the contractually regulated treatments prescribed by the health resort‘s physician. You must pay for all other additional services yourself.

Interruption or early departure You may only interrupt or prematurely terminate your stay for important reasons (e.g. illness, unforeseeable events, appointments with the authorities). You may not interrupt or prematurely terminate your stay due to events such as weddings, christenings, birthdays, graduations or attending events. Health resort stays 39

Section six – Gesundheitsvorsorge Aktiv (GVA)

The Gesundheitsvorsorge Aktiv (GVA) is an overhauled medical treatment stay programme when a patient suffers from one or more musculoskeletal disorders. The modular structure enables the patient to receive treatment tailored specifically to their individual needs.

Stays under the Gesundheitsvorsorge Aktiv programme are granted for a period of 22 days.

The same conditions and deadlines apply as for conventional treatment stays.

Splitting the stay Only active insured persons and recipients of rehabilitation benefit may apply for their stay to be split.

It is only possible to split the stay in the contract facilities.

You can, if desired, complete two weeks first and then the third week at a later date. However, the third week must be completed with six months of the first two weeks being completed.

Section seven – Contributions for health resort stays

Contributions for health resort stays are allowances paid towards the board and lodging expenses when undertaking treatment at specific health resorts recognised by BVAEB.

The medical indication and the time interval since the previous health resort stay are prerequisites for a contribution to the costs of any such stay.

Contributions to health resort treatment costs are approved for a period of 21 days (indication skin diseases: 28 days).

Time limits Within five calendar years, two contributions to the costs of a health resort stay may be approved. Rehabilitation stays (excluding follow-up treatment), Phase III outpatient rehabilitation programmes, health resort stays and corresponding benefits from other social insurance institutions are taken into account. You can apply for another health resort stay no earlier than 18 months after completion of your last stay.

Scope of services in Austria We reimburse the therapies administered and the costs of physicians at the health resort in accordance with the applicable tariff. If there is no contract between the local health resort and BVAEB, you have to pay for the therapies yourself in advance. You do not pay a treatment fee for the therapies you have completed and for the costs of physicians at the health resort.

Scope of services abroad We also approve health resort cost contributions for the following health resorts abroad. The contribution to the health resort costs already includes the therapy and physician costs at the health resort.

Travel expenses If you are exempt from the prescription fee during your stay, you can apply for the reimbursement of travel expenses to the nearest health resort or for domestic travel. However, you will not receive any reimbursement of travel expenses if you are exempt from paying prescription fees as a result of reaching the upper limit. 50 Contributions for health resort stays

Stays by the sea

Stays by the sea are approved for a duration of 28 days. The location by the sea may be chosen by you.

Indication: Skin diseases (psoriasis and neurodermatitis)

Indication: Respiratory diseases (Asthma bronchiale)

For this indication, we require a diagnosis from the lung specialist indicating lung function (spirometry or spirography) and an allergy test, in addition to the completed application form.

Convalescent stays for children and adolescents

Convalescent stays are granted to provide after-care support for children and adolescents up to the age of 18 following an operation or serious illness.

Convalescent stays are approved for a period of 21 days.

Therapy for children and adolescents

Indication: Metabolism (diabetes mellitus, adipositas), respiratory disorders (asthma), neurological and oncological diseases

These grants are provided for stays that take place demonstrably under medical supervision.

They are approved for a maximum of 28 days per calendar year. 54
