New physical and clearance information must be filled out every school year in order to be eligible to try out for any sport at Desert Vista High School. The physical must be done after March 1 of the previous school year to be accepted for current school year. All of the clearance information needs to be done once per school year. All equipment issued for each sport your athlete competes in must be turned in before your athlete will be allowed to try out/ or be issued equipment for his/her new sport. Our Equipment Manager is Jim Snow, and he can be reached at 480.706.7931. Lost equipment must be paid for in the Thunder Bookstore, and the receipt taken to Jim Snow to be released for new sport, or for transcript release in the event the student withdraws from Desert Vista HS.

The completed physical and clearance information is to be turned into the Thunder Athletics office before your student may be permitted to participate ANY sport on the Desert Vista HS campus. Please ensure that all of the steps in the checklist that is provided online on the Desert Vista School Fusion Page under the Athletics’ tab are completed. Once all steps are completed, your student athlete will be cleared for their sport. Without clearance, your student is NOT permitted to tryout out for his/her selected sport. If the student is not living with a natural parent, legal court appointed guardianship papers must be turned in with the physical and clearance information.

Regarding academic eligibility, the student must be enrolled in and passing 5 credit earning classes. An audit class does not count as a credit class. Seniors who are on track to graduate may be enrolled in 4 credit earning classes to be eligible. Grades of “F”, incomplete, or audit are considered non passing grades and will render a student ineligible to participate in competition until an academic grade check that follows the Tempe Union High School District’s guidelines indicate that the student is passing all current classes is completed by the Athletics Office. Wednesdays are the only day that grade checks are permitted to be conducted per district policy.

WELCOME TO THE ATHLETIC PROGRAM AT DESERT VISTA HIGH SCHOOL

At Desert Vista High School, we feel that our Athletic programs are among the best in the state, and we also feel that once you get involved in the programs you will agree. Our coaching staff consists of highly qualified professional people who are available to provide the best opportunity possible for each and every student Athlete. We are especially proud of the good reputation we enjoy throughout the state, a reputation based not only on our win and loss record, but also by the manner in which our teams and team members conduct themselves on and off the field. This is a reputation that we intend to keep. In order to accomplish this, it is essential that our Athletes conduct themselves in a manner that will bring credit to their families, the program, and the school at all times. Athletes, remember that you are looked upon by most of our student body to provide good positive leadership. If you do, you will always be able to remember that you participated with pride. The following pages will note the minimum guidelines that have been set forth for our student Athletes. Please become familiar with them.

ATHLETE RESPONSIBILITIES AND REQUIREMENTS

AIA Rules of Eligibility: The Athlete must fulfill all AIA requirements for eligibility. 1. No student shall be eligible to be issued equipment, to engage in practice, or to compete until there is a physical examination form on file with the Athletic Office. This form states, in the opinion of the examining physician, the student is fully able to compete in athletic contests. 2. An Athlete shall be enrolled in a minimum of five credit bearing classes the first 6 semesters, and a minimum of four credit bearing classes the 7th and 8th semesters. 3. An Athlete shall pass all classes the preceding 9 weeks/semester to be eligible for interscholastic activities. 4. An Athlete shall be enrolled no later 14th official school day of the semester to be eligible 5. An Athlete enrolled in a 4-year high school shall have the opportunity to compete in not more than four seasons in each sport, and only eight consecutive semesters after he/she first enrolls in the 9th grade. The 7th and 8th semester must be consecutive. 6. An Athlete shall be ineligible if he/she becomes 19 years on age on or before September 1st of that school year. A certified copy of the birth certificate shall be required of all contestants participating in a sport. 7. An Athlete, whether and adult by terms of age or not, is privileged with eligibility only at the school in the attendance area in which his/her parent(s) or legally appointed guardian(s) are domiciled. 8. An Athlete who transfers from one high school to another high school without an accompanying change of domicile of his/her parent(s) or legally appointed guardian(s) is ineligible to participate in interscholastic activities in the school to which he/she transfers for one calendar year from the date of transfer, except in activities in which he/she did not participate in one year prior to the transfer, or until his/her parents(s) or legally appointed guardian(s) become domiciled in the school attendance zone. (please see Athletic Director for possible exceptions) 9. An Athlete who is a member of a school team shall not practice or compete for/or with any other group, club, organization, association in that same sport during the interscholastic season of competition of the sport. 10. An Athlete shall not compete for money or other monetary compensation. 11. An Athlete shall participate and always have participated under his/her own name. 12. An Athlete who did not establish initial eligibility on the Desert Vista campus, and transfers in at a later date (after 1st day of their freshman school year), may not have received coaching in their sport by any coach associated with the Desert Vista campus.

Comply with the Tempe Union High School District No Pass/No Play Policy:

1. A student shall be required to earn a passing grade in all courses taken to be eligible to participate in athletics 2. A student receiving an incomplete for the 9 week/semester grading period may retain eligibility immediately upon satisfactory completion of all course work. A student receiving a failing grade for the 9 week/semester grading period may regain eligibility at the grade check intervals established by the district for eligibility checks. 3. A student who receives a failing grade for the 9 week/semester grading period will be grade checked by the Athletics Office, and communication to the coach will be made via e-mail, or other written communication that the student is now eligible for competition. 4. No student will be declared eligible before or after Wednesday unless he/she had an excused absence on the designated grade check Wednesday, and a grade check is completed by the Athletics Office the day the student returns to campus.

Rules and Regulations for Desert Vista: The following list of general rules and guidelines is for Desert Vista Athletes. More specific rules and regulations may be set forth by the individual coaches.

Team Classification: 1. Most sports have various levels of competition based upon grade level. Only freshman may compete on freshman teams. However, a freshman athlete may compete on a Junior Varsity or Varsity team. Senior student athletes may only compete on the Varsity level. 2. Each Athlete will be assigned to a team by his/her coach, and he/she may be moved to a higher level of competition at the discretion of the head coach.

Injuries: 1. All injuries must be reported to the coach and/or trainer immediately. 2. An accident report should be completed by the trainer. 3. If you have purchased the supplemental insurance from the insurance provider that the school provides brochures for, and you do not report an injury or accident to the trainer/coach at the time of the accident, it may complicate the payment of your medical bills. 4. Once an injury has occurred, the school trainer has final say in student participation again in the activity.

Practice/Games: 1. Team practice is generally scheduled for every school day and team members are expected to be on time for practice. 2. In case of an absence from practice/game you should contact the coach. 3. To participate in Athletic activities you are required to attend all classes. Exception: Circumstances beyond the control of the student as verified by the parents/guardians or school, and acceptable to the coach. 4. Suspended Athletes shall not practice/participate/compete during the time of their suspension. 5. Team members who have a regular Physical Education class will be expected to participate in all class work as per the teacher.

Athletic Trips: 1. All trips for athletic contests shall be made on school transportation. Should a parent/guardian desire to take their son/ daughter home from a contest, they must submit an Administration authorized Transportation Waiver prior to the contest. The parent/guardian should also speak with the coach after the contest for permission. 2. Behavior on the bus during the trip should be such that reflects positively on Desert Vista High School. 3. Removal of towels or any other equipment from the host school is inexcusable and grounds for immediate removal from the team 4. Athletes should remember that they are representing Desert Vista and should exhibit good moral judgment and appropriate dress at all times. 5. Cleats and or spikes shall not be worn on the bus or van. 6. Students who are ineligible for any reason may not travel with the team for away competition.

Equipment/Uniforms: 1. School uniforms/equipment is checked out to you to be used or worn in competition only. Uniforms should not be worn for personal use. 2. Normal wear and tear is expected; however, you will be held responsible for unusual abuse or loss of equipment. 3. Athletes shall turn in all equipment issued to them immediately after completion of their sports season or after dropping from the team, whichever comes first. 4. Athletes shall pay for all items which are not returned. Their money may be refunded in the event the equipment is returned at a later date with the receipt proving return of said items from the Equipment Manager. 5. No Athlete may check out equipment for another sport until he/she has been cleared from the previous sport both by the Athletics Office and the Equipment Manager.

On Campus Appearance/Behavior: 1. Appearance: It is the belief of the Administration and the Athletic Department that pride, dedication, morale, team unity, safety and community relations are influenced by the general appearance of our Athletes. With current styles and the above in mind, the coaches will be responsible for setting the guidelines for the Athletes. a. Athletes shall not wear clothes with obscene or objectionable writing or drawings on them. b. Team dress on game days will be determined by the Head Coach of each sport. 2. Conduct: We look upon our Athletes as a special group who, more than any other single group, set the tone for the general behavior of the student body. It is the Athlete’s responsibility to conduct themselves in a manner that will bring credit

Sportsmanship: 1. Field and court procedures will be specified by each coach. All Athletes are expected to obey these regulations and procedures, and to conduct themselves in a commendable manner. 2. Officials in a game are there to enforce the rules of the game/contest and should be treated with respect. 3. Any unsportsmanlike behavior which results in an ejection from a game/contest shall result in the Athlete being ineligible to participate in the next game/contest.

Terminate a Sport: 1. Any Athlete who tries out for a sport and withdraws from the team before the team has completed its second full week of competition does not jeopardize himself/herself for the consecutive season. 2. If an Athlete withdraws from team after that time, the Athlete is not eligible for the next sport until the team he/she was on has completed its season, without prior approval from the Athletics office. 3. If the Head Coach of the team, the Athletic Director, and the Athlete all agree that the Athlete may withdraw, there is not a penalty. 4. If an Athlete is terminated by the coach for reasons other than disciplinary, there is not a penalty.

Discipline and Due Process: 1. At all times, Athletes are expected to follow training rules as outlined by their coaches and the Athletic Department. Smoking, drinking, association with or use of drugs, and poor citizenship are absolutely unacceptable, and are grounds for immediate disciplinary action. 2. Discipline for Athletes will be the same as for all other students at Desert Vista. 3. The coaches shall be informed by the Assistant Principal in charge of discipline. 4. If in the event a rule or policy is not followed, disciplinary action will be taken against that Athlete, and due process procedures will be followed.

Athletic Training Rules-Tobacco/Drug/Alcohol: Tobacco Policy-Any student/athlete discovered to be in possession of, or using tobacco product during their season or sport shall be subjected to disciplinary action. This is separate than that action that may occur if the school administration has to also determine discipline in the event of these issues taking place on school/district property.

REQUIREMENTS TO GO OUT FOR A SPORT

To become eligible to participate in interscholastic sports, the following must be completed and on file in the Athletic Office. 1. A.I.A. Pre-physical and Physical forms: Athletic Participation and Health History Form which may be obtained in the Athletic Office and includes the following: a. Athlete directory information b. Annual Health questionnaire c. Current physical, signed by the examining physician and dated after March 1st of the previous school year. d. Emergency medical information e. Both Pre-Physical and Physical must be signed/stamped by authorizing physician 2. Birth Certificate-the Athlete’s original or certified copy of birth certificate shall be presented to the Athletics Office. A co py will be made on campus, and original returned to parent/guardian. This is for new athletes who transfer in to Desert Vista after the start of their freshman year, and all freshman athletes. Returning athletes do not need to do this, it will be retained on file in Athletics. 3. Participation Fee-a $50 participation fee is charged for each sport and must be paid in the Bookstore and a receipt presented to the Athletics office before a clearance for activity will be issued. 4. Emergency Consent/Acknowledgement Sheet. This includes the following: a. Parent/guardian and student signatures b. Insurance information. Every athlete must have insurance to participate c. Includes emergency contact information d. Acceptance of expenses should injury occur outside of insurance coverage e. Acknowledges repercussions for any falsification on any of the submitted documents for athletic clearance f. Parent current contact information g. Allows emergency care and/or trainer treatment should it be necessary for the athlete. h. Includes student health history for emergency care purposes 5. Sports Risk Video-Athletes must view the Sports Risk Video once per school year before being cleared for their first sport of the new school year, and acknowledge viewing of video on the Athletic Participation Screening Form. 6. Before Athletes can compete, or be eligible in any way, their most recent official grades from the previous 9 weeks/semester grading period will be checked. All grades must be passing grades. Also, Athlete must be enrolled in at least 5 credit bearing classes to be eligible. (exceptions for Seniors only-a Senior Athlete in the 7th and 8th semester meeting all graduation requirements will be allowed to be enrolled in a minimum of 4 credit bearing classes). 7. Athletes must live within the Desert Vista attendance boundaries, or have a boundary exception form from the Registration Office on file to be able to participate in athletics at Desert Vista. 8. Completion of A.I.A. BrainBook course, certification of completion of course with a 80% or better must be turned in to the Athletic office. 9. Signature on A.I.A. Annual Statement and Acknowledgement Form, form turned in to the Athletic Office. 10. Proof of insurance (copy of insurance coverage) must be submitted with all forms 11. District Athletic Participation Screening Form, which includes verification of student viewing video and receipt of this DVHS Athletic Handbook, and parent signature. 12. District required thermometry form regarding parent decision on overheating protocol must be submitted for first-time athletes.

NOTE: DO NOT GIVE ANY OF THE FORMS REQUIRED FOR ATHLETIC CLEARANCE TO ANYONE OTHER THAN THE ATHLETICS OFFICE. DO NOT GIVE TO COACH-THEY WILL NOT BE RESPONSIBLE FOR ANY FORMS THAT ARE HANDED IN TO THEM INSTEAD OF DIRECTLY TO THE ATHLETICS OFFICE.

WHAT TO DO TO GO OUT FOR ANOTHER SPORT AFTER SEASON IS COMPLETE

1. Turn in all equipment from the previous sport to the Equipment Manager. 2. Pay the $50 activity fee at the Bookstore and present to the Athletics Office for the new sport.

2015-16 ACTIVITY FEE SCHEDULE To help offset the M&O expense of providing extra-curricular student activities in the Tempe Union High School District, a $50 participation fee will be charged for the activities listed below. • Activity fees will be charged to participants in all activities and athletics for which a sponsor or coach is paid a stipend for that activity. • Fees will be $50 per activity with a maximum of $150 for any one student and no family maximum. • Principals will be responsible to determine if/when financial assistance is needed. Financial Assistance forms may be picked up in the Administration Office at each school. • Students must pay their activity fee in the bookstore prior to official participation in any activity. • Refunds can be approved by site administration only. • Tax credit may be used to pay activity fees. Under state law ARS 4-1089.01, taxpayers can receive a dollar-for-dollar tax credit when they contribute up to $200 per year ($400 for married couples filing a joint return) to extracurricular activities in public schools. The $50 Activity Fee receipt can be used for tax-credit purposes, however it is a separate account from any tax-credit donation accounts.
