Post Summary: This new specialty doctor post arises from the expansion of intermediate care and rehabilitation facilities at the newly opened East Lothian Community Hospital in Haddington, less than 20 miles east of Edinburgh. There will also be opportunities to contribute to the Hospital at Home team and support of Psychiatry of Old Age organic and functional inpatient service, with the possibility of supporting the Parkinson’s disease clinic. The post is an opportunity to help shape new and expanding services resulting from the Haddington hospital redevelopment. The successful candidate will join the existing senior medical team of 2 Consultants in geriatric medicine, Consultant in acute medicine and associate specialist, specialty doctor and a GP with a specialist interest who support the inpatient MOE and Hospital at Home teams in East Lothian, as part of a coordinated approach for East Lothian patients between acute services in Edinburgh and assessment and rehabilitation in East Lothian. Those services are supported by 4 junior doctors and a full multidisciplinary team including an expanding team of advanced nurse practitioners. The main duties of the post will be to provide medical support for and leadership of the multidisciplinary team for intermediate care/step down beds in ELCH (a mixture of general MOE patients but also some orthopaedic and stroke rehabilitation), supporting the junior medical staff and to provide medical input to the old age psychiatry inpatient ward (currently complex dementia patients, but with plans to create a small number of beds for patients with functional illness). Some input to the maturing Hospital at Home service will also be part of the role, providing acute care and multidisciplinary assessment in the community for frail elderly patients at risk of admission, or those stepped down from acute services. There may be an opportunity to develop an interest in Parkinsonian disorders in the elderly and contribute to the specialist clinic for those patients. The East Lothian Hospital at Home service provides 8am to 8pm cover by Advanced Nurse Practitioners of a cohort of patients managed in the community as an alternative to admission (and to support early discharge) with new referrals generally assessed 9am-5pm Monday to Friday. Hospital at Home is supported by a wider multidisciplinary team in the now integrated health and social care service. The successful candidate will be part of the team providing new assessments and ongoing review of these patients. There are plans to extend the core hours of Hospital at Home in the future. Departmental Information Overview of East Lothian Comprehensive Geriatric and General Medicine Service We currently operate as a needs-based comprehensive geriatric and general medical service comprising inpatient and hospital at home acute assessment, rehabilitation (including geriatric orthopaedic rehabilitation), stepdown, continuing care, and outpatients. Given the relative rurality of East Lothian and distance from central services we also provide all age terminal care and rehabilitation on a case-by-case basis. The overarching aim is to provide a service in East Lothian for East Lothian patients. We operate a rapid referral review and “repatriation” service to all specialties in the Royal Infirmary and Western General Hospital to facilitate transfer of East Lothian patients admitted to Edinburgh hospitals to either our inpatient or outpatient services. We pride ourselves on a close working relationship and excellent two-way communication with local GPs. East Lothian Community Hospital and associated services ELCH in Haddington is the base hospital of the East Lothian service, which has recently undergone a major rebuild and expansion project, opening in November 2019 with additional beds and facilities. Inpatient beds are as follows: 24 acute assessment and rehabilitation care of the elderly beds in Ward 3 (divided into wards a, b and c with two female ward areas and one male ward area). Approximately 40% of these beds are designated geriatric orthopaedic rehabilitation beds. 20 continuing NHS care beds in Ward 1 which are used for NHS continuing care (HBCCC) and terminal care. 24 stepdown beds for stepdown rehabilitation/intermediate care in Wards 4 and 5. Current junior medical staff cover of ELCH wards is 8am to 9pm Mon-Fri, with out of hours cover being provided by the senior nurse on the ELCH site and the RIE Hospital at Night team overnight, and senior nurse and ANP cover during the day Saturday and Sunday. There are excellent outpatient facilities on the ELCH site. Visiting consultants provide specialist clinics including cardiology, rheumatology, psychiatry, ophthalmology, urology, dermatology, ENT, general surgery, gynaecology, diabetes and orthopaedics. Dr Morrison currently offers combined general medical and geriatric medicine clinic and Parkinsonian Disorders Clinic at ELCH. http://careers.nhslothian.scot.nhs.uk At the community hospitals in North Berwick (the Edington) and Dunbar (Belhaven) there are approximately 20 (in total) GP run beds, which are not an integrated part of the East Lothian MOE service but there is a close working relationship between the units. At ELCH an X-ray department provides a plain film and ultrasound service Monday to Friday 9am to 5pm and an ECG department providing access to ECG/24 hour BP and ECG/Echo/spirometry. East Lothian Hospital at Home Service Since 2015 this team has offered an acute response service for frail elderly (and some under 65s) with medical and functional problems, where acute admission to hospital would otherwise have been actively considered.
