Introduction from CEO The engagement strategy which is helping to shape the ‘Culture Fit for the Future’ has been moving ahead. Fiona Hogg will be giving a detailed update on our progress with our Culture Programme later in the agenda. I am pleased with the progress which is being made, it’s a long-term piece of work which needs careful research and planning to make sure we understand the problems we need to address. There are some key milestones being achieved and presented at this Board meeting. A governance structure is in place and our revised plans are ready to be agreed and rolled out. Many of the actions planned have been shaped by our ongoing engagement with the Board and with our colleagues across North Highland and Argyll & Bute and I’m delighted that our impending review in Argyll & Bute is going to provide further valuable insights and information. In terms of the cost improvement programme, we continue to make good progress, with the current level of identified opportunities valued at £29M which when adjusted for the likelihood of delivery reduces to £22M. Of particular note is that 80% of the forecast savings are recurrent so the savings gain will benefit future financial years. Workstreams are ensuring the remaining plans in this financial year are approved, that ideas are progressed to plans and that changes already implemented go on to deliver the expected savings.

Summary On 29th November 2016 NHS Highland Board approved the recommendations of ‘‘Caithness Maternity and Neonatal Services – A Public Health Review’ including the move to set up midwife-led Community Maternity Unit (CMU) in Caithness and the designation changed on Monday 4th December 2016. An interim report on progress was presented to the board in May 2017. This paper provides an update on the arrangements as of the 1st November 2017, including a summary of clinical activity. 1.1 Number of Births. Transfers and Retrievals Over the eleven month period from 1st December 2016 to 31st October 2017 there have been total of 218 births to women resident in Caithness. Of these, 199 births were in Raigmore Hospital, and 18 (9%) normal births in the CMU, with one planned delivery elsewhere Transfers and Retrievals are lower than before the change in designation and this is an important indicator that the new arrangements are working as planned.

Migdale Hospital A Case Study 1 Migdale Hospital Overview The replacement of the Migdale Hospital in Bonar Bridge is a good example of a rural community hospital and sets a helpful precedent for community facilities in remote locations. The product of 15 years work, the project involved considerable consultation with community groups, local councillors, staff, patients, and patient representatives before being constructed and completed in June 2011. The new facility replaces an older hospital building, originally designed as a Victorian ‘Poorhouse’ to support and provide housing for the dependent or needy. A decision was made to sell the original hospital which had become unfit for purpose. This was in part due to restrictions caused by the narrow corridors, 4 and 5 bed wards, inadequate sanitary facilities and split-level Front elevation of Migdale hospital, accommodation, all of which affected the use by long-stay elderly patients. overlooking the Kyle of Sutherland Retention would require a total refurbishment which was deemed unviable. Image credit: John Paul Photography It was considered that a new hospital could more easily provide better integrated care and single-bed rooms, in line with Scottish Government Health and Social Care Directorate (SGHSCD) policy. The new Migdale hospital is a 22-bed community health unit serving the north and west areas of NHS Highland. The hospital has two wards: an Older Adult Mental Health Assessment Unit (Strathy) and a GP Acute Unit (Kyelsku). A number of outpatient services are also provided including Physiotherapy, Occupational Therapy, Audiology, Rehabilitation Classes, and Memory Clinics. Built on a steeply sloping site in Bonar Bridge, the hospital lies close to the main travel routes between Inverness and the North and West of Scotland.

PE1845/C Caithness Health Action Team submission of 11 January 2021 Caithness Health Action Team (CHAT) support this call for an independent Agency designated to mediate healthcare issues within rural and remote Scotland. Initially a response to downgrading our maternity model we now support local health services for Caithness under threat from what we consider to be stealthy centralisation under various disguises, with continued broken promises, a non-executive Board that has been poorly informed and lack of corporate memory. Over years many patients’ emotional accounts of suffering the impacts of an increasingly centralised health care service to Raigmore have motivated CHAT. Decisions made by NHS Highland (NHSH) have been based on strategic plans for NHSH, a one-size-fits-all service, inappropriate for the rural and remote communities. Groups such as Women for Independence have repeatedly plead for the Scottish Government to review women’s services in Caithness. NHS Highland’s policy of standardising and centralising services has diminished Caithness maternity model. NHS Highland justifies it as, ‘now in line with our other mainland CMUs’. Since 2016, 85% of Caithness mothers give birth over 100 miles away, with many interventions to the birth process and mothers and families suffering associated risk, inconvenience and expense. At the same time, our Orkney neighbours had a maternity review, but the Health Board mediated to strengthen their maternity model to make it safer and community orientated. The majority of their mothers now deliver safely within their own community. A damning NHSH report (2016) focussed on negatives of having an Obstetric led maternity model for Caithness but failed to consider the risks and inconvenience of travel, especially on advanced labour and frequent road closure.

It’s a great place to bring up a family with good local schools and reasonable property prices. Oban provides the advantage of a high quality of life in a rural setting with access to the main centres of Glasgow, Stirling and Perth all within 2½ hours travel time.
