Applications are invited for the post of Speciality Doctor which is available from April 2021 on a fixed-term basis to cover for a maternity leave. You will be based at Raigmore Hospital, and will work alongside Consultant and other middle grade colleagues.

Applicants must have completed a minimum of 2 years full time postgraduate training in the specialty (or equivalent), and have experience of Middle Grade on call.

Your duties will involve general Paediatrics and Neonates. Whilst this is a non training post, you will be encouraged to develop new skills and maintain existing special skills and interests.

You will be expected to take part in the middle grade resident on call rota.

INFORMATION ABOUT THE PAEDIATRIC DEPARTMENT

An opportunity has arisen for a fixed term contract to a suitably qualified Specialty Doctor Paediatrician from April 2021 to cover for a maternity leave.

We see this as an opportunity to continue to develop as a centre of excellence in secondary paediatric care. We are looking for individuals who would relish the opportunities and challenges of delivering high quality care across the Highland Region. The role is within the Paediatric Service within NHS Highland, participating in a range of general paediatric outpatient clinics and hotweek/oncall responsibilities for the Highland Children’s unit which includes level 2 neonatal cover. We are also looking to develop certain sub specialist services.

The establishment currently comprises 10 hospital consultant paediatricians (5 WTE, 4 part-time 2.9 WTE), 1 combined post 0.9wte) W.T.E in Hospital and community paediatrics and 5 (3.7 WTE) community consultant paediatricians (the latter do not take part in out of hours work).

Our team of paediatricians are supported professionally by Regional and National Managed Clinical Networks. Local Lead clinicians are supported by visiting specialist clinics in Neurology, Oncology, Gastroenterology, Nephrology, Rheumatology, Genetics, Endocrinology, Urology, Cardiology, Complex Respiratory and HIV/Immunology, and metabolic medicine. Raigmore Hospital is part of the North of Scotland Neonatal Network.

Comprehensive local guidelines (with links to national guidance) for common neonatal, paediatric and neurodevelopmental problems are available on the hospital intranet.

NEONATOLOGY Raigmore Hospital has a modern well-equipped Level 2 Neonatal Unit with two intensive care cots and 14 special care cots and we act as a referral centre for the maternity units in Wick, Fort William, Stornoway and Elgin. Wherever possible, neonates under 28 weeks gestation are transferred in-utero. The Neonatal Unit functions as part of the Northern Neonatal MCN, and also enjoys significant support from the Northern Neonatal Retrieval Service. The neonatal nurse complement includes two Advanced Neonatal Nurse Practitioners. The network consists of Aberdeen and Dundee as tertiary centres.

GENERAL PAEDIATRICS

We provide general paediatric clinics in Raigmore Hospital, Fort William, Wick, Thurso, Broadford and Portree Hospitals on Skye, and Golspie. This gives staff an opportunity to experience the challenges of rural medicine and explore the stunning scenery of the Highland Region. Specialist clinics include regular Oncology, Epilepsy and Cystic Fibrosis clinics held within the dedicated paediatric outpatient suite and weekly Diabetes Clinics in the new Diabetes Centre. Associate specialists in community paediatrics are based throughout the Highlands and out patient clinics are held in various locations.

The Children’s Unit has recently undergone an exciting refurbishment, thanks to enthusiastic fundraising by the local community, supported by the ARCHIE foundation. The children’s ward has 32 in-patient beds, including eight cubicles and a four bedded high dependency area and 6 day case beds.

In addition to the planned consultant staffing level of 10 general paediatricians (8.4 WTE), the post is supported by three SASG (2.6 wte), five specialty trainees in paediatrics, four GPSTs, two FY2s, two ANNPs and two advanced paediatric nurse practitioners. We have a robust middle grade rota , consultantsare on call 24/24 but do not provide residential on-call cover.

CLINICAL ACTIVITY Each year approximately 1300 new patients and 4100 return patients are seen in the Medical Paediatric clinics at Raigmore and peripheral hospitals in Highland.

There are approximately 1750 medical paediatric admissions to Children's ward each year and a further 350 day cases. There are around 350 infants admitted to SCBU each year. The SCBU undertakes long-term neonatal intensive care, and has facilities to ventilate up to four infants at a time. Babies under 28 weeks gestation are transferred to a tertiary neonatal unit, ideally in-utero or following delivery by a regional neonatal transport team. Annual delivery rate is around 2100.

There are regular Audit Meetings including Morbidity and Mortality meetings within the Department of Paediatrics and close links with the Consultant Obstetricians, Community Paediatricians and Consultants in the Department of Child & Family Psychiatry.

COMMUNITY CHILD HEALTH

The Community Paediatric department is based at Morven House in the grounds of Raigmore Hospital, Inverness. Other bases for the Community Paediatric Department are in Invergordon (Ross-shire), Golspie (), Wick (), Portree (Skye & Lochalsh) and Fort William (). Out patient clinics are held throughout the Highlands primarily at the bases above and also within special education schools.

There is a consultant-led service in Community Paediatrics, which will soon be staffed by five Consultant Community Paediatricians. The community consultants are supported by seven (5.0WTE) experienced associate specialists, who also are leads for specialist interests within the department, including adoption and fostering, hearing impairment, learning disability and autism. The service is currently working closely with the Highland Council to deliver a new Neurodevelopmental Assessment Service, which is delivering an improved multidisciplinary/multi agency pathway

The Birnie Child Development Centre adjacent to Morven House provides an excellent clinical facility and opportunity for close working within the Child Development Teams for Inverness and localities within a 30-mile radius. Children and Adolescent Mental Health Services (CAMHS) are co-located in the same building allowing for enhanced communication and joint working.

The department remains part of the NHS despite recent integration of NHS and Council services. Work on pathways and guidelines are well established to assist with this reconfiguration and also recruitment to locality teams to support visiting paediatricians.

CAMHS (Child Psychiatry and Child Psychology services)

There is an integrated team with Consultant Child Psychiatrists, Psychiatric nurses and Child Psychologists based on the Raigmore site. There is currently no overnight or weekend cover. CAMHS are also involved in the new Neurodevelopmental Assessment Service.

Teaching The Centre for Health Sciences is a new purpose built facility on the Raigmore site, encompassing a remarkable collaboration between NHS Highland, the , the University of the Highlands and Islands and Lifescan. It incorporates state of the art teaching facilities and provides excellent personal opportunities for research.

Members of the paediatric department play an important role in the undergraduate programme of the Department of Child Health at the . The planning of the teaching programme and supervision of students is facilitated by one of our SASG staff, and all consultants share in the department's formal teaching programme. Commitments currently include:

 Five-week attachments of 4th year Aberdeen students  Seven-week attachments of 5th year Aberdeen students and elective students from elsewhere  FY2s and STs, clinical and educational supervision, contribution to weekly seminars, monthly audit sessions and MRCPCH/ DCH tuition.

The main duties and responsibilities of your post include:

The postholder will be responsible to the Consultants in the Department – currently Dr T Reddy, Dr R El-Adle, Dr D Goudie, Dr A Webb, Dr V Franklin, Dr R Aurora, Dr A Webb, Dr S Ghaydda, Dr Van Der Heide, Dr S Henderson, Dr L Ferguson and Dr A Hosenie.

Daytime duties will comprise ward cover, ambulatory care, ward rounds, outpatient clinics and teaching according to the needs of the service and the candidates training needs at that time. The postholder will take part in the shift pattern and out of hours work including covering labour ward, postnatal wards and the Neonatal Unit.

The post holder’s responsibilities are expected to include:–

1. Possible contribution to rapid access clinics, depending on experience.

2. To assist and supervise the work of the FY2’s and GPST’s in Children’s Ward and Special Care Baby Unit.

3. To manage acute admissions.

4. To undertake ward rounds in Paediatric ward / SCBU independently and with Consultants.

5. To carry out Out-Patient Clinics at Raigmore Hospital.

6. To participate in the medical student and midwife training programmes

7. To take part in the Clinical Meetings and Clinical Audit Programme of the Department.

8. So far as is consistent with the proper discharge of the above duties, to undertake to deputise from time to time for absent colleagues

9. To share the work load with middle grade staff in completing discharge summaries in SCBU and also Immediate Discharge Letters in Paediatric ward

10. To hold bleeps of FY2’s, GPST’s and StR’s /SpR’s staff to provide bleep free teaching period from time to time

11. To undertake, exceptionally, to perform duties in occasional emergencies and unforeseen circumstances

12. Exceptionally to be available for such irregular commitments outside normal roster duties as are essential for continuity of patient care.

Your salary will be credited monthly, in arrears, at 1/12th of the annual rate to an account at a bank/building society of your choice on the 27th of each month.
