Introduction

Applications are invited for the post of Consultant General Surgeon, based at Raigmore Hospital, Inverness. This is a new Consultant post that will build upon the existing surgical clinical network that covers the NHS Highland area, and will result in closer clinical links between the hospitals based in Inverness, Fort William and Wick. This appointment will enhance the delivery of emergency surgical care on the Raigmore site, as well as contributing to the delivery of elective surgery for patients from the wider NHS Highland Board area.

The successful applicant will have a broad experience of general surgery that complements the subspecialty interests that currently exist within the wider surgical department. It is our vision that these skills will both support and enhance the surgical services delivered in Raigmore Hospital, as well as enhancing the elective service delivered by Caithness General Hospital in Wick and the , Fort William.

The successful applicant will be a General Surgeon with gastrointestinal experience, and will contribute to the emergency on call rota at Raigmore Hospital (outlined in job plan, 1 in 11 with this post). Familiarity with the management of trauma is essential. The applicant will possess laparoscopy and endoscopy skills appropriate for a forward-looking General Surgical service. They would also be expected to have a sub-specialty interest complimentary to those currently existing within the wider NHS Highland surgical network. All members of the general surgical team deal with emergency paediatric admissions, supported by the local paediatric department and the paediatric surgical service in Aberdeen. An understanding of the challenges faced in delivering a modern surgical service in a largely rural, and often remote, region is desirable. The successful applicant will be encouraged to develop a Supporting Professional interest that is of value to the wider service, and an interest in medical education would be highly desirable.

NHS Highland is currently undertaking a review of surgical services across the North of Scotland. It is anticipated that there will be changes to the ways in which surgical care is delivered across the various NHS Highland sites, with an emphasis placed upon improved networking and collaboration between the surgical teams. The surgical service based at Raigmore Hospital links directly with Caithness General Hospital, providing a weekly visiting elective service to the population of Caithness and Sutherland. This post specifically links into that programme by enhancing links with the Belford Hospital, but it is anticipated that there will be opportunities to work flexibly across this wider network if desired by the successful applicant.

Raigmore Hospital

Raigmore Hospital is the principal acute general hospital serving the population of NHS Highland (215,000, excluding the 90,000 in Argyll & Bute generally served by Glasgow hospitals). It also provides some tertiary services to NHS Western Isles (population (25,000) and some western parts of NHS Grampian area. It is supported by smaller hospitals in Fort William (the Belford Hospital), Wick (Caithness General Hospital) and Oban (the Lorn & Isles Hospital), each of which have Consultant staff including general surgeons. Raigmore is a modern, well-equipped, general hospital, and its relative geographical isolation demands levels of services greater than that seen in many general hospitals of comparable size. It has a total of over 400 beds, employs more than 2,500 staff and has an annual budget of approximately £130m. The hospital is one of Scotland’s five cancer centres. Raigmore is also the second campus for the Medical School and for the School of Nursing and Midwifery, and is the home of a unique, multi-agency Centre for Life Sciences. NHS Highland is investing widely in the redevelopment of the theatre suite and Critical Care facilities, with a new Intensive Care Unit, Surgical HDU and 2 re-designed theatres opened in late 2018. The theatre upgrade programme is due for completion in early 2020.

The Department benefits from the services of Specialist Nurses in Colorectal/ Stoma Care (three), Vascular (one), Urology (two), Breast Care (two) and Upper and Lower GI Endoscopy (two shared with gastroenterology – vide infra).

Visiting Consultants from Aberdeen and Edinburgh hold clinics in Neurosurgery, Cardiothoracic surgery, Plastic Surgery and Clinical Genetics. The visiting Plastic Surgeons also carry out elective operating lists at Raigmore Hospital.

The Department of Surgery

Upper GI / General Surgery The full range of oesophago-gastric and hepato-pancreato-biliary surgery is currently provided by Mr Ron Coggins, Mr Appou Tamijmarane and Mr Stephen McNally, with junior medical staff as detailed above. Raigmore offers an onward referral service to Fort William, Caithness and Western Isles for patients with benign and malignant disease. This includes resectional surgery for oesophageal, gastric and pancreato-biliary cancer. Hepatic resection for both metastatic colorectal cancer and primary liver cancer has been successfully introduced since 2005. Facilities also exist for radiofrequency ablation. In addition, Miss Hogg and Mr Grant provide a General Surgery service that dovetails with the subspecialty UGI surgical service. The UGI unit plays an important role in the delivery of benign general surgery for Inverness and surrounding areas, as well as for onward referrals from the peripheral units, and the management of routine gallbladder disease and abdominal herniae contribute significantly to the overall workload. The unit is active in the management of benign UGI disease, and the majority of benign cases are managed laparoscopically, including fundoplication, splenectomy and bile duct exploration. Laparoscopy has also been successfully introduced into the surgical management of patients undergoing oesophagectomy and liver resection.

The Upper GI service is supported by excellent diagnostic and interventional radiology, critical care services, oncology and medical gastroenterology, and all cancer cases are discussed weekly at a GI MDT. There are currently two ERCP lists weekly. There is active participation in the North of Scotland Cancer Network (NOSCAN) upper GI managed clinical network. There is a Scottish National MCN for HPB cancer.

Coloproctology Services Specialist colorectal surgical services are at present provided by five consultant surgeons, Mr Raymond Oliphant, Mr Kenneth Walker, Professor Angus Watson, Mr Mike Lim and Mr Mike Walker, with junior medical staff as above.

In the surgery of colorectal cancer, all patients with rectal cancer in Raigmore, and most of those with colon cancer, come under the colorectal unit’s care. Laparoscopic colectomy is established in the unit. A multidisciplinary team meets weekly to discuss all GI cancer patients, and comprises surgeons, radiologists, pathologists & oncologists (each with GI interests), gastro-enterologists and other support staff. It includes clinicians from all three acute hospitals in NHS Highland, and from Stornoway, usually via video-conferencing. Liver resections for colorectal metastases are performed in Raigmore Hospital by the Upper GI / HPB surgeons. The unit participates in the North of Scotland Cancer Network.

The surgery of inflammatory bowel disease (including ileo-anal pouches) is relatively common, and most other benign and functional disorders are managed in house. A specialist physiotherapist with an interest in continence is in post, and is developing a dynamic ultrasound biofeedback service. Anorectal physiology measurement is undertaken by the Dept of Medical Physics.

There are three colorectal/stoma nurse specialists, who also provide both hospital and community- based services, and two nurse colonoscopists. Most of the fast-track rectal bleeding clinics and colorectal cancer follow-up clinics are now nurse-provided.

Vascular Surgery The vascular service is currently delivered by three Consultant Vascular Surgeons supported by a Vascular Nurse Specialist, middle grade staff and foundation doctors. At present one surgeon has general surgical responsibilities as well as vascular surgery.

The department offers a full range of vascular surgical services. Screening for adnominal aortic aneurysm has been in place for ten years and all men in Highland (excluding ) between the ages of 65 and 84 have been offered screening. Approximately 40 abdominal aortic aneurysm repairs are undertaken each year with 40% of these by EVAR. The department works closely with the stroke physicians and 50 carotid endarterectomies are performed annually. The hospital supports a significant sized renal dialysis unit and the department provides access surgery for these patients. A full range of interventional radiological procedures are undertaken with approximately 250 lower limb angioplasty procedures each year. The department has had an established amputee rehab team for 20 years and limb fitting is on site.

The Vascular Department works as an integrated unit within the Department of General Surgery and Urology. At present there are three general surgical/urology wards and a six bedded high dependency unit. All emergencies are admitted to one area for triage and initial management.

Breast Surgery The breast service is currently delivered by three Consultant Breast Surgeons, supported in the out-patient department by a breast physician and by two full time breast care nurses.

The breast screening and out-patient services are accommodated in a new, purpose-built department which was opened in 2004. There are extensive waiting and changing areas, two mammography suites, two ultrasound rooms, prosthesis fitting and counselling rooms and five out- patient consulting rooms. In addition to the breast screening administration and the medical secretaries, the Breast Centre has its own staff of radiographers and clinic nurses.

Supporting Departments

There are excellent Radiology and Nuclear Medicine Departments that offer a wide range of imaging including MR, state-of-the-art volume acquisition CT scanning, ultrasound, angiography, and radionuclide scanning. These services are provided largely by consultants, each of whom have subspecialty interests. The range of radiological interventions includes image-guided aspiration, drainage and biopsy; nephrostomy and antegrade stenting; angioplasty, arterial stenting and embolisation; biliary stenting and colorectal stenting.

The department enjoys good support from the Histopathology Department, which has five consultants, and provides a very comprehensive and authoritative service.

The Department of Clinical Oncology provides a non-surgical cancer service to the population of Highland, Western Isles and parts of Grampian NHS Board areas. The Radiotherapy Department has recently been completely re-equipped with modern state of the art radiotherapy equipment including a new linear accelerator, new simulator and new CT planning facilities. Inpatients are accommodated in an Oncology ward within Raigmore Hospital. Outpatients are seen in a purpose- built day case chemotherapy facility. Chemotherapy is also administered by trained nurses in Wick, Fort William and Stornoway under the supervision of the four oncologists based at Raigmore.

Palliative Care is provided by a team led by two consultants based at the Highland Hospice in Inverness, where there is an inpatient unit of ten beds and a Day Hospice. The palliative care advisory service extends throughout the Highland NHS Board Area. Within Raigmore Hospital it is served by three Macmillan nurses working exclusively with inpatients.

Medical Education and Other Facilities

Raigmore Hospital is the second campus for the University of Aberdeen’s Medical School. The Centre for Health Science houses the Highland Medical Education Centre headed by the Director of Teaching, Professor John Duncan. At any one time NHS Highland hosts 25% of the Clinical Medical Students in the University of Aberdeen.

Within the Department of General Surgery fourth and fifth year students are attached to various sub-specialties throughout the year. They attend out-patient clinics, MDTs, ward rounds and theatre as well as formal tutorials. Final year students are attached for 32 weeks per year and 4th year for 45 weeks per year. The Department wishes to develop its Undergraduate Teaching and recent funding should enable that to take place. The post holder will be expected to take a share in the teaching responsibility. This post also includes the opportunity to be involved in University examinations at all stages of the clinical course. The successful candidate is likely to have experience in Undergraduate Teaching and an educational qualification would be desirable.

Inverness is also a centre of excellence in postgraduate medical education, and the General Surgery Department has received particularly good feedback from its Specialty Trainees on the North of Scotland Programme. NHS Education for Scotland’s North deanery has an office in Inverness, and the Associate Postgraduate Dean for surgical specialties in the North Deanery is based in Inverness. Interest from the post-holder, in Educational Supervision and innovation, would be supported. A recent innovation is the Highland Surgical Boot Camp, which is for new Core trainees from around the UK and Ireland, and part of an emerging interest in Simulation in Surgical Training in the region.

Also on site is the Centre for Health Sciences, an innovative facility jointly funded by NHS Highland, Aberdeen University, Stirling University, the University of the Highlands and Islands, Lifescan, and Inverness and Nairn Enterprise. This includes a Clinical Skills Centre, library, lecture theatres and other venues, College of Nursing, dental training facilities and several research units.

The Centre for Rural Health – Research and Policy of the University of Aberdeen, established in 2000 under the Directorship of Professor David Godden, provides support, guidance and co- ordination to clinical research into healthcare delivery in remote and rural areas of Scotland.

Research, Development and Innovation

Over the last five years the department has invested heavily in expanding its research activities. Two new chairs in surgery have been created (AJMW & KGW) and three clinical research fellow posts have been instituted. These activities are supported by a rapidly expanding Research, Development and Innovation (RD&I) department. There are dedicated clinical research facilities and a wide range of research laboratories which include immunology, tissue culture, genetics and mass spectrometry. The department recruits widely into a range of clinical trials including CRIB, CREST, FIAT, ROCCS, HuBBLE, ETHOS, FOXTROT and ARISTOTLE. The department is fully supportive of colleagues who wish to develop and lead their own research theme. There are 13 research nurses that provide trial support. The cancer centre contributes to over 80 oncology trials. The department has recently supervised eight postgraduate degree students. We are home to the Scottish Surgical Boot Camp and play a leading role in the Scottish Surgical Simulation Collaborative. Particular research interests include.

The Highland Clinical Research Facility The UHI Millennium Institute’s (UHI) Clinical Research Facility, which opened in January 2009, is housed on the ground floor within the Highland Diabetes Institute, Phase III of the new Centre for Health Science building adjacent to the Raigmore Hospital site.

The Clinical Research Facility is a joint NHS/UHI initiative aiming to provide a high quality clinical environment in which participants can take part in research programmes safely according to ethically approved study protocols. It comprises a bedded clinical research unit in which patient monitoring may be conducted on a day case or 24 hour basis. The facility has the capacity to be used by departments or specialties within NHS Highland, UHI or other research institutions for clinical research.

The facility is supported by the CRF Advisory Group and approved studies are reviewed by the CRF User’s Group.

It is staffed by a small team comprising a dedicated manager, research nurses, research pharmacist, part-time pharmacy technician and administrative support under the leadership of a part-time CRF Director, Prof Sandra MacRury.

Duties Medical Negligence NHS Highland takes responsibility for expenses and damages arising from medical negligence where they, as the employer, are vicariously liable for the acts and omissions of their medical and dental staff. However, the appointee is strongly advised to maintain separate medical defence or insurance cover for all work which does not fall within the scope of the Board's indemnity scheme, details of which are given in NHS Circular 1989(PCS) 32. Medical Fitness All prospective members of staff are asked to submit a confidential health questionnaire to the Occupational Health Service. If your badge needs replacing for any reason you are required to contact the Fire/Security Office, Estates Department to arrange for a replacement.
